Description
Für die Anwendung beim Menschen geeignete Fibronektin- lösung und Verfahren zu ihrer Herstellung
Die Erfindung betrifft eine für die Anwendung am Men¬ schen geeignete Fibronektinlösung und ein Verfahren zu ihrer Herstellung.
Fibronektin ist ein hochmolekulares Glykoprotein,wel¬ ches aus zwei Untereinheiten mit einem Molekulargewicht von jeweils etwa 220.000 besteht und das in löslicher Form im Plasma in einer Konzentration von etwa 0,3 mg/ml vor- kommt. In unlöslicher Form findet man Fibronektin im Binde¬ gewebe und mit der Basal embran assoziiert. Lösliches Fibro¬ nektin tritt mit Kollagen. Heparin, Fibrin und Staphylo- kokken in Wechselwirkung, wobei es eine Funktion in der Ver¬ mittlung von Adhäsion von Zellen,wie z.B. Thrombozyten, der Organisation von Fibringerinnseln und der Bildung von Binde¬ gewebe, insbesondere aber auch als einer der hauptsächlich opsonisierenden Faktoren bei der Phagozytosefunktion des re- tikuloendothelialen Systems besitzt.
Fibronektin wird durch aktivierten Faktor XIII mit Fibrin quervernetzt und bewirkt die Verfestigung des Fibringerinn¬ sels, wobei es in diesem Zusammenhang Fibroblasten anlockt. Entsprechend seiner Inkorporation in das Fibringerinnsel ist die Serumkonzentration von Fibrin um etwa 20-50% geringer als diejenige des Plasmas. Klinisch von besonderer Bedeutung ist die Funktion von Fibronektin als Opsonin, und es gibt deutliche Hinweise, daß für die normale Funktion des retikulo-endothelialen Systems in Milz, Leber und" Knochenmark Fibronektin erforderlich ist. So findet man in tierexperimentellen Untersuchungen, daß die Phagozytosekapazität nach artifizieller Depletierung von
Fibronektin massiv unterdrückt ist. Weiters vermittelt Fibro¬ nektin die Bindung von Fibrin an Makrophagen und steigert die Phagozytose von Streptokokken. Bei Patienten findet man ei¬ nen verminderten Fibronektinspiegel nach größeren Operatio¬ nen oder Traumata und bei fortgeschrittenen malignen Er-
krankungen. Die Verminderung von Fibronektin könnte weiters mit der. Entstehung der "Schocklunge" bei Sepsis in Zusam¬ menhang stehen. Bei Patienten an Intensivstationen fand man, daß höhere Plasmafibronektin-Spiegel bei der Einwei- sung mit einer signifikant geringeren Mortalität korreliert werden konnten und daß Patienten mit Fibronektinspiegeln, die während des Aufenthaltes unter 0,195 mg/ml absanken, eine um 65% höhere Mortalität aufwiesen als solche Patien¬ ten, bei denen während des Aufenthaltes der Fibronektin- spiegel nicht unter 0,195 mg/ml absank.
Fibronektin ist in Frischplasma, frisch gefrorenem Plas¬ ma, antihämophilem Plasma und Kryopräzipitaten enthalten und wurde therapeutisch in Form von Kryopräzipitat ange¬ wandt, wobei zum Teil dramatische Verbesserungen bei'Patien- ten nach Traumata, insbesondere hinsichtlich der kardio-pul- monalen Funktion gefunden wurden. Diese Verabreichungsformen von Fibronektin haben jedoch den Nachteil des relativ großen Volumens und der Tatsache, daß diese Darreichungsformen nicht durch Hitzebehandlung analog zu dem hepatitis-sicheren Albumin behandelt werden können.
Aufgabe der Erfindung ist es, ein Fibronektinpräparat in Form einer Lösung zu schaffen, das virussiche-r und daher unbeschränkt verabreichbar ist. Zur Lösung dieser Aufgabe wurde erfindungsgemäß eine Fibro- nektinlösung geschaffen, die durch Hitzeinaktivieι?ung virus¬ sicher gamachtes Fibronektin enthält.
Das erfindungsgemäße Verfahren zur Herstellung dieser Lösung besteht darin, daß man Fibronektin enthaltendes Aus¬ gangsmaterial in Form von Kryopräzipitat, fresh frozen Plas- ma, outdated Plasma, kryopräzipitatarmem Plasma oder fibro- nektinhältigen, humanen Plasmafraktionen, z.B. mit organischen Fällungsmitteln, wie Polyäthylenglykol, Alkoholen, Äther, oder mit anorganischen Fällungsmitteln, wie Ammonsulfat, Natriumsulfat, Natriumchlorid, gefälltem Plasma, oder chro- matographisch hergestellten fibronektinhältigen Plasmafrak¬ tionen, wie z.B. Ionenaustauschchro atographie oder Affini¬ tätschromatographie an Gelatine-, Heparin- oder Arginin— sepharose, Fraktionierungsstufen unterwirft, em so erhalte¬ nen nativen Fibronek inpräparat mindestens einen Stabili-
sator zusetzt, es einer Hitzebehandlung mindestens bei
50°C zur Trennung des Fibronektins und hierauf einer neuerlichen Pasteurisierung bei mindestens 60°C während mindestens 10h unterwirft, daß man den dabei erhaltenen Fibronektin enthaltenden überstand einengt und von Elek¬ trolyten und niedermolekularen Peptiden befreit und daß man die so erhaltene Fibronektinlösung unter Zusatz weite¬ rerLösungsmittel wie Albumin, Plasmaproteinlösung zu einem intravenös verabreichbaren Präparat für den Menschen ver- arbeitet.
Im Gegensatz zu den bisher bekannten Darreichungsformen stellt die erfindungsgemäße Fibronektinlösung eine hitzein¬ aktivierte Fibronektinpräparation dar, welche z.B.2mg/ml Fibronektin in seiner dimeren Form, gelöst in einer 5%- igen Plasmaproteinlösung enthält. Trotz Hitzeinaktivierung besitzt dieses Präparat seine volle Funktion hinsichtlich Bindung an Kollagen und Fibrin, der Vermittlung des Haftens von Makrophagen und der Steigerung der Phagozytoserate von Makrophagen. Sowohl die Haftung von Makrophagen wie auch die Phagozytoserate wird durch das erfindungsgemäße Präparat dosisabhängig beeinflußt, wobei bei einer Konzentration von 100 j.g/ml eine Steigerung der Haftung um etwa 90% und eine Steigerung der Phagozytoserate um mehr als 70% erhal¬ ten wird. In vorklinischen Untersuchungen war dieses Prä- parat bei fünf Patienten in der höchsten getesteten Konzen¬ tration von 1 ml/kg mit einer Infusionsgeschwindigkeit von 2 ml/min ohne Nebenwirkungen verträglich.
Die Erfindung wird durch das folgende Beispiel näher er¬ läutert: BEISPIEL: Humanes Plasma von gesunden Spendern wird mit ge¬ reinigtem Wasser auf eine Proteinkonzentration von 3,8% verdünnt und mit gesättigter Ammoniumsulfatlösung bis zu einer Sättigung von 25-28% gebracht. Der pH-Wert ist 6,3 oder 8,0. Der Ammoniumsulfat-Niederschlag wird bei Zimmertempera¬ tur in Natriumcitrat-Lösung, 0,0β Mol/Liter, aufgelöst und bei pH 7,6-8,0 mit der gleichen Menge gesättigter Natrium¬ chloridlösung versetzt. Der entstandene Niederschlag wird wieder bei Zimmertempera-
tur in 0,06 molarer Natriumcitratlösung aufgelöst, mit 0,02 Mol/Liter Natriumcaprylat und 0,02 Mol/Liter Natrium- N-acetyl-DL-tryptophanat bei einem pH-Wert von 7,6 - 8,0 stabilisiert. Sodann wird mit 50 g Glucose pro Liter Lösung versetzt. Es folgt eine 10-stündige Pasteurisation bei einer Temperatur von 57-βO°C.
Nach Abtrennen des entstandenen Niederschlages wird der klare fibronektinhaltige überstand diafiltriert, um die Elektrolyte und Peptide unter 20 000 Dalton zu entfernen. Anschließend wird mit demselben Ultrafilter das Fibro¬ nektin konzentriert. Das Fibronektin-Konzentrat wird zur klinischen Anwendung mit Plasma-Proteinlösung versetzt und mit 0,03 Mol/Liter Natriumcaprylat und mit 0,01 Mol/Liter Natrium-N-acetyl-DL-tryptophanat stabilisiert. Die Iso- tonie der Lösung wird durch Zugabe von Natriumchlorid erreicht (Na-Gehalt 130 - 160 mMol/Liter) . Der pH-Wert wird auf 7,0 +_ 0,3 eingestellt.
Das Endprodukt wird abgefüllt und in den entsprechenden Behältern 10 Stunden bei 6θ°C pasteurisiert. E-rfindungsgemäße fibronektinhaltige Plasma-Protein- Lösungen, wie sie zur Anwendung am Menschen geeignet sind, können beispielsweise folgende Zusammensetzungen haben, wobei die einzelnen Bestandteile in g/1 angegeben sind:

Claims
1. Für die Anwendung am Menschen geeignete Fibronektin¬ lösung, dadurch gekennzeichnet, daß sie durch Hitzeinakti- vierung virussicher gemachtes Fibronektin enthält.
2. Verfahren zur Herstellung der Fibronektinlösung nach Anspruch 1, dadurch gekennzeichnet, daß man Fibronektin enthaltendes Ausgangsmaterial in Form von Kryopräzipitat, fresh frozen Plasma, outdated Plasma, kryopräzipitatarmem Plasma oder fibronektinhältigen humanen Plasmafraktionen, oder von z.B. mit organischen Pällungsmitteln, wie Poly- äthylenglykol, Alkoholen, Äther, oder mit anorganischen Pällungsmitteln, wie Ammoniumsulfat, Natriumsulfat, Natrium¬ chlorid, gefälltem Plasma oder chromatographisch herge¬ stellten, fibronektinhältigen Plasmafraktionen (z.B. Ionenaustauscherchromatographie oder Affinitätschromato¬ graphie an Gelatine-, Heparin- oder Argininsepharose) Fraktionierungsstufen unterwirft, dem so erhaltenen nativen Fibronektinpräparat mindestens einen Stabilisator zusetzt, es einer Hitzebehandlung bei mindestens 50°C zur Trennung des Pibronektins und hierauf einer neuerlichen Pasteurisie¬ rung bei mindestens 6θ°C während mindestens 10 Stunden unterwirft, daß man den dabei erhaltenen Fibronektin enthal¬ tenden überstand einengt und von Elektrolyten und nieder¬ molekularen Peptiden befreit u'nd daß man die so erhaltene Fibronektinlösung unter Zusatz weiterer Lösungsmittel wie
Albumin, Plasmaproteinlösung zu einem intravenös verabreich¬ baren Präparat für. den Menschen verarbeite .
