527181 A7 B7 五、發明說明(/ ) 本發明關於申請專利範圍引文的一種髖內義肢。 (請先閱讀背面之注意事項再填寫本3) 各依患者髖關節的磨損程度以及患者預期壽命’而將 關節拍檔的骨物質對應地利用義肢取代’俾能如此再創造 關節功能。如果髖關節球窩仍緊密不變,則將股骨頭全部 或只部分取代就可足夠。當磨損情形與二個關節的拍檔 (Partner)都有關時,則必須植入一個人工股骨頭以及植入一 個髖關節臼。特別是完全更換時,會造成它們深深嵌入骨 物質中,並使骨物質對應地損失。因此人們一直硏究設法 將骨物質及其壽命效力儘量保持。 在美專利US 3,521,302提到,將一球窩嵌入骨盆骨中 ,它利用一根用骨水泥固定入該骨盆骨中的銷保持住。股 骨頭的關節接合面利用一帽蓋代替,它與股骨頭配合’且 同樣利用骨水泥固定到該股骨頭之骨物質中的銷固定。當 把股骨頭部作處理以配合一帽蓋時,一般大腿頸的供應血 管會進一步破壞。因此不能再確保該帽蓋下方的骨殘肢供 血。特別是當帽蓋將骨組織蓋住很大面積時,則骨組織會 萎縮而帽蓋會變鬆。 經濟部智慧財產局員工消費合作社印制衣 在德專利DE-PS 1 164 019提到一種股骨頭用的義肢。 其形狀呈一空心球形式,由一可生物相容的金屬構成,且 利用焊在其內面上的銷鎖固在大腿頸骨中。當帽蓋套上去 時’銷必須根據其位置準確地並正確地關聯地對準那些孔 。股骨頭義肢利用可硬化塑膠(它們充塡到該空心球內部 )而配合該骨面。此外,該塑膠由於有絕緣作用,會使銷 的固定部份(亦即在球內部的焊接位置之間)的可能的腐 4 本紙張尺度適用中國國家標準(CNS)A4規格(210 X 297公釐) " "" " 527181 A7 ----- B7 經濟部智慧財產局員工消費合作社印製 五、發明說明(>) 影響減少。要將義肢固定,需要一種裝置,藉之可鑽孔, 以將銷鎖固在大腿頸骨中的正確位置。這些孔穿過整個骨 ’這點在一般用三根銷的場合,會導致骨物質對應地變弱 。空心球充以塑膠料的作業須立刻達成,俾不會產生空腔 。此外’塑膠用量必須使得它硬化時,帽蓋的位置不會改 變(例如不會由於隆起而改變)。 本發明的目的在提供一種髖義肢,它在使用時,骨物 質的損失可限制在需要的最小量,俾能(特別是在年輕病 患’其預期之可活時間還很久)保持較多的骨成份,當過 期作再手術時,可在此骨成份開始。 這種目的係利用申請專利範圍第1項的特徵點達成。 本發明有利的進一步特點見於申請專利範圍副項。 利用本發明的髖內義肢,可各依磨損狀態,將關節拍 檔對應地置換。如果髖臼(Acetabulum)的狀態仍能做關節拍 檔的功能’則股骨頭要替換的部分主要爲圍住關節接合面 的部分。在此,依本發明,係用一實心球作取代,它完全 由相同之可生物相容的材料構成。這種球的尺寸與自然的 股骨頭的關節接合面的尺寸一樣。和空心球或帽蓋比起來 ,實心球有一好處,即:它在受負荷時不會變形。此外, 與帽蓋不同者’可避免以很大面積蓋住骨組織(故可避上 述缺點)。固定作用只要在大腿頸骨中用一根銷,因此骨 物質只須在一個位置鑽孔。 球固定在銷上的作用係以有利方式利用一錐形插接的 接合方式達成。球利用錐形插接方式接合固定在髖內義肢 5 (請先閲讀背面之注意事項再填寫本頁) 言 r ί 本紙張尺度適用中國國家標準(CNS)A4規格(210 χ 297公釐) 527181 A7 B7 經 濟 部 智 慧 財 產 局 員 工 消 作 社 印 製 五、發明說明(,) 中的作業很簡單,且多年以來證實很有效。錐形插接的接 合方式可以使銷先植入骨中,然後將球放上。在此,球與 帽蓋不同,不需作對準。銷由一錐形栓(用於將球插上) 及一個接到該栓上的鎖固部(它植入骨中)構成。 在本發明另一有利特色中,球有一凹隙。利用此凹隙 ,球嵌到大腿頸骨的股骨頭之突部(它與此凹隙配合)。 凹隙設計成使得該關節受負荷時(特別是垂直於銷方向者 )突伸進去的骨組織將球支持住防止側移。作用到銷上的 剪力遂度小,因避免銷之局部一邊的力作用到骨上。 球的錐形凹隙乃是凹隙的一種容易製造且有效的形式 。在大腿頸骨上對應地銑出股骨頭的突部。斜角可依球的 大小(特別是其直徑)以及現有的骨物質而決定(球要支 持在該骨物質上)。其値在80〜170度範圍。孔延伸過該 作成錐形的骨的尖端,銷植入孔中,球以該銷固定在骨上 〇 藉選設球的大小及銷的形狀,該義肢可最適當地調定 成所要再製造之關節的幾何性質。球的中點可位在一根直 銷的中央線上。但球相對於其凹隙的對稱線也可與該銷嵌 入骨中的鎖固部的中央線夾成一個多達20度的夾角。如此 ,這種偏移亦可存在栓的中‘央線與銷的鎖固部的中央線之 間。 爲了使銷更容易放入,以及爲了施某種夾緊力。故該 放入骨中的銷朝向其背向球的一端延伸,此該係背向球方 式且呈錐形延伸。 6 (請先閱讀背面之注意事項再填寫本頁) -¾ 訂: •線- 本紙張尺度適用中國國家標準(CNS)A4規格(210 X 297公釐) 經濟部智慧財產局員工消費合作社印製 527181 A7 ___ B7 五、發明說明(/) 當球鎖固在大腿頸骨中,如果該鎖固在骨中的銷的長 度與直徑依骨的尺寸決定,使鎖固作用主要在硬骨組織中 造成,則很有利。因此,銷的直徑與長度須依患者的年齡 與身體構造及現有的骨組織而決定。因此,在未長大的青 少年及矮小患者的場合,銷的尺寸在直徑及長度方面都可 和具有完全發育之骨構造及強壯之體形的成人用的尺寸有 所不同。 此外,在本發明另一有利設計中,球由可生物相容的 材料製成。所有在義肢醫學可用的材料及陶瓷材料都可稱 爲可生物相容材料。由於球的固定銷不須與球牢接,医I此 球的材料的選擇可依各種用途情形(特別是依球窩材料) 而決定。 使用陶瓷材料顯得很有利,尤其是當球頭有一植人白勺 球窩做爲關節拍檔時尤然。當內義肢中,陶瓷材料用陶瓷 材料作關節接合時,則磨損速率最低。陶瓷球用金屬銷鎖 固在骨中,因爲陶瓷與金屬不會互相造成引起腐蝕的交換 作用。 在本發明一有利設計中,球之非關節接合面做成粗縫 狀。它可另外或單獨地帶有一個可傳導骨細胞的層,例如 由羥基磷灰石構成,它可使骨生長到球中,因此使球牢牢 地鎖固在骨中。 本發明的髖內義肢可使得髖臼仍在適合直接作關節接 合狀態之時,使球直接作關節接合到髖臼中。然而,如果 髖臼磨損或有病,則依本發明只要把髖白的主負荷面利用 7 本紙張尺度適用中國國家標準(CNS)A4規格(210 X 297公釐) ------------壤------- 丨訂---------線 (請先閱讀背面之注意事項再填寫本頁) 527181 經濟部智慧財產局員工消費合作社印製 A7 B7 五、發明說明(s) 一植入的球窩取代,該球窩再和球作關節接合。 爲了要在植入時使骨物質可喪失及損壞得儘量少,球 窩的作用角度限制在90〜170度間。 從義肢要有長久使用壽命的觀點看,在考慮患者的年 齡與磨損的情況下,如果該球窩之與球作關節接合的面的 材料配合球的材料作選定,使這二種材料的配對在關節磨 損學(Tribology)上是最適合者,則甚宜。 在關節磨損學上可能的配對的例子,如銘一鉻製的球 與聚乙烯製的球窩嵌入物之間、或陶瓷球與聚乙烯製的球 嵩嵌入物之間的配對。關節磨損學上最適當的配對爲陶瓷 球窩與陶瓷頭,如髖內義肢業者已習知者。 由於當本發明之髖內義肢的球窩嵌入在骨盆的骨中時 ,只需將骨物質除去極少量(此量係嵌入時絕對必須除去 中),因此如果該球窩之嵌入骨中的表面部分含有一些元 件,可提高傾轉穩定性,則甚有利。爲此,舉例而言,可 設有框條,它們沿垂直或近乎垂直方向從該球窩之嵌入骨 中的平面隆起。它們嵌入髖骨的對應地銑出的凹隙中。 一如在球的情形,球窩也可在該要嵌入骨中的表面上 做成粗糙狀。球窩也可同樣地單獨地(或除了有粗糙面外 )另外,還帶有一種可傳導骨細胞的層,以使骨更容易更 快地長入髖骨中。 茲利用實施例詳細說明本發明。 圖式中: 第一圖係陶瓷製的本發明髖內義肢之一球窝的剖面圖 8 本紙張尺度適用中國國家標準(CNS)A4規格(210 X 297公釐) --------------------訂---------線 (請先閱讀背面之注意事項再填寫本頁) 527181 A7 B7 五、發明說明() 〇 第二圖係做爲股骨頭義肢的一球的剖面圖, 第三圖係一球的剖面圖,它利用一銷鎖固在大腿骨中 ,且與一個植入髖骨中的球窩作關節接合, 第四圖係一義肢,具有球與銷的組合,當作錐形插合 連接件,其中銷的中央線與球的對稱軸夾成一個10度的角 度,而植入之球直接與髖白作關節接合。 經濟部智慧財產局員工消費合作社印製 (請先閱讀背面之注意事項再填寫本頁) -I線· 〔圖號說明〕   (1) 髖關節球窝  (2) 功能面  (3) 表面  (4) 框條  (5) 粗糙構造  (6) 工能角度  (7) 實心球(股骨頭義肢)  (8) 關節接合面  (9) 錐形孔  (10) 凹隙  (11) 對稱線  (12) 尖端(錐形角度)  (14) 粗糙部  (15) 髖骨  (17) 大腿頸  (18) 骨輪廓(殘肢) 9 本紙張尺度適用中國國家標準(CNS)A4規格(210 X 297公t ) ---------- 527181 A7
