Abstract
Das Injektionsanästhesiemittel enthält eine hydrophile Flüssigkeit mit einem lipophilen Gas. Argon, Krypton, Xenon, Lachgas oder Gemische von mindestens zwei dieser Gase werden als lipophiles Gas eingesetzt. Das lipophile Gas ist vorzugsweise in Form von Mikrogasbläschen in der Flüssigkeit enthalten. Als hydrophile Flüssigkeit dienen beispielsweise Proteinlösungen oder Polysaccharidllösungen.

injektionsanästhesiemittel mit hydrophiler Phase
Die Erfindung betrifft Anästhesiemittel zur Verabreichung durch Injektion, die ein Edelgas enthalten, und Geräte zur Applikation dieser Anästhesiemittel.
Zu den Anästhesiemitteln zählen Inhalationsnarkotika, z. B. Diethylether, Lachgas, Cyclopropan und Halothan, und Injektionsnarkotika, z. B. Ketamin, Barbiturate.
Xenon wird schon seit vielen Jahren als Inhalationsanästhesiemittel eingesetzt. Gegenüber dem heute üblichen Lachgas (N2O) bieten sich eine Reihe von medizinischen Vorteilen. Der breiten Einführung von Xenon für- die Anästhesie standen jedoch bisher die sehr viel höheren Stoffkosten entgegen.
Bei der Anwendung von Xenon als Inhalationsanästhetikum sind große Mengen für eine narkotische Wirkung erforderlich. Bei einer zweistündigen Operation werden z. B. minimal 15 Liter Xenon im geschlossenen Beatmungssystem verbraucht. Die Konzentration von Xenon in einem Atemgas für die Narkose beträgt maximal 79 Vol.-% um einen
Sauerstoffgehalt von mindestens 21 Vol.-% zu gewährleisten. Es wird eine Anästhesie erreicht, die aber allein nicht ausreicht. Es müssen zusätzliche Beruhigungsmittel (Sedativa), intravenöse Anästhetika und Analgetika ergänzend verabreicht werden. Bei bestimmten Eingriffen müssen Muskelrelaxanzien zusätzlich eingesetzt werden.
Die bekannten Injektionsanästhesiemittel haben eine Reihe von Nachteilen. So haben diese Mittel nur eine geringe schmerzhemmende Wirkung, werden im Körper nur allmählich abgebaut oder über Leber und Niere ausgeschieden, haben in der Regel unerwünschte Nebenwirkungen und sind schlecht steuerbar. Zur intravenösen Anästhesie müssen bisher immer Wirkstoffkombinationen eingesetzt.
Der Erfindung liegt die Aufgabe zugrunde, ein Mittel für die Anästhesie bereitzustellen, das die genannten Nachteile nicht aufweist.
Überraschend wurde gefunden, daß bei einer direkten Verabreichung von flüssigen Präparationen, die ein lipophiles Gas wie Xenon in einer hydrophilen, vorzugsweise viskosen Flüssigkeit enthalten, in die Blutbahn von Säugetieren, insbesondere in die Blutbahn des Menschen, eine Anästhesie eingeleitet und unterhalten werden kann, wobei schon geringe Mengen von lipophilem Gas, insbesondere Xenon, eine deutliche Anästhesie und Analgesie bewirken.
Gegenstand der Erfindung ist somit ein Injektionsanästhesiemittel, das eine hydrophile Flüssigkeit mit einem lipohilen Gas enthält.
Das lipophile Gas ist ein reines Gas oder ein Gasgemisch, das ein oder mehrere lipophile Gase enthält. Lipophile Gase sind beispielsweise Lachgas (N20) oder lipophile Edelgase wie Argon, Krypton und Xenon.
Hydrophile Flüssigkeiten sind beispielsweise Proteinlösungen, Polysaccharidlösungen oder Gemische, die vorzugsweise viskos sind.
Die lipophilen Gase sind in der hydrophilen Flüssigkeit dispergiert und/oder gelöst. Vorzugsweise sind die lipophilen Gase als Mikrobläschen in der hydrophilen Flüssigkeit enthalten.
Als Bestandteil eines Injektionsanästhesiemittels eignen sich vorzugsweise Argon, Krypton oder Xenon, insbesondere Xenon.
Das Injektionsanästhesiemittel enthält vorzugsweise keine lipophile Flüssigkeit.
Die Flüssigkeiten enthalten z. B. Glykole oder Polyglykole wie Polyethylenglykol (PEG). Die Polyglykole haben in der Regel ein Molekulargewicht unter 100000 dalton, vorzugsweise unter 40000 dalton.
Zur Applikation werden vorteilhaft in sterile wäßrige Systeme mit Zusätzen zur Einstellung der Viskosität, Oberflächenspannung, pH-Wert und osmotischem Druck eingesetzt. Als hydrophile Flüssigkeit eignet sich beispielsweise ein wäßriges System mit folgender Zusammensetzung: 1 % Gelatinelösung, 1 % Albuminlösung, 10 % Glycerinlösung, 15 % Propylenglykollösung, 1 % Methylcellulose, 1-2 % Dextranlösung, 1 % Agarlösung, 2 % Tweenlösung (Tween 80) und 1 % Gummi arabicum.
Im folgenden wird die Erfindung am Beispiel von Xenon und xenonhaltigen Gasen erläutert, woraus keine Beschränkung auf Xenon oder xenonhaltige Gase abzuleiten ist und wobei die Angaben auf lipophile Edelgase übertragen werden können.
Das eingesetzte Xenon-Gas hat im allgemeinen die natürliche Isotopenzusammensetzung. Die Isotopenzusammensetzung des Xenons kann sich von der natürlichen Isotopenzusammensetzung unterscheiden. Das Xenon-Gas wird vorzugsweise in hoher Reinheit, wie für medizinische Gase üblich, eingesetzt. Das Xenon-Gas dient als reines Gas oder im Gemisch mit anderen Gasen in dem Injektionsanästhesiemittel oder Injektionsnarkosemittel. Im Unterschied zu Xenon als Inhalations- narkosemittel, das immer im Gemisch mit Sauerstoff eingesetzt werden muß, kann das Injektionsnarkosemittel mit dem reinen Gas in die Blutbahn appliziert werden.
Xenonhaltige Gase sind Gasgemische die Xenon enthalten. Vorzugsweise enthalten die Gasgemische mehr als 50 Vol.-% Xenon, besonders bevorzugt mehr als 70 Vol.-% Xenon, insbesondere mehr als 80 Vol.-% Xenon. Die
Gasgemische können neben Xenon ein oder mehrere Gase oder bei Körpertemperatur und Normaldruck gasförmige Stoffe enthalten.
Zur Herstellung von Injektionsanästhetika mit kleinen Gasblasen, insbesondere Mikrobläschen, aus Xenon oder xenonhaltigem Gas in einer Flüssigkeit werden vorzugsweise Flüssigkeiten mit kleiner oder geringer Lipophilie verwendet, beispielsweise dickflüssige, vorzugsweise wäßrige Lösungen von makromolekularen Substanzen wie Polysacchariden, Peptiden oder Proteinen. Die Herstellung erfolgt vorteilhaft durch teilweise oder vollständige Lösung von Xenon oder xenonhaltigem Gas in der Flüssigkeit unter hyperbaren Bedingungen, das heißt unter Druck bei einer vorzugsweise abgesenkten Temperatur (z. B. 10 °C bis minus 10 °C, vorzugsweise um 0 °C). Bei einer Entspannung des Drucks kommt es zur Ausbildung von kleinen und kleinsten Gasblasen. Die Flüssigkeiten enthalten in der Regel Zusätze, die die Flüssigkeiten körperverträglich machen. Flüssige Injektionsanästhetika lassen sich beispielsweise mittels peristaltischer Pumpen oder konventioneller Infusionspumpen dosiert verabreichen, wobei der Xenon-Gehalt und/oder die Infusionsrate, vorzugsweise gesteuert, variiert werden können. Die Verabreichung der flüssigen Injektionsanästhetika kann auch einfach mittels Infusionsbeutel/ Infusionsleitung/Katheter oder mittels einer Spritze in die Blutbahn erfolgen. Die Abgabe des flüssigen Injektionsanästhetikums aus einem Infusionsbeutel kann beispielsweise anhand eines Regelventils in der Schlauchleitung und einer Steuerung gesteuert werden.
Das Injektionsanästhesiemittel kann dem Blut auch außerhalb des Körpers zugeführt werden und das mit dem Injektionsanästhesiemittel beladene Blut in den Körper injiziert oder infundiert werden.
Das in den Körper injizierte Xenon wird nach derzeitigen Erkenntnissen über die Lunge ausgeatmet. Besonders vorteilhaft wird das ausgeatmete Gas (Exspirationsgas) des Patienten während der Narkose analysiert und der
gemessene Xenon-Gehalt im Exspirationsgas oder ein entsprechendes Meßsignal zur Überwachung der Narkose oder zur Steuerung der Dosierung von Injektionsanästhesiemittel oder Injektionsnarkosemittelgenutzt werden.
Vorteilhaft wird die Zusammensetzung des Exspirationsgases mit Hilfe eines Analysegerätes wie einem Massenspektrometer online verfolgt, wobei das Meßsignal des Massenspektrometers, das dem Xenon-Gehalt oder dem Gehalt eines anderen Gases im Exspirationsgas entspricht, einem Steuergerät zugeführt wird, das die Dosierung des Injektionsanästhesie- mittels mittels des Gasdosiergerätes oder eines anderen Dosiergerätes steuert. Der Einsatz des Massenspektrometers und die Steuerung der Anästhesiemittelmittelzufuhr erfolgt beispielsweise analog zu dem für die • Inhalationsnarkose in WO 97/20591 (interne Bezeichnung MG 1970) beschriebenen Verfahren, worauf hiermit Bezug genommen wird.
Die Verabreichung des Injektionsanästhesiemittels kann in Kombination mit einem oder mehreren zusätzlichen pharmakologisch wirksamen Mitteln erfolgen, und zwar als Bestandteil des Injektionsanästhesiemittels und/oder durch getrennte Verabreichung von Injektionsanästhesiemittel und pharmakologisch wirksamem Mittel.
Es wurde gefunden, daß die flüssigen Präparationen mit vorzugsweise Xenon oder xenonhaltigen Gasen sedierend, analgetisch, antiinflammatorisch und muskelrelaxierend sind. Demgemäß können insbesondere xenonhaltige flüssige Präparationen als
Injektionssedierungsmittel (Sedativum), als Injektionsanalgetikum, als Injektionsantiinflammationsmittel oder als Injektionsmuskelrelaxans eingesetzt werden. Die Mittel werden in pharmakologisch wirksamer Menge verabreicht.
Ein weiterer Gegenstand der Erfindung ist somit ein Arzneimittel, enthaltend eine hydrophile Flüssigkeit und ein lipophiles Gas.
lm Gegensatz zu einer Inhalationsanästhesie mit Xenon kann mit xenonhaltigen Injektionsanästhesiemitteln eine komplette Anästhesie und Analgesie erzielt werden.
Claims
1. Injektionsanästhesiemittel, das eine hydrophile Flüssigkeit mit einem lipohilen Gas enthält.
2. Injektionsanästhesiemittel nach Anspruch 1 , dadurch gekennzeichnet, daß Argon, Krypton, Xenon, Lachgas oder Gemische von mindestens zwei dieser Gase als lipophiles Gas eingesetzt werden.
3. Injektionsanästhesiemittel nach nach Anspruch 1 oder, dadurch gekennzeichnet, daß das lipophiles Gas in Form von Mikrogasbläschen in der Flüssigkeit enthalten ist.
4. Injektionsanästhesiemittel nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß als hydrophile Flüssigkeit eine Proteinlösung, eine
Polysaccharidlösung oder deren Gemisch eingesetzt wird.
5. Verwendung eines Injektionsanästhesiemittels nach einem der Ansprüche 1 bis 4 zur Narkose.
6. Verwendung von flüssigen Präparationen mit einem lipophilen Gas zur Herstellung eines Injektionsanästhetikums, Injektionssedativums, Injektionsanalgetikums, Injektionsantiinflammationsmittels oder Injektionsmuskelrelaxans.
7. Verwendung nach Anspruch 6, dadurch gekennzeichnet, daß Argon, Krypton, Xenon, Lachgas oder Gemische von mindestens zwei dieser Gase als lipophiles Gas eingesetzt werden.
