Description of the invention
The useful model relates to medicine, in particular, orthopedics and can be used for the treatment of bone 2 cavities in the lytic phase in dysplastic and other lesions.
Bone cavities arising, for example, in aneurysmal and solitary bone cysts, cause severe functional and anatomical disorders. Especially severe disorders occur at the first stage of these diseases - the lytic stage. Surgical treatment of such patients is complex and insufficiently developed. The problem consists in the progressive destruction of the bone with its cortical layers being actively involved in the process, as a result of which it loses its strength, and subsequent deformations occur, both axially and in shape.
There are known methods of treating bone cavities, which include introducing into it inhibitors of proteolysis |1) or a synthesized implant I(2|) by puncturing the cavity. However, in some cases, bone cavities have multi-chambered internal linings, which are combined with high activity of the process, which leads to the ineffectiveness of the puncture In addition, the disadvantages include the inability to actively influence 12 bone shape correction.
There is a known method of surgical treatment of dysplastic tumor-like diseases and benign bone tumors in children |Z), which consists in removing the pathological focus within healthy tissues (by bone resection) with plastic cortical allografts. The disadvantages of this method are the impossibility of restoring the normal shape of the bone with periosteum and the filling of bone defects of various shapes with hard cortical grafts.
The method of surgical treatment of dysplastic tumor-like diseases and benign bone tumors in children |4) is the closest to the proposed method, which consists in removing the pathological focus within healthy tissues and plasticizing the bone defect with ceramic hydroxylapatite granules.
However, when applying such an operation due to the destruction of the cortical layer, loss of strength and deformation (swelling) of the bone, the use of porous granules of ceramic hydroxylapatite does not allow restoring the bone to its normal shape, but fixes the existing deformation.
These factors make it difficult not only to perform the above surgical interventions, but also to achieve a positive final functional result.
The basis of a useful model is the task of improving the method of plasticity of bone cavities in the lytic phase of dysplastic and other lesions in children and adolescents, in which due to the combination of implants with different properties, for example, cortical allografts to create a framework of strength and porous granules of ceramic hydroxylapatite for plastic filling of defects, and placing internal sutures on a layer of thickened, swollen periosteum eliminate swelling of the bone, restore its shape. Ha
The task is solved by the fact that in the method of plasticity of bone cavities in children with the lytic phase of dysplastic diseases, which involves filling the cavities with implants, according to a useful model, implants with the possibilities of plastic filling of cavities and increased strength are combined, and internal sutures are applied to the periosteum.
In the lytic phase of dysplastic bone diseases, when the cortical layer is destroyed, the combination of implants with different properties creates stability and conditions for good bone regeneration. The imposition of 50 internal sutures on the thickened, enlarged (swollen) periosteum allows you to create adequate conditions for restoring the bone shape while preserving the periosteum, since it is not removed, and creates optimal conditions for reduction

Fig. 6 - 2 years after the operation. The proposed method is performed as follows. The patient is anesthetized, and if possible, a tourniquet is applied. Next, access is used: after the skin is cut, the bone is opened in the metaphyseal part, the pathological focus is removed within the healthy tissue while preserving the periosteum. Perform hemostasis. 29 First of all, internal sutures are placed on the thickened and enlarged soft periosteum, the defect is filled with cortical allografts, which allows creating a spacer between the bone fragments, stabilizing their relationship, after that the sutures are tightened and the bone shape is normalized by reducing swelling, and the remaining cavity is filled with porous ceramic granules hydroxylapatite, the tourniquet is removed, drainage is inserted, the wound is sutured in layers. Immobilization in the postoperative period with a plaster cast for the 60th period of the formation of cortical plates.

She was ill for three months before the appeal. On radiographs of the leg in the back and side projections, there is a focus of destruction in the upper third of the proximal end of the fibula. Clinical and radiological diagnosis: aneurysmal cyst of the fibula bone in the upper third. Conservative treatment was carried out for three months: unloading by walking with crutches, fixation with a plaster cast, puncture therapy with the introduction of proteolysis inhibitors. The control radiographs after 3 months revealed the progression of the pathological process with complete destruction of the cortical layers of the bone at the site of the lesion and swelling of the bone. The operation was performed - intraosseous resection of the pathological focus of the fibula in the upper third with plasticity of the defect according to the proposed method. Postoperative period without complications. In the future, the restoration of the shape of the bone, its uniform growth without disruption of the function of the germinal zone was observed. In the process of reconstruction, the restoration of the shape of the bone without swelling was noted. The observation period is up to 2 years. There was no shortening and deformation of the limb. According to the proposed method, 2 more patients were operated on. All received a positive 7/o result: restoration of bone shape with functioning growth zones, absence of limb deformations.
