Description
本文揭露的創新概念係概括有關一手術植入物。更特別來說,該等創新概念係有關一用於將軟組織、韌帶或腱附接至骨頭之手術植入物。本發明的實施例係有關一用於將軟組織、韌帶或腱附接至骨頭之方法,有關一用於植入根據本發明的一手術植入物至一患者的一身體部分內之工具,且有關一包含一工具及一或多個手術植入物之用於將軟組織、韌帶或腱附接至骨頭之套組。
本申請案係對於2010年11月23日提申的美國臨時申請案No.61/416,668作權利主張,該案整體合併於本文以供參考。
本文所揭露的創新概念之一目的係在於提供一最終將軟組織附裝至骨頭之前容許諸如一韌帶或一腱等軟組織被定位且固持於一骨頭的表面上之手術植入物。
本文揭露的創新概念係有關一手術植入物,其包含:一肘釘,其具有由一橋所連接之兩或更多個腳,
該肘釘包含一具有拉伸強度TS1之材料;及一板,其具有一上側及一下側,且其中肘釘可以橋沿著於板上方而固定至一骨頭。
一實施例中,肘釘的腳可穿過板的穿孔。板可因此藉由肘釘的腳而被穩固地固持於其側向位置中。
根據本文揭露的創新概念之手術植入物係容許軟組織藉由板之一暫時固定,且在軟組織的位置之一可能矯正或調適之後,可藉由將肘釘推押穿過經暫時固定的板或推押於其上方來進行最終固定。
根據本文揭露的創新概念之手術植入物係為一用於固定軟組織撕裂且容許更好的軟組織重新定位及更好的骨固定之簡單且多用途的植入物。
另一實施例中,板係包含一具有拉伸強度TS2<TS1之材料,故板可從其上側至其下側被肘釘的腳所穿孔。此組態係容許下列優點:板可建構為不具有穿孔且可在肘釘衝擊時從其上側至其下側被肘釘的腳所穿孔。
另一實施例中,板具有以與肘釘的腳相同的幾何結構所配置之一數量的盲孔或貫通穿孔。此組態係容許肘釘更容易衝擊穿過板。若板包含貫通穿孔,板可由與肘釘相同的材料所組成。若板包含盲孔,板可更容易被肘釘的腳所穿孔,故利於肘釘的插入。貫通穿孔或盲孔可具有小於或等於肘釘的腳橫剖面積之一橫剖面積,故防止板相對於肘釘作側向運動。或者,貫

含下列步驟:藉由壓抵一板以其下側抵住骨頭以將軟組織固定至骨頭表面;從自由上側衝擊一肘釘以其腳穿過板或位於板旁邊,故使腳在板下方被錨固於骨頭中,且軟組織被夾固於板與骨頭之間。
根據另一態樣,提供一用於植入一手術工具至一患者的一身體部分內之工具,其包含一縱向軸線且與之同軸地配置:一套筒,其具有一前端附有用於可釋放地對其固定一板之部件;一衝擊桿,其可滑地配置於套筒中;一衝擊塊,其被附裝至衝擊桿且可滑地配置於套筒的外周邊表面上並包含用於可釋放地對其固定一肘釘之部件,其方式係使得肘釘的腳被導引朝向套筒的前端。

現在參照圖式,且更特別參照圖1至3,顯示根據本文揭露的創新概念構成之一手術植入物10的一實施例。廣言之,手術植入物10包括一肘釘12及一板13。
肘釘12顯示成具有四個腳14,其由一可以一概呈正方形框架形狀所形成之橋16予以連接。然而,應瞭解腳14的數量可改變,橋16形狀亦然。譬如,可利用兩或更多個腳,且橋16可形成為具有任何幾何、非幾何、不對稱、或新奇設計。譬如,橋16可形成為一矩形,圓形(圖4),三角形,或八角形,或其他形式諸如星或心形。橋16具有一用於以下述方式接收一工具的一部分之中央開口17。腳14的各者係被附裝至橋16的一角落。腳14係彼此平行且從橋16的底側垂直地延伸。腳14的自由端可為尖化,且腳14可作圓柱形建構。各腳14可包含一三維結構,諸如一鋸齒18。一實施例中,鋸齒18在一面對橋16的外周邊之側位居各腳14上。並且,鋸齒18的陡峭側腹係被導引朝向各腳14的固定端。肘釘12可由任何適當的材料形成,諸如一具有拉伸強度TS1的652L不銹鋼。
一實施例中,板13可具有一概呈正方形形狀,其具有一上側22、一下側24、四個側向側26a、26b、26c與26d及一正交於下側24之中央軸線27。就像上文討論的肘釘12之橋16,板13可形成為具有多種不同形狀。範例中,圖4顯示一手術植入物10a的另一實施例。手術植入物10a係包括一肘釘12a及一板13a。肘釘12a具有複數個腳14a及一橋16a。橋16a及板13a在構造及功能上實質類似於橋16及板13,差異在於橋16a及板13a具有一概呈圓形形狀。
現在再參照圖1至3,板13的上側22具有一凹部28。板13的凹部28係建構且設定維度藉以對接式接收肘釘12的橋16。板13的下側24較佳設有一三維結構性表面,諸如複數個稜錐形齒30。板13具有一中央孔31,中央孔31係從上側22延伸穿過板13至下側24以下述方式接收一工具的一部分。
板13進一步設有用以接收肘釘12的腳14之複數個穿孔32。各穿孔32位居凹部28的一角落中,其建構為接收肘釘12的橋16。當肘釘12被植入時,肘釘12的腳14係被驅動穿過板13中的穿孔32且進入骨頭內直到橋16抵靠凹部28的表面為止,故肘釘12及板13皆被固定至骨頭。板13可由與肘釘12相同的材料組成,其在一實施例中係為一652L不銹鋼。
或者,板13可由一具有比肘釘12拉伸強度更小的拉伸強度TS2之材料構成。因此,圖3A顯示一板
13b的另一實施例。板13b類似於板13,差異在於板13b形成為不具有穿孔32。反之,板13b設有可在肘釘12衝擊時被肘釘12穿孔之複數個盲孔32b。
或者,板13可建構且設定維度使得手術植入物10固定至一骨頭之後令肘釘12包圍板13。肘釘12的腳14可側向地接觸板13或可對於板13的周邊具有一小間隔。另一實施例中,板13可設有四個切口或凹口以接收肘釘12的腳14,其中切口可配置於板13的角落或側中。
凹部28較佳係建構為匹配肘釘12的橋16,而容許板13與肘釘12之間的對接式接合。或者,穿孔32可具有在肘釘12插入時容許肘釘12的腳14可滑式穿透過穿孔32之直徑,故板13被肘釘12的腳14所固定以對抗相對於肘釘12的側向運動。如熟習該技術者將瞭解,板13可經由錨固於骨頭中的肘釘12而被側向地固定,若肘釘12的橋16匹配於板13中的凹部28、及/或腳14匹配於穿孔32則藉由一確實鎖定;或者若肘釘12的橋16以一餘隙被接收於凹部28中且若腳14在穿孔32內具有一餘隙、或者若肘釘12的腳14對於板13的周邊配置有一小間隔則藉由一摩擦接合。
板13的中央孔31係容許板13以下文參照圖5至7所描述的方式附接至一工具的一前端。因此,中央孔31可形成為具有多種不同形狀,諸如圓柱形,只要
板13可被附接至工具的前端即可。或者,板13中的中央孔31可具有比起工具前端略微較小的一直徑,或者板13中的中央孔31可被定形為圓柱形以接收及夾固於一工具上。
尚且,板13係可設有對於一中央軸線34正交地穿透過板13之一第一及一第二通路33a及33b。第一及第二通路33a及33b係朝向板13的上側22開啟,藉以界定切割線以利肘釘12及板13之植入。
圖5至9顯示一用於利用手術植入物將諸如韌帶及腱等軟組織附接至骨頭之工具40的一實施例。工具40係包含一縱向軸線41及一與其同軸配置的套筒42、一可滑式配置於套筒42中之衝擊桿44、及一附裝至衝擊桿44且可滑式配置於套筒42的外周邊表面上之衝擊塊46。衝擊桿44可在套筒42中從一其中使肘釘12及板13位於一分離的插入位置中之縮回位置(圖5至8)位移至一其中造成肘釘12的腳14穿透過板13且進入骨頭內之延伸位置(圖9)。衝擊桿44係包含一中央通道44a,故工具40可被推押於一克氏線(Kirschner-wire)(未圖示)上方。工具40的套筒42係具有一前端54、一後端56、及一握柄57。與前端54軸向地分隔開來,套筒42係包含沿著縱向軸線41延伸之兩個直徑方向相對配置的槽58(圖6及7中只可看見槽58的一者)。槽58係建構且設定維度成接收一對的銷60(圖7至9中只可看見銷60的一者),該對銷60
係被附裝至衝擊桿44及衝擊塊46藉以互連衝擊桿44及衝擊塊46。
套筒42可在其前端54包含彈性夾固鉗(未圖示)。彈性夾固鉗係可建構及設定維度以被插入板20的中央孔31內且韌性地握持板13使得板13可被可釋放式固定至套筒42。或者,如圖7清楚顯示,套筒42的前端54可建構為被接收於中央孔31中,故板13可藉由一干涉配合被可釋放式附接至套筒42的前端54;或者套筒42的前端54可作圓錐形建構,以使一具有一對應圓錐形建構的中央孔31之板13可摩擦性接合於套筒42的前端54。
工具40的衝擊桿44具有一前導端64、一尾隨端66、及一配置於尾隨端66之衝擊頭68。當衝擊桿44在套筒42中被往回抽引至縮回位置內時,衝擊桿44的尾隨端66係從套筒42的後端56突出,且衝擊桿44的前導端64位居套筒42內。衝擊頭68係建構且設定維度以在肘釘12插入骨頭內期間受到鎚件行程所衝撞。
在接近套筒42的前端54處,衝壓塊46係被可同軸滑動地配置於套筒42周圍。如圖4清楚顯示,肘釘12的中央開口17係設定尺寸且建構為使肘釘12可被定位於套筒42上相距套筒42的前端54呈一距離,其中中央開口17係滑動地接收套筒42以使肘釘12藉由一干涉配合被可釋放式附接至套筒42。或者,衝擊塊
46可在其前端54包含彈性夾固鉗(未圖示)。彈性夾固鉗係可建構及設定維度以韌性地握持肘釘12的橋16使得肘釘12可被可釋放式固定至衝擊塊46。衝擊塊46的一前端70係可具有一比肘釘12的橋16周邊表面呈現略微更小維度之凹部,故使肘釘12可藉由一干涉配合被可釋放式附接至衝擊塊46的前70。
如上述,衝擊塊46藉由銷60連接至衝擊桿44。銷60係在衝擊桿44的前導端64被固定至衝擊桿44並各延伸穿過套筒42中之兩個槽58的一者。肘釘12及板13彼此被同軸地定位。由於銷60在槽58中被引導,衝擊塊46係可軸向地滑動,但可相對於套筒42作不可旋轉式位移。衝擊塊46相對於套筒42之非旋轉式定位係當肘釘12衝擊時造成肘釘12的腳14穿透過板13。
現在參照圖8及9,使用中,肘釘12及板13的一組合係安裝在工具40上。依意願,工具40可以一克氏線(Kirschner wire)配置穿過工具40的通道44a而對準於一所想要的固定點。由於板13附接至工具40前方,藉由將板12下側壓抵住一軟組織72,可以板13使得軟組織72(圖8)被臨時地阻絕或固持抵住一骨頭74。肘釘12隨後可藉由鎚打於衝擊頭68上而被衝擊,以使肘釘12沿著套筒42移動並驅動肘釘12的腳14穿過板13且進入骨頭74內,故使肘釘12的腳14被固定至骨頭74,其中軟組織72被定位於板13與骨
頭74之間且板13被定位於肘釘12的橋16與軟組織72之間(圖9)。工具40隨後可縮回,藉以從工具釋放肘釘12及板13。
本所揭露的手術植入物之下列應用係為可行:膝區-膝關節側韌帶重建,及髖腱修復;肩區-肩旋轉袖修復,肱二頭肌腱固定術,喙鎖韌帶修復,AC關節分離降低,及三角肌撕裂修復;足區-跟腱修復;及一般手術-小骨韌帶成形術(small bone ligamentoplasty),小切口,切口及關節鏡手術程序係為可能。

Claims (25)
- 一種手術植入物,包含:一肘釘,其具有一橋及從該橋延伸之複數個腳,該橋具有一延伸穿過該橋之中央開口;及一板,其具有一上側及一下側,該板與該肘釘分離,其中該肘釘包含一具有一拉伸強度的材料,且其中該板係包含一具有比該肘釘之拉伸強度更小的一拉伸強度之材料,其中該等腳可對準於該板的方式為當該肘釘的腳被驅動穿過該板且進入一骨頭內時可藉該板定位於該肘釘的橋與該骨頭之間而使該肘釘被固定至該骨頭。
- 如申請專利範圍第1項之手術植入物,其中該板具有一中央開口,當該肘釘的腳被定位為穿過該板時,該中央開口係軸向對準於該肘釘的中央開口,且其中該肘釘的中央開口不帶有螺紋(non-threaded)。

- 一種用於固定一軟組織至一患者的一骨頭之套組,其包含:一工具,其包含:一套筒,其具有一前端;一衝擊桿,其可滑地配置於該套筒中;及一衝擊塊,其被附裝至該衝擊桿且可滑地配置於該套筒的外周邊表面上;至少一肘釘,其具有一橋及從該橋延伸之複數個腳,該橋具有一延伸穿過該橋之中央開口,使得該肘釘可被定位於該工具的套筒上相距該套筒的前端呈一距離,且對準於該衝擊塊,以使得衝擊該衝擊桿將造成該衝擊塊沿著該套筒驅動該肘釘朝向該套筒的前端;及至少一板,其具有一上側及一下側,該板可被定位於該套筒的前端上,藉以當該肘釘被定位於該套筒上時與該肘釘分離,且其方式使得可使用該工具操 縱該板以固持該軟組織抵住該骨頭,且其方式可以當該肘釘的腳被驅動穿過該板且進入該骨頭內時,令該板被定位於該肘釘的橋與該軟組織之間,而使該肘釘被固定至該骨頭。
- 如申請專利範圍第11項之套組,其中該板具有一中央開口,該中央開口係建構為接收該套筒的前端。
