Definitions
- the present invention relates to a device for producing a continuous passive displacement generally called CPM, corresponding to the abbreviation of the phrase in English "Contin ous passive motion". Such a device is frequently used in physical and functional rehabilitation therapy for the articulated limbs of a subject.
- the present invention is particularly directed to a device capable of subjecting the lower limb of a person to imposed flexion-extension movements.
- a patient may not have sufficient muscle power or capacity to actively control the alternative flexion and extension of a limb. This is the reason why a method of rehabilitation has been advocated involving a passive displacement of the limb to be rehabilitated by the application of forces external to the limb.
- Such passive displacement can be carried out manually by experienced therapists, but this method requires real and good knowledge of the human anatomy and involves considerable physical expenditure.
- Such passive displacement for these reasons, is very often imposed on the member to be re-educated by means of a device, generally called a splint, capable of imposing on the member to be re-educated adapted cycles of movement in flexion-extension.
- the prior art has proposed a relatively large number of technical solutions for constructing such devices.
- One of the drawbacks of the CPM devices of the prior art lies in the fact that they are generally bulky when it is necessary to respect, for a lower limb splint which is composed of articulated segments of great length, a requirement of flexion-extension complete. Such congestion can be an obstacle to commissioning which involves placing such a device directly on the mattress of the subject's bed.
- Another drawback is due to the difficulty, if not most often the impossibility, of being able to make the adjustment of the proximal articulation axis of the device coincide with the coxofemoral articulation of the limb to be reeducated, and this for all the anatomical conformations and for all lengths of limbs that may be encountered.
- the application without induced residual physical constraint, of rehabilitative passive movements imposed on the member to be rehabilitated is to eliminate the drawbacks mentioned above by proposing a CPM device which, moreover, can be easily adapted to a right or left member for which it offers the same faculty of anatomical adjustment.
- the device for producing a continuous passive movement for the flexion and extension of an articulated member of a person according to a predetermined movement cycle is characterized in that it comprises:
- a drive element capable of moving back and forth along a linear path defined as a basic element
- connecting element connecting said upper element to said driving element, said connecting element being mounted, so as to be able to pivot, both relative to said upper support element and to said driving element around axes which s 'extend essentially transversely to said path, and said lower member support member being adapted to be retained in a sliding engagement on said connecting member.
- - fig. 1 is a perspective view of the device for producing continuous passive displacement (CPM) according to the present invention
- FIG. 2 is a perspective view of the proximal end of the CPM device according to the present invention, which shows the fixing element articulated in the middle position for its transport and its storage, and
- a CPM device designated as a whole by the reference 10 comprises a base 20 provided with a distal end 21 and a proximal end 25 and on which is mounted a member 30 for supporting a limb .
- the support element 30 is linked to the base 20, and more particularly to the distal end 25, by an articulated fixing element 35 removably fixed to said end by an extension element 55.
- the fastening element 35 is mounted on the distal end 25 by means of an adjusting nut 36 cooperating with a slide 37 presented by the end 25 to extend over the entire transverse width of the base 20. From this In this way, the element 35 can be arranged on the right side, as shown in FIG. 1, on the left side or in the middle of the base 20, as shown in FIG. 3.
- the CPM device is therefore suitable for being used for a left limb, a right limb or else for its storage and transport by immobilizing the fastening element 35 in a suitable position by means of the nut 36. Immobilizing the element fixing 35 in the middle of the slide 37 as shown in FIG. 2, makes it possible to fold the CPM device in the form of a unit having smaller dimensions.
- the support element 30 is formed by an articulated structure comprising upper and lower member support elements.
- the articulated structure is, in fact mainly constituted by a crural segment or cradle 31 and by a leg segment or cradle 32 which are arranged in the extension of one another by being articulated together on an axis xx 'transverse to the general management occupied by the articulated structure.
- the crural segment 31 is composed of two proximal bars 65a and 65b, joined together by a spacer 85 able to be fitted with a comfort hammock for supporting the thigh of a lower limb to be rehabilitated.
- the bars 65a and 65b are advantageously tubular and provided with locking buttons 60a and 60b making it possible to link either one or the other of these bars to an extension extension 55, itself fixed to the element 35 by a pivot 40.
- the extension 55 is, preferably telescopic and adjustable in length by means of an adjustment member 50.
- the leg segment 32 is composed of two bars 90a and 90b joined together by a spacer, not visible in the drawing, allowing the mounting of a comfort hammock for the support of the leg of the member to be rehabilitated.
- the bars 90a and 90b carry, at the distal end, a footrest 100, preferably fixed removably and adjustable by means of buttons 75a and 75b.
- the bars 90a and 90b are linked to the bars 65a and 65b by pivots 80a and 80b which materialize the axis x-x '.
- the bars 90a and 90b are provided with extensions 130a and 130b which extend towards the base and which are provided with sockets, rings or the like 72a and 72b which are capable of sliding on support bars 70a and 70b whose proximal ends are articulated on the bars 65a and 65b by pivots 82a and 82b which are located between the axis xx 'and the proximal ends of said bars.
- the distal ends of the support bars 70a and 70b are mounted by pivots 120a and 120b on a drive element 125, such as a carriage, forming part of a drive mechanism carried by the base 20.
- This mechanism comprises a screw 115 mounted on the base 20 and cooperating with the carriage and two guides 110a, 110b also mounted on the base 20 to support and guide the carriage.
- the screw 115 is connected, by means of a flexible coupling, to a geared motor 105, preferably carried by the distal end 21 of the base 20.
- the rotary drive of the screw 115 by the motor leads to a linear displacement of the drive carriage 125 along the guides 110a, 110b, which causes, by means of the support bars 70a and 70b, the movement of flexion-extension of the articulated structure 30 which imposes such a movement on the member that it supports by the segments 31 and 32.
- the mounting of the leg segment on the support bars 70 involves for each bar 90 a kind deformable quadrilateral comprising a sliding point constituted by the corresponding socket 72.
- Such an arrangement introduces into the movement a multiplier effect which makes it possible to obtain a flexion-extension movement of the articulated structure 30 with a reduced useful stroke of the carriage 125.
- Such an arrangement is therefore favorable to a reduction in size, in particular of the base 20.
- Such an arrangement thus makes it possible to implement a motor member 105 of more power low, which further reduces the size and the mass of the device.
- An electronic control unit 140 (fig. 3) is used to preset the extension limit, the bending limit and the speed of movement.
- a copy potentiometer located on the pivot 80b closes the control loop.
- Another advantage of the construction of the device is due to the mounting of the crural segment on the base 20 by means of an extension 55. This makes it possible to use shorter bars 65a and 65b offering greater adjustment adjustment for short femurs.
- the lateralized link to the right or to the left by the element 35 makes it possible to eliminate any functional interference between the base 20 and the thigh of the subject, regardless of the right or left limb mobilized, since the element 35 can be fixed , indifferently, on the right or on the left.
- extension 55 is telescopic and adjustable, which makes it possible to exactly adjust the length of the crural segment between the axis xx 'and the pivot 40 and, consequently, to adapt at least one such adjustment. to make this pivot coincide with the coxofemoral or acetabular joint of the lower limb to be rehabilitated.
- the fixing element 35 is permanently fixed to the base.
- the extension bar 55 is separated from the proximal bar 65a, the fastening element 35 is loosened to allow sliding on the slide 37 and a new fixing is ensured on the left side. This adaptability is particularly appropriate if the CPM device is installed in the patient.
- proximal bars and the lower limb support bars can be deployed telescopically, thus offering a wide range of adjustments atypically, for patients ranging in size from 112 cm to 206 cm. . LIST OF REFERENCES
- the invention finds a particularly advantageous application for the construction of a mobilization splint reserved for a rehabilitation treatment at home.
