Description
本發明係關於脊椎用之動態穩定系統,能夠穩定脊椎,不需脊椎湊合。按照申請專利範圍第1項,彈性連接元件係以新穎方式連接於骨釘或梗節釘。
按照習知技術,金屬桿可有複數桿/釘接頭,主要用於湊合手術(脊椎湊合)。只有少數彈性系統只支持和穩定(但不湊合)脊椎段,因而只有少數裝置可藉梗節釘附設彈性連接件。
原則上,適於金屬桿之桿/釘接頭,不必然亦適於彈性連接件,因為塑膠製彈性桿例如擁有與較硬金屬製桿不同的性能。因此,彈性塑膠桿不能利用非正向套合(或施力套合),以持久方式單純加以夾持,因為往往會因流動降低夾持力。因此,亟需有連接構想,除可能非正向套合型(基本)夾持外,包括正向套合之利,甚至理想的是動態正向套合,隨增加負載而暫時遞增,並隨降低負載而遞減。
以下列舉已知桿/釘接頭,及其相對於本發明之缺點。
雖然下述發明包括轉動式保持元件,唯一目的也是做為對釘之連接桿,不垂直於釘軸延伸。因此,此等保持元件只在附設階段可以轉動,在附設階段完成後,不再運動。專利申請案WO 01/06939(Martin Benlloch等人)、專利申請案WO 97/43974(Domlorowski等人);公告說明書US 4,987,892號(Krag和Pope)。
因此,本發明之目的在於把具有連續圓滑表面的塑膠材料製彈性桿,以連續而安全的方式,連接於骨釘或梗節釘,在過程中,於相鄰椎骨間傳送抗力和壓縮力,以及剪力和扭力。
此目的解決方案之特徵為,連接係持久(基本)非正向套合(或施力套合)和動態形式套合之利的組合。動態形式套合之利,是由塑膠材料製桿的局部彈性或塑膠變形所達成。此項動態形式套合之利隨桿中軸向力增加而自動遞增,並隨桿中軸向力降低而自動遞減。
因此,如申請專利範圍第1項所界定具有動態形式套合之利的非正向套合桿/釘連接,為本發明之標的。此項連接容許塑膠材料製的滑順彈性桿,可連接於骨釘或梗節釘之釘頭,使預期力可因其應用做為腰脊椎的動態穩定,而持久並安全傳送。此相關優點是兼具非正向套合連接(容易定位且免於間斷),和形式套合之利對連接的可靠性。在過程中,初期正向套合是利用形式套合隨軸向力增加而動態遞增,隨軸向力降低又遞減來加以支持。此舉可節省塑膠桿,並大為防止負載下的流動過程。
茲參照附圖詳述本發明如下。
第1a圖表示彈性連接件(2)在(斷面圖示)梗節釘(1)內,轉動性填充件(3)在樞點(4)之中立位置。第1b圖表示彈性連接件(2)一旦受到拉力或伸展,填充件(3)即轉動,此處填充件(3)之配置,使其力方向的對立邊緣碰到彈性連接件的表面,而發生形式套合之利,實質提高固定力。
第2a、2b和2c圖分別為彈性連接件(2)、梗節釘(1)、填充件(3)和夾持元件(5)之側視圖、前視圖和俯視圖。於此,夾持元件(5)直接把可轉動填充件(3),經球面(4a)緊壓於連接件(2)。
第3a、3b和3c圖分別為彈性連接件(2)、梗節釘(1)、填充件(3)和夾持元件(5)之側視圖、前視圖和俯視圖。於此,夾持元件(5)經由軸線(4b)把轉動性填充件(3)壓緊於連接件(2)。
第4a圖亦表示彈性連接件(2)在(斷面圖示之)梗節釘(1)內,轉動性填充件(3)在對樞點(4)之中立位置。第4b圖表示彈性連接件(2)一旦受到拉力或伸展,填充件(3)如何轉動。為提供彈性連接件表面之改進碰撞,填充件(3)包括肋條(6)。此等肋條(6)即使中立位置時,也稍會碰到彈性連接件(2)的表面,因而提高對填充件(3)之初期阻力,支持其在施力方向轉動。
第5圖表示例如圓形連接件(2)在梗節釘(1)內之俯視圖,有填充件(3)、球面(4a)之樞點,以及經鋸齒螺紋(7)連接於釘之夾持元件(5)。於此,鋸齒螺紋可防止二釘翼(1a)壓縮,因而可能限制填充件(3)之活動性。
第6圖表示例如圓形連接件(2)在梗節釘(1)內之俯視圖,有填充件(3),在軸線(4b)之樞點,此軸線同時用做夾持元件(5),利用釘翼(1a)之彈性展開,加以定位。

Claims (11)
- 一種脊椎植入體,包含連接件(2),複數骨釘(1),各一填充件(3),和各一夾持元件(5),使夾持元件(5)把連接件(2)以非正向方式夾持在釘頭的插座和填充件(3)之間,其中,填充件(3)為活動式,並以提高軸向力,增進連接件之夾持,又其中填充件之配置,使其力方向的對立邊緣碰到彈性連接件的表面,而發生形式套合之利,實質提高固定力者。
- 如申請專利範圍第1項之脊椎植入體,其中填充件(3)可繞具有夾持元件(5)共有的球面之接合樞點轉動者。
- 如申請專利範圍第1項之脊椎植入體,其中填充件(3)可繞利用夾持元件(5)保持的軸線(4b)轉動者。
- 如申請專利範圍第1項之脊椎植入體,其中填充件(3)包括肋條(6),供連接件(2)的表面更有效碰撞者。
- 如申請專利範圍第4項之脊椎植入體,其中連接件(2)為桿者。
- 如申請專利範圍第5項之脊椎植入體,其中桿為圓形者。
- 如申請專利範圍第1項之脊椎植入體,其中連接件(2)係彈性塑膠材料製成者。
- 如申請專利範圍第7項之脊椎植入體,其中連接件(2)係PCU(聚碳酸酯胺基甲酸酯)製成者。
- 如申請專利範圍第1項之脊椎植入體,其中夾持元件(5)係螺帽者。
