Description
本發明係屬於免疫療法之領域,且係關於一種判定某種免疫療法治療效力之方法。本發明方法包括在開始免疫療法治療之後一段時間,測定特殊的生物標記,以評估該治療法之臨床結果。本發明因此具有醫學領域之應用。
傳統疫苗技術係將一種可誘發免疫反應的抗原(例如肽、蛋白質)引入至動物系統中,並藉此保護該動物免受例如感染,該技術已知多年。該等技術進一步包括活疫苗及失活疫苗兩者的發展。活疫苗通常為感染媒介之經減毒的非致病形式,其能夠引發針對該感染媒介之致病形式之免疫反應。

因此,近年來主要致力於例如發現藉由刺激免疫系統某些關鍵態樣而可增加由免疫療法所誘發之免疫反應的新穎藥物化合物,稱為免疫反應改質劑(IRM)或佐劑之大多數該等化合物表現為經類-Toll受體(TLR)誘發多種重要細胞激素生物合成(例如干擾素、介白素、腫瘤壞死因子、等。參見例如Schiller等人,2006,Exp Dermatol.,15,331-341),透過基本免疫系統機制起作用。已顯示該等化合物會刺激某些源於樹突狀細胞、單核細胞/巨噬細胞之細胞激素快速釋放,且亦能夠刺激B細胞分泌抗體(其在IRM化合物之抗病毒及抗腫瘤活性上起重要作用)。
替代性地已提出免疫療法策略,其中大多數係以引發-加強接種疫苗療程為基礎。根據該等「引發-加強」免疫療法方法,首先藉由向病患投與引發性組合物來誘發免疫系統,隨後再藉由投與加強性第二組合物來加強(參見例如EP1411974或US20030191076)。
此外,業已顯示,在健康護理領域,一種治療法可能只對特定病患群有效。因此希望提供一種能夠制定出最優且個性化之病患療法的生理工具及方法(亦即能夠在正確時間為正確病患提供正確的療法,能夠提供較高治療成功率,能夠監測對治療法之反應,能夠增加藥物效力,能夠避免採用對病患不適用之療法不必要的治療病患,能夠減少病患不必要的毒性及副作用,能夠減少病患之花費及關於不必要或危險無效藥物之保險費用,且能夠改善病患之生活品質,最終使得癌症成為可治癒之疾病),及(若適宜則)提供後續檢定法。
基於上述考慮,文獻提出許多工具及方法,諸如(例如):
-藥物遺傳學,其係由研究對藥物之個別反應成基因差異之函數所組成。該等反應係關於一種藥物如何在某個指定個體中起作用,其如何代謝、其毒性及劑量要求。得益於人類基因體計畫,藥物遺傳學已擴展至藥物基因體學。藥物基因體學之應用範圍超越藥物遺傳學,其有潛力用於自藥物發現及發展中找出用途,標靶性發現及校驗,及臨床試驗。
-新陳代謝學亦可用於預測性藥物之領域。與受限於遺傳因素的藥物遺傳學不同,藥物-新陳代謝學能夠基於遺傳學因素及非遺傳學因素(諸如病患體內之其他藥物、病患當前之健康狀態等)預測個體對藥物之反應。
-在臨床發展療法上,生物標記的作用的重要性與日俱增。生物標記可為正常生物學過程、疾病過程、或對療法介入之藥理學反應的指示劑。其角色範圍係幫助鑑別出具有反應者與沒有反應者,從而將病患群體分級,至判定治療法之效力。生物標記為一種用於作出較佳決定之重要工具,其將減少藥物在發展上之花費,並使療法更快地提供給正確病患群體。
本發明提供一種評估涉及向病患投與免疫原性組合物之治療法(亦即免疫療法治療法)之效力的物質及方法,其係採用一種已判定為大體上可靠之與所需免疫反應相關之信號物的生物學標記(生物標記)。該等生物標記存在於自該病患獲得之生物樣本中。於一種治療法啟動後不久,預測該治療法之臨床結果的能力將使醫生及病患能夠鑑別出無效療法,作出關於治療過程的明智決定(包括放棄還是允許實施替代性療法)。
如整個申請案通篇所用,除非上下文另外指出,否則使用「一」(「a」)及「一」(「an」)係意指「至少一個」、「至少第一個」、「一或多個」或「複數個」所指化合物或步驟。例如,術語「一細胞」包括複數個細胞,包含其混合物。更明確地,「至少一個」及「一或多個」意指是1的數目,或大於1的數目,特佳者指1、2或3。
全文所用之術語「及/或」包括「及」、「或」及「由該術語相連之元素的全部或任一其他組合」之含意。
如文中所用,術語「大約」或「約」意指20%以內,較佳10%以內,且更佳5%以內。
術語「病患」、「受檢者」係指脊椎動物,特別是許多哺乳動物物種,且包括但不限於畜養動物、競技類動物、包括人類之靈長類動物。
如文中所用,術語「治療法」或「治療」包括預防及/或治療。因此,本發明免疫原性組合或方法不限於治療應用,且可用於預防。其包括在文中術語「以發展一種預防性或治療性反應,較佳免疫反應」。「預防」不限於預防立即性疾病(例如感染性疾病),其進一步包括預防該等感染之長期結果,諸如硬化或癌症。

本發明提供一種於活體外評估向病患投與免疫原性組合物之治療法(亦即免疫療法治療法)之效力的方法。
根據本發明,術語「評估」應理解為「監測、修改或調整」涉及向病患投與免疫原性組合物之治療法。
在某些態樣中,該方法包括在免疫療法治療之後,以病患的經活化T淋巴細胞(CD3+ CD69+)含量為基礎,評估免疫療法治療之效力。

根據本發明特定實施例,「免疫療法治療法」係由至少一次向病患投與免疫原性組合物所組成,根據特定實施例,該「免疫療法治療法」包括向病患連續投與免疫原性組合物,更特定言之,其包括於至少兩週期間,較佳於至少六週期間投藥。

根據本發明一項特定實施例,該等抗體為單株抗體。
可使用之抗體實例為抗-CD3(Beckman Coulter目錄編號A07748)及抗-CD69(Beckman Coulter目錄編號IM2656U)。
根據本發明一項特定實施例,該等抗體係經例如螢光、放射標記、酵素、生物素加以標記,或任何其他設計用於將細胞以可偵測到該等抗體標記之方法。該等技術廣泛應用於相關技術中,並為已知的。
根據本發明一項較佳實施例,經活化T淋巴細胞(CD3+ CD69+)含量係利用對CD3及CD69具有特異性的抗體測定。
因此,測定經活化T淋巴細胞(CD3+ CD69+)含量係藉由:收集周邊血液,並將細胞與單株抗體(例如與抗-CD3、及CD69)培養。隨後,利用例如由Instrumentation Laboratory-Beckman Coulter所製造之具有可識別四種不同螢光(螢光素異硫氰酸酯FITC、藻紅蛋白PE/RD1、ECD、PC5/PE)之波長的He-Ne雷射光的流式細胞儀,測定經活化T淋巴細胞(CD3+ CD69+)含量。
經活化T淋巴細胞(CD3+ CD69+)含量以下列其中之一表示:(i)佔可以表現CD3及CD69細胞表面抗原之周邊血液淋巴細胞之百分比(%);或(ii)每微升全部周邊血液中經活化T淋巴細胞(CD3+ CD69+)之絕對數量。
如文中所用,於較佳實施例中,術語「高於經活化T淋巴細胞(CD3+ CD69+)中位數含量」意指(i)經活化T淋巴細胞(CD3+ CD69+)含量高於約10.4%,或(ii)在每毫升全血中大於148個CD3+ CD69+淋巴細胞。適宜地,該「經活化T淋巴細胞(CD3+ CD69+)之中位數含量」係在向病患投與免疫原性組合物有關之治療法開始後第43天測定。
在本發明一項較佳實施例中,本發明方法進一步包括初始步驟,該步驟係由投與該免疫原性組合物之前測定該病患體內之經活化T淋巴細胞(CD3+ CD69+)含量所組成。

根據本發明之變化,免疫原性組合物包含至少2種抗原,或編碼至少2種抗原之異種核苷酸序列,或編碼至少2種抗原之至少2種異種核苷酸序列,或其組合。
根據另一項特定實施例,該本發明異種核苷酸序列編碼選自以下組成之群的HPV抗原之全部或部分:HPV之E6早期編碼區域、HPV之E7早期編碼區域、及其衍生物或組合。
根據本發明之由重組載體編碼之HPV抗原係選自由以下組成之群:HPV E6多肽、HPV E7多肽或HPV E6多肽及HPV E7多肽。本發明包括使用對p53之結合力改變或至少顯著減小之任一HPV E6多肽,及/或使用對Rb之結合力改變或至少顯著減小之任一HPV E7多肽(Munger等人,1989,EMBO J. 8,4099-4105;Crook等人,1991,Cell 67,547-556;Heck等人,1992,Proc. Natl. Acad. Sci. USA 89,4442-4446;Phelps等人,1992,J. Virol. 66,2148-2427)。適於本發明目的之非致癌性HPV-16 E6變體之位於約第118位至約第122位之一或多個胺基酸殘基已缺失(+1表示天然HPV-16 E6多肽之首個甲硫胺酸殘基,以全部第118至第122位殘基缺失者(CPEEK)為特別佳)。適於本發明目的之非致癌性HPV-16 E7變異體之位於約第21位至約第26位上之一或多個胺基酸殘基已缺失(+1表示天然HPV-16 E7多肽之首個甲硫胺酸殘基,以第21至第26位殘基全部缺失者(DLYCYE)為特別佳)。根據較佳實施例,用於本發明之一或多種HPV-16早期多肽係經進一步之修改,以改善I類MHC及/或II類MHC展現,及/或以刺激抗-HPV免疫。HPV E6及E7多肽為核蛋白質,且先前已顯示膜展現可改善其治療效力(參見例如WO99/03885)。因此,建議修改至少一種HPV早期多肽,使其可錨定於細胞膜。可藉由在HPV早期多肽中併入膜-錨定序列及(若天然多肽缺少,則)併入分泌序列(亦即信號肽)輕易地實現膜錨定化。在相關技術中已知膜-錨定及分泌序列。簡言之,分泌序列存在於膜展現或分泌多肽之N-末端,並引發該等多肽傳遞至內質網(ER)。其通常包括15至35個基本上疏水性之胺基酸,隨後藉由特定之位於ER內之肽鏈內切酶將其移除,產生成熟多肽。天然膜-錨定序列通常高度疏水,並可將多肽錨定於細胞膜(參見例如Branden及Tooze,1991,in Introduction to Protein Structure p. 202-214,NY Garland)。
多種膜-錨定及分泌序列可用於本發明之範圍。其可獲自包含其之任一膜-錨定及/或分泌多肽(例如細胞性或病毒性多肽),諸如兔醣蛋白、HIV病毒外被膜醣蛋白、或麻疹病毒F蛋白質、或可合成。用於插入根據本發明使用之各早期HPV-16多肽的膜錨定及/或分泌序列可具有相同或不同之來源。分泌序列之較佳插入位點為開始轉譯之密碼子的N-末端下游,且膜-錨定序列之較佳插入位點係位於C-末端,例如終止密碼子之緊鄰上游。
用於本發明之HPV E6多肽較佳係藉由插入麻疹F蛋白質之分泌及膜-錨定信號序列加以修飾。視需要或在組合中,用於本發明之HPV E7多肽較佳係藉由插入兔醣蛋白之分泌及膜-錨定信號序列加以修飾。
亦可藉由使用編碼免疫增強劑多肽之一或多種核酸改善重組載體之治療效力。例如,宜使HPV早期多肽連接至以下多肽:諸如鈣網蛋白(calreticulin)(Cheng等人,2001,J. Clin. Invest. 108,669-678)、結核分枝桿菌熱休克蛋白70(HSP70)(Chen等人,2000,Cancer Res. 60,1035-1042)、泛素(Rodriguez等人,1997,J. Virol. 71,8497-8503)或諸如綠膿桿菌外毒素A之細菌毒素的移位區域(ETA(dIII))(Hung等人,2001 Canser Res. 61,3698-3703)。
根據另一項實施例,根據本發明之重組載體包括編碼一或多種上述定義之早期多肽(且更特定言之,HPV-16及/或HPV-18早期E6及/或E7多肽)的核酸。
根據另一項特定且較佳實施例,該本發明異種核苷酸序列係編碼MUC 1抗原或其衍生物之全部或部分。
根據另一項特定實施例,該本發明異種核苷酸序列編碼以下一或多種多肽之全部或部分:HCV外被膜蛋白質E1或E2、核蛋白質、NS2、NS3、NS4a、NS4b、NS5a、NS5b、p7或其衍生物。根據另一項特定實施例,該本發明異種核苷酸序列編碼一或多種非天然組態之融合蛋白質,其意指至少其中一種NS多肽所存在之順序與天然組態不同。因此,若該融合蛋白質包括NS3多肽、NS4A多肽及NS5B多肽,則天然組態可為NS3-NS4A-NS5B,其中NS3位於N-末端,且NS5B位於C-末端。與之相對的,非天然組態可為NS5B-NS3-NS4A、NS5B-NS4A-NS3、NS4A-NS3-NS5B、NS4A-NS5B-NS3或NS3-NS5B-NS4A。特定言之,根據本發明之融合蛋白質包括至少一種以下融合蛋白質:

於本發明融合蛋白質之該特定部分中,各NS多肽可相互獨立地為天然或經修飾。例如,包含於NS4A-NS3部分之NS4A多肽可為天然,而NS3多肽則包括至少一種如下所述之修改。
若需要,用於本發明之核酸分子可經最優化而提供於特定寄主細胞或生物(例如人類寄主細胞或生物)中高度表現之標靶抗原(例如HP V早期多肽)。密碼子最優化之常用方法為:將一或多個對應於不常用於哺乳動物寄主細胞之密碼子的「天然」(例如HPV)密碼子替換為一或多個較常使用之編碼相同胺基酸之密碼子。此可藉由常用突變化方法或藉由化學合成技術(例如導致一合成核酸)達成。並不一定替代所有對應於不常使用之密碼子的天然密碼子,因為即使部分取代亦可使表現增加。此外,從嚴守密碼子至最優化密碼子用法之某些變化誤差可以加以製造,以方便限制性位點之引入。
如文中所用,術語「重組載體」係指病毒及非病毒載體,包括染色體外(例如,離合染色小體)、多副本及整合載體(亦即,用以併入至寄主染色體之載體)。對本發明具特別重要性的係用於基因療法之載體(亦即能夠遞送核酸至寄主生物者)及用於多種表現系統之表現載體。適當的非病毒載體包括質粒,諸如pREP4、pCEP4(Invitrogene)、pCI(Promega)、pCDM8(Seed,1987,Nature 329,840)、pVAX及pgWiz(Gene Therapy System Inc;Himoudi等人,2002,J. Virol. 76,12735-12746)。適當的病毒載體可源自於多種不同病毒(例如逆轉錄病毒、腺病毒、AAV、痘病毒、疱疹病毒、麻疹病毒、泡沫病毒及類似病毒等)。如文中所用,術語「病毒載體」包括載體DNA/RNA以及其所產生之病毒顆粒。病毒載體可呈複製-感受態形式,或是在遺傳學上是失能狀態而呈現為複製-缺陷態或複製-受損態。如文中所用之術語「複製-感受態」包括選擇性複製及條件下複製的病毒載體,其係經改造而可在特定寄主細胞(例如腫瘤細胞)中複製得更好或選擇性複製。
在一態樣中,用於本發明之重組載體為腺病毒載體(參見「Adenoviral vectors for gene therapy」,2002,Ed D. Curiel及J. Douglas,Academic出版社)。其可源自多種人類或動物來源,且可使用自腺病毒血清型1直至51的任一血清型。以人類腺病毒2(Ad2)、5(Ad5)、6(Ad6)、11(Ad11)、24(Ad24)及35(Ad35)為特別佳。該等腺病毒可獲自美國類型菌種收集中心(American Type Culture Collection)(ATCC,Rockville,Md.),且已有許多文獻闡述其序列、機制、製造方法,相關技術者可使用該等訊息(參見例如US 6,133,028;US 6,110,735;WO 02/40665;WO 00/50573;EP 1016711;Vogels等人,2003,J. Virol. 77,8263-8271)。
用於本發明之腺病毒載體可呈複製-感受態。熟習此項技術者可輕易獲得複製-感受態腺病毒載體之許多實例(參見例如Hernandez-Alcoceba等人,2000,Human Gene Ther. 11,2009-2024;Nemunaitis等人,2001,Gene Ther. 8,746-759;Alemany等人,2000,Nature Biotechnology 18,723-727)。例如,其可藉由於E1A CR2區域中刪除(參見例如WO00/24408)及/或藉由以組織、腫瘤或細胞狀態-特異性啟動子代替天然E1及/或E4啟動子(參見例如US 5,998,205、WO 99/25860、US 5,698,443、WO 00/46355、WO 00/15820及WO 01/36650),自野生型腺病毒基因體改造而得。
或者,用於本發明之腺病毒載體係呈複製-缺陷態(參見例如WO94/28152;Lusky等人,1998,J. Virol 72,2022-2032)。較佳複製-缺陷態腺病毒載體為E1缺陷型(參見例如US 6,136,594及US 6,013,638),其具有自約第459位延伸至第3328位,或自約第459位延伸至第3510位之E1-缺失(可參見揭示於GeneBank之登錄號M 73260或揭示於Chroboczek等人,1992,Virol. 186,280-285之5型人類腺病毒之序列)。可藉由刪除腺病毒基因體之其他位點(非必須E3區域或其他必須E2、E4區域之全部或部分)而進一步改善選殖能力。如Chartier等人(1996,J. 在另一且較佳態樣中,用於本發明之載體為痘病毒載體(參見例如Cox等人,「Viruses in Human Gene Therapy」Ed J. M. 熟習此項技術者可獲得之許多文獻已闡述插入核酸之基本技術及如何在痘病毒基因體中表現所需之相關規則要素(Paul等人,2002,Canser gene Ther. 9,470-477;Piccini等人,1987,Methods of Enzymology 153,545-563;US 4,769,330;US 4,772,848;US 4,603,112;US 5,100,587及US 5,179,993)。通常係透過存在於病毒基因體及帶有欲插入核酸之質粒上的重疊序列(亦即所需插入位點)之間進行同源性重組作用。
較佳將編碼本發明抗原之核酸插入至痘病毒基因體之非必須位點,以保持重組痘病毒之活性及感染性。非必須區域為不編碼之基因間區域,或為在失去活性或缺失後不顯著損及病毒生長、複製或感染之任一基因。亦可考慮插入必須病毒位點,其限制條件為於導入病毒顆粒期間,以反式方式提供所缺失之功能,例如藉由使用攜帶對應於在痘病毒基因體中之缺失序列的互補序列的助手細胞株。
當使用哥本哈根痘病毒時,較佳將編碼抗原之核酸插入至胸苷激酶基因(tk)(Hruby等人,1983,Proc. Natl. Acad. Sci USA 80,3411-3415;Weir等人,1983,J. Virol. 46,530-537)。然而,其他插入位點亦為適宜,例如插入至血球凝集素基因(Guo等人,1989,J. Virol. 63,4189-4198)、插入至K1L基因座、插入至u基因(Zhou等人,1990,J. Gen. Virol. 71,2185-2190)或已有文獻報導之存在多種自發性或經改造之缺失的痘病毒基因體之左末端(Altenburger等人,1989,Archives Virol. 105,15-27;Moss等人1981,J. Virol. 40,387-395;Panicali等人,1981,J. Virol. 37,1000-1010;Perkus等人,1989,J. Virol. 63,3829-3836;Perkus等人,1990,Virol. 179,276-286;Perkus等人,1991,Virol. 180,406-410)。
當使用MVA時,可將編碼抗原之核酸插入至所定義缺失型I至VII之任一種、及插入至D4R基因座,但以插入至缺失型II或III為較佳(Meyer等人,1991,J. Gen. Virol. 72,1031-1038;Sutter等人,1994,Vaccine 12,1032-1040)。
當使用雞痘病毒時,雖然亦可考慮插入胸苷激酶基因,但較佳係將編碼抗原之核酸導入至位於ORF 7及9間之基因間區域(參見例如EP 314 569及US 5,180,675)。
根據一項特定實施例,該重組載體為重組質粒DNA或重組病毒載體。

啟動子具特別重要性,且本發明包括使用在多種寄主細胞中指導核酸表現的持續表現性啟動子、或僅於某些寄主細胞中指導核酸表現、或因應特定時間或外源因素(例如溫度、營養添加劑、激素或其他配體)指導核酸表現的持續表現性啟動子。適宜啟動子廣泛闡述於文獻中,且相關技術者可使用更明確言之病毒啟動子,諸如RSV、SV40、CMV及MLP啟動子。用於痘病毒載體之較佳啟動子包括但不限於疫苗啟動子7.5K、H5R、TK、p28、p11及K1L、介於早期及晚期痘病毒啟動子之間之嵌合啟動子、及合成啟動子,諸如彼等闡述於Chakrabarti等人(1997,Biotechniques 23,1094-1097);Hammond等人(1997,J. 熟習此項技術者應瞭解,控制本發明核酸分子表現之調節要素可進一步包括用於下列過程之其他要素:在寄主細胞或生物體中針對轉錄作用做出恰當的起始作用、調節及/或終止(例如,polyA轉錄終止序列)、mRNA轉運(例如,核定位信號序列)、處理(例如,剪接信號)、及安定化(例如,內含子及非編碼性5'及3'序列)、轉譯作用(例如,肽信號、前肽、三聯先導序列、核糖體結合位點、Shine-Dalgamo序列等)。
或者,用於本發明之重組載體可進一步包括至少一種編碼至少一種細胞激素之核酸。適宜細胞激素包括但不限於介白素(例如IL-2、IL-7、IL-15、IL-18、IL-21)及干擾素(例如IFNγ、INFα),以介白素IL-2為特別佳。當本發明重組疫苗包括表現細胞激素之核酸時,該核酸可由編碼一或多種抗原之重組載體所攜帶或由具有相同或不同起源之獨立重組載體所攜帶。
根據一項較佳實施例,本發明所使用之重組體是編碼MUC1抗原或其衍生物之全部或部分、及至少一種上述所列之細胞激素,且較佳者是介白素,特別是IL2。
包括上述重組病毒載體之感染性病毒顆粒可藉由常規性方法製得。示例性方法包括以下步驟:
a.將病毒載體導入至適宜細胞株中,
b.在適宜條件下培養該細胞株,以便產生該種感染性病毒顆粒,
c.自該細胞株培養物中回收所產生之感染性病毒顆粒,及
d.選擇性地純化該經回收之感染性病毒顆粒。
適於培養腺病毒載體之細胞為(例如)293細胞、PERC6細胞、HER96細胞、或揭示於WO 94/28152、WO 97/00326、US 6,127,175中之細胞。
適於培養痘病毒載體之細胞為鳥類細胞、且最佳者為獲自受精雞蛋之雞胚的初代培養的雞胚胎纖維母細胞(CEF)。
可自培養物上清液或經裂解後之細胞(例如藉由化學方法、冷凍/融解、滲透休克、機械休克、超音波及類似方法)回收感染性病毒顆粒。可藉由連續幾輪之斑塊純化作用,分離出病毒顆粒,並隨後利用相關技術中之技術(層析法、於氯化銫或葡萄糖梯度上之超離心分離)純化。
根據另一項實施例,本發明方法可與用於預測治療效力,更特定言之用於預測免疫療法治療之效力的其他方法組合。例如生物標記之含量,諸如經活化NK細胞之含量(參見主張EP 08305876.8之優先權的專利申請案)或sICAM-1之含量(參見主張EP 09305032.6之優先權的專利申請案)。
如於主張EP 09305256.1之優先權的專利申請案中所揭示,根據一項較佳實施例,本發明方法進一步包括測定病患之干擾素γ含量。
在某些態樣中,本方法因此包括在免疫療法治療之後數週,至少一次測定病患之干擾素γ含量;並以經活化T淋巴細胞(CD3+ CD69+)含量及干擾素γ含量為基礎評估免疫療法治療效力。
開始免疫療法治療與測定干擾素γ之間之時間可為1天至約48週或更長時間(例如,約1天至約1週、約1週至約2週、約2週至約4週、約4週至約8週、約8週至約12週、約12週至約16週、約16週至約24週、約24週至約48週、或更長時間)。在本發明之一項較佳實施例中,時間間隔約5週。類似地,其他測定(亦即第3次、第4次、第5次等測定)可在第2次測定之後,隔相同時間間隔進行。
在相關態樣中,本方法包括在向病患投與免疫原性組合物後,測定該病患之干擾素γ含量;比較該含量與截斷值;並以如文中所述之經活化T淋巴細胞(CD3+ CD69+)含量及與截斷值比較之干擾素γ含量為基礎,評估該免疫療法治療法之效力。
根據本發明之特定實施例,「干擾素γ含量」意指可測得之干擾素γ含量,「可測得」之定義為≧檢測極限。
根據特定實施例,干擾素γ含量之截斷值及/或檢測極限約4 pg/ml(例如,於血漿中為4.6 pg/ml)。
根據特定實施例,本發明係關於一種評估涉及向病患投與免疫原性組合物之治療法之效力的方法,其包括:
(i) 向該受檢者投與一或多劑該免疫原性組合物;
(ii) 在至少其中一次該投與後,測定該受檢者體內之干擾素γ含量及經活化T淋巴細胞(CD3+ CD69+)含量。
根據特定實施例,本發明係關於一種評估涉及向病患投與免疫原性組合物之療法的效力之方法,其包括:
(i) 向該受檢者投與一或多劑該免疫原性組合物;
(ii) 在至少其中一次該(等)投與後,測定該受檢者體內之干擾素γ含量及經活化T淋巴細胞(CD3+ CD69+)含量;
(iii) 其中干擾素γ含量高於約4 pg/ml(例如4.6 pg/ml)及經活化T淋巴細胞(CD3+ CD69+)含量高於中位數含量則表示,該受檢者對該治療顯示具有成功的臨床結果,即存活率增加。
有利地,該「干擾素γ含量」及該「經活化T淋巴細胞(CD3+ CD69+)含量」係在該涉及向病患投與免疫原性組合物之治療法開始後第43天測定。
根據特定實施例,咸已顯示在第43天於具有可測得之干擾素γ含量的病患與在第43天具有高於中位數之CD3+ CD69+淋巴細胞含量的病患之間在統計學上顯著相關(Mann-Whitney,p=0.02)。
在本發明之一項較佳實施例中,本發明方法進一步包括起始步驟,其係由在投與免疫原性組合物之前測定病患體內之干擾素γ含量所組成。
根據本發明,干擾素γ含量係在獲自該病患之生物樣本中測定。生物樣本包括但不限於血液及其他生物來源液體樣本、固體樣本(諸如活組織切片樣本)。在一項較佳實施例中,該生物樣本為血液、血漿或血清,在該種情形下,自病患獲得該等樣本之方法相對簡單,且為非侵入性之方法。獲得血液或血清之方法在相關技術中已熟知,非為本發明之部分。
此外,已知許多用於偵測並定量包括瞬時生物標記之多肽的方法。該等方法包括但不限於以抗體為基礎之方法,更明確言之,以單株抗體為基礎之方法。偵測及定量生物標記之特定方法對本發明並不重要。例如,本發明之材料及方法可採用Luminex技術(Luminex公司,Austin,Tex.)或酶聯免疫吸附檢定(ELISA,可自例如CliniScience、Diaclone、Biosource購得許多ELISA套組)。
若需要,投與根據本發明免疫原性組合物可與一或多種常用治療方法(例如放射術、化學療法及/或手術)一起進行。使用多種治療方法可提供給所選病患更為廣泛基礎之介入。在一項實施例中,投與根據本發明免疫原性組合物可在手術介入之前或之後進行。在另一項實施例中,其可在放射療法(例如伽馬放射)之前或之後進行。彼等熟習此項技術者可輕易地制定出適宜放射療法方案及可使用之參數(參見例如Perez及Brady,1992,Principles及Practice of Radiation Oncology,第二版. JB Lippincott Co;彼等熟習此領域技術者可進行改變及修改)。在另一項實施例中,投與根據本發明免疫原性組合物係與使用一或多種藥物(例如常用於治療或預防病毒感染、病毒相關之病症、癌症、及類似物)之化學療法聯合。
本發明因此係關於一種改善正接受使用化學治療劑之化學療法治療的癌症病患的治療效果之方法,該方法包括如下步驟:
- 向病患投與一或多劑根據本發明免疫原性組合物及一或多劑化學療法藥劑;
- 在至少其中一次該投與免疫原性組合物後,測定該受檢者體內之經活化T淋巴細胞(CD3+ CD69+)含量;

利用多種相關技術中之標準檢定法,於活體外或於活體內評估動物或人類生物體經投藥而誘發或刺激免疫反應的能力。對評估引發及活化免疫反應之可獲得技術的一般性闡述請參見例如Coligan等人(1992及1994,Current Protocols in Immunology;ed J Wiley & Sons Inc,National Institute of Health)。細胞免疫之測定法可為:藉由測定經活化效應細胞所分泌之細胞激素型態(包括彼等源自CD4+及CD8+ T-細胞者(例如藉由ELIspot,對可產生IL-10或IFN伽馬之細胞定量));藉由確定免疫效應細胞之活化狀態(例如利用經典[3
H]胸苷吸收之T細胞增殖檢定法);藉由檢定經激敏受檢者之抗原特異性T淋巴細胞(例如細胞毒性檢定中之肽特異性裂解)或藉由利用螢光MHC及/或肽多聚體(例如四聚體),偵測抗原特異性T細胞。可藉由抗體結合及/或結合時之競爭力測定刺激體液反應的能力(參見例如Harlow,1989,Antibodies,Cold Spring Harbor出版社)。本發明方法對於受適宜腫瘤-誘發性抗原激發之動物模型(例如表現MUC1之RMA細胞)亦是有效的,以測定抗腫瘤活性、反映對抗-抗原免疫反應之誘發或加強作用。
因此,本發明進一步係關於使接受免疫原性組合物而治療人類疾病之病患的存活時間延長之方法,該方法包括如下步驟:
- 向病患投與一或多劑根據本發明之免疫原性組合物,
- 在投與根據本發明免疫原性組合物之後,至少一次測定該病患體內之經活化T淋巴細胞(CD3+ CD69+)含量(參見上文)。
根據另一項實施例,本發明係關於一種以經活化T淋巴細胞(CD3+ CD69+)及/或經活化T淋巴細胞(CD3+ CD69+)含量作為生物標記之用途,用以預測接受免疫原性組合物治療之受檢者是或否對發展出預防性或治療性反應具有感受性,且以在投與免疫原性組合物之後出現預防性或治療性免疫反應為較佳。
根據另一項實施例,本發明係關於一種以經活化T淋巴細胞(CD3+ CD69+)及/或經活化T淋巴細胞(CD3+ CD69+)含量作為生物標記之用途,用以監測、修改或調整涉及向病患投與免疫原性組合物之治療法。
更明確言之,本發明係關於一種以經活化T淋巴細胞(CD3+ CD69+)及/或經活化T淋巴細胞(CD3+ CD69+)含量作為生物標記含量之用途,用以監測、修改或調整涉及向病患投與免疫原性組合物之治療法,其中在該投與後所測得之高於根據本發明中位數含量的經活化T淋巴細胞(CD3+ CD69+)含量表示,該受檢者對該治療法顯示具有成功的臨床結果,即存活率增加。
本發明亦提供套組,其包括用於實施文中所述方法的部分及在文中所提供之實例中顯見的部分。該等部分之套組可包括用以收集及/或測定血清中干擾素伽馬之含量的試劑。該等試劑可包括抗體。該等套組可進一步包括收集及/或處理生物樣本之設備。該等套組亦可包括使用說明、截斷值及/或其判定說明、及用於解釋使用該套組後所獲之數據的說明。
根據一項特定實施例,該等套組部分或該等套組可進一步包括上文所揭示及/或以下實例章節所揭示之免疫原性組合物。
本發明進一步提供電腦程序及/或規則系統,以監測臨床試驗及經活化T淋巴細胞(CD3+ CD69+)含量(及最後監測其他生物標記之含量,諸如,例如干擾素伽馬含量)、確定該等含量是高於還是低於閾值、及/或為治療方法建議修改,以改善病患對免疫療法治療之反應。該等電腦程式或規則系統可與必要之硬體一起提供,例如呈套組或裝置形式,其亦可接受生物樣本並測定其中存在之經活化T淋巴細胞(CD3+ CD69+)之相對含量(及最終測定其他生物標記含量,諸如例如干擾素伽馬含量)。上述電腦程式及/或裝置可以適當工具及試劑(包括抗體)提供給醫生或臨床實驗研究者。
本發明業已依闡述方法加以說明,應瞭解所使用之術語係用於闡述而非限制。顯而易見地,可對本發明之上述教示作出修改及改變。因此應瞭解,在附屬申請專利範圍內,可依不同於文中所特定闡述之方法實施本發明。

實例
在臨床研究TG4010.09(處於非小細胞肺癌晚期之MVA- MUC1-IL2)中,測試癌症免疫治療劑TG4010併與標準化學治療法之組合,並與僅使用化學療法作比較。在療法開始後(於第43天,第6次注射TG4010後一週),藉由流式細胞儀分析淋巴細胞表現型,顯示經活化T淋巴細胞(CD3+ CD69+)含量具有不均勻性。藉由Spearman相關係數比較CD3+ CD69+淋巴細胞含量與病患存活時間顯示出:僅在接受TG4010+化學療法之病患中,在第43天時之CD3+ CD69+淋巴細胞含量才與存活時間之間具有顯著相關,而在僅接受化學療法之病患中並非如此。對應於CD3+ CD69+淋巴細胞含量的病患存活時間之Kaplan-Meier存活圖(Kaplan-Meierplots survival)顯示,接受TG4010+化學治療的病患的存活時間與在第43天時之CD3+ CD69+淋巴細胞含量之間具有顯著相關。
實例1:
併與標準化學療法組合使用,將名為疫苗TG4010之免疫原性組合物用以治療非小型細胞肺癌(NSCLC)病患。
TG4010是一種重組經改質的安卡拉病毒(Modified Virus Ankara;MVA),其會表現IL2及腫瘤相關性抗原MUC1。
隨機分配148名病患接受如下治療:
-只有化學療法(於每3週之第1天投與順鉑75 mg/m2
且於第1天及第8天投與吉西他濱(Gemcitabine) 1250 mg/m2
,計達6個週期)(第2研究群)或
-化學療法連同TG4010(第1研究群)
每6週評估腫瘤(WHO標準)。評估指標是在6個月時之無疾病進展存活率(progression-free survival;PFS)及治療分析之整體存活率。
於第43天採集血液樣本(每週1次注射,第6次後一週),並立即送至中心免疫學實驗室,於該處使周邊血液單核細胞(PBMC)純化,並冷凍貯存至直至成批分析。
採用特異性針對淋巴細胞CD標記CD3、CD8、CD16、CD56及CD69之抗體,藉由5色流式細胞儀,分析PBMC樣本中之淋巴細胞子組之含量。
可採用之抗體實例為抗-CD3(Beckman Coulter目錄編號A07748)、抗-CD8(Beckman Coulter目錄編號6607102)、抗-CD16(Beckman Coulter目錄編號IM0814U)、抗-CD56(Beckman Coulter目錄編號A07788U)及抗-CD69(Beckman Coulter目錄編號IM2656U)。
圖1顯示,當以TG4010疫苗及化學療法二者治療時,具有高於經活化T淋巴細胞中位數含量(由第43天時之標記CD3+ CD69+鑑別)病患[第1研究群:TG4010-+化學治療]的存活時間(存活時間中位數=21.9個月)比具有低於CD3+ CD69+淋巴細胞中位數含量的病患的存活時間(存活時間中位數=10.4個月)長。
圖2中之數據證實:於第43天的高於CD3+ CD69+淋巴細胞中位數含量與總體存活率之間的正相關性僅侷限於接種治療疫苗之病患。對於僅接受化學療法之第2組病患而言,於第43天具有高於CD3+ CD69+淋巴細胞中位數含量的病患並未比於第43天具有低於CD3+ CD69+淋巴細胞中位數含量的病患活得更長久(11.4個月相對11.3個月)。

Claims (10)
- 一種評估涉及向病患投與包括編碼標靶抗原之至少一種重組病毒載體之免疫原性組合物之治療法效力的活體外方法,其包括:測定取自該病患體內之生物樣本中經活化CD3+CD69+T淋巴細胞含量,其中該病患已被投與一或多劑該免疫原性組合物,且其中該生物樣本係取於至少一種該投與之後;其中經活化CD3+ CD69+T淋巴細胞含量高於約10.4%時,表示該病患對該治療法顯示成功的臨床結果。
- 如請求項1之方法,其中該成功的臨床結果為存活率增加。
- 如請求項1或2中之方法,其中該免疫原性組合物進一步包括至少一種免疫反應改質劑。
- 如請求項1或2中之方法,其中該病患罹患癌症。
- 如請求項4之方法,其中該癌症為非小細胞肺癌或腎癌。
- 如請求項1或2中之方法,其中該標靶抗原為腫瘤特異性抗原。
- 如請求項6之方法,其中該腫瘤特異性抗原為MUC1。

- 一種以經活化CD3+ CD69+T淋巴細胞含量作為生物標記之用途,用以監測、修改或調整涉及向病患投與包括編 碼標靶抗原之至少一種重組病毒載體之免疫原性組合物之治療法,其中經活化CD3+ CD69+T淋巴細胞含量高於約10.4%時,表示該病患對該治療法顯示成功的臨床結果。
