Specification of Letters Patent.I
Application filed August 20,1917. Serial No. 187,074.
To all whom it may concern.'
Be it known that I, FREDERICK H. CLARK, a citizen of the United States, residing at Providence, in the county of Providence and State of Rhode Island, have invented certain new and useful Improvements in Container-Records, of which the following is a specification.
My invention relates to a device or means for conveniently identifying and recording the dispensing of narcotics or opiates.
A Federal enactment requires the physi; cian or other dispenser of drugs which contain a narcotic, to record the quantity of the narcotic in the drug dispensed on each occasion, as well as to preserve such record for at least two years. The time and labor thus involved when the physician personally dispenses a drug is excessive even if he should know the quantity of the narcotic in the composition without a search for the forml ula of it in a drug dealers catalogue.
Further delay is occasioned in identifying a composition in a well filled laboratory, and this is increased when the paper to receive the record is not at hand or misplaced.
The essential objects of my invention are to save the time of the dispenser; to insure an accurate record of every element of the composition containing the narcotic, of the quantity of Vthe composition dispensed, as well as the quantity of the narcotic, and of the date, name and address of the patient, and the dispensers name; to insure the instant availability of the paper to receive the record; to insure a secure and facile attachment of the same to the receptacle; and to attain these ends in an inexpensive and simple construction.

A container 5, which may be a boX, bottle, carton, or other convenient receptacle is in the present instance a bottle preferably having plane or fiat sides. In this container is a compound, either liquid or dry, containing as an element an opiate or narcotic.' In this instance the compound or composition is in the form of tablets 6. A strip of paper, fabric, or other flexible material 7 has upon its limer face an adhesive coating 8 such as dry mucilage so that when the latter is moistened the member may be applied to the bottle as a binding strip. This strip embraces the back board l0 of a pad 11, oblong in outline, and holds the board or strip tightly against the face of the bottle. The pad comprises in addition to the strip 10, a body portion of superposed oblong leaves 12 attached at one of their narrower ends by staples 13 or otherwise to the back strip, andl provided preferably near their tops with a transversely disposed series of piercings 14 to facilitate severance of the leaves along the line of piercings. The pad itself may be directly Xed to the receptacle if desired.
Upon the face of each leaf about a third of the length of the leaf downwardly from its top is a transverse line 16 inclosing an area 17 adapted to receive the data 18 constituting the formula of the composition of the tablets 6. The area below the line 16 has several longitudinally disposed lines for additional data, namely, a line 20 for reception of the date the sale was made, a line 21 for the name of the patient, a line 22 for his address, a line 23 for the name of the dispenser, and a line 24 for the quantity of tablets dispensed.
It will be noted that the formula 18 is permanently printed, as well as are the words, Date,7 Patient,. Address and Quantityf7 so that when one or more tablets are dispensed the operator has only to insert the obvious incidental data on lines 20, 21, 22, and 24: to have a complete record, and after such insertion the leaf is torn 0E and preserved.
