Abstract

Спосіб реконструкції шлунково-кишкового тракту при панкреатодуоденальній резекції включає видалення гастропанкреатодуоденального комплексу, який включає головку підшлункової залози, дванадцятипалу кишку, дистальні відділи шлунка, холедох із жовчним міхуром та регіональні лімфатичні вузли. Проводять збереження гастроентероанастомозу шляхом відсічення шлунка дистальніше останнього, а в подальшому при видаленні жовчного міхура, зону колецистоентероанастомозу зашивають у поперечному напрямку, таким чином зберігають всю початкову ділянку тонкої кишки, раніше використану для обхідних анастомозів. В подальшому на даній ділянці тонкої кишки формують гепатикоєюноанастомоз, який відключають від гастроентероанастомозу шляхом міжкишкового сполучення із заглушкою, кишку, яка є відвідною від гепатикоєюноанастомозу, розділяють на відстані дистальніше гепатикоєюноанастомозу. Таким чином гастроентеро- і гепатикоєюноанастомози опиняються на першій ізольованій петлі тонкої кишки, відповідно наступну (другу) петлю тонкої кишки використовують для формування панкреатоєюноанастомозу. Для цього другу петлю проводять позаду ободової кишки, формують класичний панкреатоєюноанстомоз за принципом "протока-із-слизовою", в подальшому першу петлю тонкої кишки "включають" в шлунково-кишковий пасаж на дистальніше панкреатоєюноанастомозу за методом Ру.The method of reconstruction of the gastrointestinal tract with pancreatoduodenal resection involves the removal of the gastro-pancreatoduodenal complex, which includes the head of the pancreas, duodenum, distal gastric, cholesterol, and regional lymphatics. Thus, the gastroentero- and hepaticoynoanastomoses are on the first isolated loop of the small intestine, respectively, the next (second) loop of the small intestine is used to form pancreatic anoanastomosis.
