Abstract

The machine-assisted method for gastrojejunoplasty following the subtotal resection for acute bleeding gastric cancer includes the distal subtotal resection of the stomach and gastrojejunoplasty. After the distal subtotal resection of the stomach, the proximal margin of dissection in stomach is generated throughout the gastric lumen up to esophagus with the aid of Proximat-100 apparatus. The purse-string suture is applied onto the distal stump of the stomach near the greater curvature. The segment of jejunum with the vascular arcade is tailored and its proximal part is sutured in two-barrel mode with Proximat-100 apparatus. The anastomosis between the proximal end of the cross-linked segment of jejunum and the stump of the stomach is fashioned with the aid of the circular stapler Ethicon 29 mm. The distal end of the segment of the small intestine is sutured with duodenum with the aid of the circular stapler Ethicon 25 mm. The opening in the segment of jejunum is sutured. The continuity of the jejunum is restored with circular staplers Ethicon 21 mm via the transverse enterotomy of the efferent limb. Finally, enterotomy is sewed with two-row suture.
