Abstract
Спосіб реконструкції шлунково-кишкового тракту при панкреатодуоденальній резекції шляхом видалення гастропанкреатодуоденального комплексу включає головку підшлункової залози, дванадцятипалу кишку, дистальні відділи шлунка, холедох із жовчним міхуром та регіональні лімфатичні вузли. При цьому проводять збереження гастроентероанастомозу шляхом відсічення шлунка детальніше останнього. В подальшому при виділенні жовчного міхура зону холецистоентероанастомозу зашивають у поперечному напрямку, таким чином зберігають всю початкову ділянку тонкої кишки, раніше використану для обхідних анастомозів. В подальшому на даній ділянці тонкої кишки формують гепатикоєюноанастомоз, який "відключають" від гастрентероанастомозу шляхом міжкишкового сполучення із заглушкою. Кишку, яка є привідною до гастроентероанастомозу від зони зв'язки Трейца, використовують для формування панкреатоєюноанастомозу. Для цього привідну кишку відсікають проксимальніше гастроентероанастомозу і формують класичний панкератоєюноанстомоз позаду ободової кишки за принципом "проток-із-слизовою". Subsequently, this gut is "included" in the gastrointestinal passage to the distal hepaticoynoanastomosis by the method of Ru.
