Abstract
Buluş tetrahidropirimidin ile ilgilidir. Burada R bir hidrojen atomu veya azami sekiz karbon atomlu bir hidrokarbon kısmı, R' hidrojen, OH veya OR'' dir ve R''R veya COR olup, R'nin cerrahi müdahalelerde mekanik yüklenmeler nedeniyle özellikle karın/bağırsak nahiyesinde ortaya çıkan, transplant ile ilgili stres ve ağrılar hariç, ameliyat sonrası inflamatuar stres ve ağrıların önlenmesi veya tedavisinde kullanılması ile ilgilidir. (Şekil 1)
Description
TEKNIK ALAN
Bulus, tetrahidropirimidin ve bunun ameliyat sonrasi inflamatuar stres ve agrinin
önlenmesinde ve tedavisinde kullanimi ile ilgilidir.
TEKNIGIN BILINEN DURUMU
Asiri sert kosullarda yasayabilen ekstremofil organizmalardan izole edilmis düsük
moleküler organik kimyasal bilesikler, biyolojik sistemler ve yapilar üzerinde dikkat
çekici bir etkiye sahiptir. Bu tür bilesikler, osmolitler veya uyumlu çözünmüs
maddeler olarak adlandirilir ve bu arada çok sayida kozmetik preparatlarda bunlara
kullanim alani bulunmustur.
Uyumlu çözünmüs maddelere, sekerler, kükürt bilesikleri, polioller ve amino asitler
yani sira özellikle ectoin ve hidroksiectoin gibi belirli tetrahidropirimidin türevleri
dahildir. Bunlar, asiri sert kosullarda yasayabilen ekstremofil mikroorganizmalar
tarafindan stres sartlarinda sentezlenirler ve asiri isi, kimyasal ve fiziksel kosullar
altinda bu mikroorganizmalarin hücresel yapilarinin stabilitesini saglarlar.
Büyümesi için, tuz içerikli ortamda degisken tuz ihtivasina mutlaka uyabilmek ve
bunun içinde hayatta kalabilmek için tuz açisindan zengin bir ortama ihtiyaci olan
mikroorganizmalar bunlara bir örnektir.
Ectoin ve türevlerinin, biyolojik madde ve farmasötik preparatlarin hazirlanmasi için
kullanilabilen, protein ve nükleik asit yapilari üzerindeki stabilize edici etkisi
bilinmektedir.
Ectoin ve türevleri için bu arada bir dizi tibbi uygulamalar, örnegin Dermatologikum
yani sira etkinlik arttirmak için protein içeren aktif maddeler tarif edilmistir. Ayrica
endotel islev bozukluklarinin tedavisinde terapötik bir etkisi kanitlanmistir. ABD
patenti US 6,267,973 B1, nemlendirici, yani cildi nemlendirici madde olarak ectoinin
yardimiyla stabilizasyonu ortaya konulmaktadir. DE 100 06 578 A1,
makromolek'üler organik polimerlerin enzimatik olarak indirgenmesinin inhibitörleri
olarak ectoin gibi uyumlu çözünmüs maddelerin kullanimi ile ilgilidir.
Ectoinin bilinen fizyolojik etkileri göz önünde bulunduruldugunda, ekseriya cerrahi
müdahalelerin bir sonucu olarak özellikle karin boslugunda ortaya çikan ameliyat
sonrasi inflamatuar stres ve agrilarda simdi bulunan etkinlik gerçekten sasirticidir.
BULUSUN TANIMI
Buna uygun olarak bulus, asagidaki formüldeki tetrahidropirimidin ile ilgilidir
burada R bir hidrojen atomu veya azami sekiz karbon atomlu bir hidrokarbon kismi,
R” hidrojen, OH veya OR” dir ve R”R veya COR olup, Rinin cerrahi müdahalelerde
mekanik yüklenmeler nedeniyle özellikle karin/bagirsak nahiyesinde ortaya çikan,
transplant ile ilgili stres ve agrilar hariç, ameliyat sonrasi inflamatuar stres ve
agrilarin önlenmesi veya tedavisinde kullanilmak üzere yukarida belirtilen anlami
Ameliyatlar veya bulus anlaminda müdahale, örnegin endoskopik muayeneler veya
eklem nahiyesinde veya karin boslugunda yapilan endoskopilerde vücut üzerinde
mekanik bir strese neden olan her müdahaledir. Ancak müdahaleler, özellikle de
sadece derinin nokta benzeri açilmasi/ayrilmasindan daha büyük olan cerrahi
müdahalelerdir.

Sek. 1, sadece polysollü (P) için ve ectoin (E) eklenmesi halinde U/I olarak Iaktat
dehidrojenaz (LDH) salim degerlerini gösterir.
Sek. 2, polysol (P) ve ectoin (E) uygulanan örnekler için mmoI/L olarak NOx salimini
gösterir.
Sek. 3, polysolde (P) ve ectoin (E) ile 30 dakikalik oksijenli reperfüzyondan sonra
doku örneklerinin elektron mikroskobik görüntülerini gösterir. AM, ectoin uygulanan
örneklerin çok daha iyi durumda oldugu aplikal hücre zari, MV mikrovillüsleri
gösterir.
BULUSUN DETAYLI AÇIKLANMASI
Bulus anlaminda ectoinler, (48)- 1,4,5,6-tetrahidropirimidin-4-karb0ksilik asitler ve
onlarin türevleridir. Tetrahidropirimidin karbon asitleri 2-konumunda, örnegin bir alkil
grubu ya da bir metil grubu gibi azami sekiz karbon atomu olan bir hidrokarbon
grubu içerebilirler. Tetrahidropirimidin ayni zamanda 5-k0numunda bir hidroksi
grubu ile, 'özellikle bir (58) hidroksi grubu ile ikame edilmis olabilir. Hidroksi grubu
farmasötik olarak kabul edilebilir ölçüde eterlenmis ya da esterlenmis olabilir.
Tercih edilen ectoinler, ectoinin kendisi (48)-2-metil-1,4,5,6-tetrahidropirimidin-4-
tetrahidropirimidin-4-karb0ksilik asittir.
Bulusa göre birden fazla ectoin birlikte kullanilabilir. Türevler olarak, büyük ölçüde
ilgili baz madde ile ayni veya ondan daha iyi etkiler sergileyebilen maddeler söz
konusudur
Bulusa konu olan tetrahidropirimidinler dogal olarak standart adjuvanlar ve katki
maddeleri ile birlestirilebilir. Kullanilan maddeler agizdan veya parenteral, örnegin
tablet, kapsül veya çözelti olarak alim için öngörülmüs olabilir. Bir ya da daha fazla
günlük, kg olarak vücut agirligi ve gün basina 1 - 250 mg, tercihen 5 - 150 mg ve
Agizdan veya parenteral uygulama yani sira tedavi edilecek bagirsak nahiyesine
veya karin bosluguna, ayni zamanda örnegin tetrahidropirimidin içeren, farmasötik
olarak fizyolojik çözelti gibi kabul edilebilir bir çözelti ile Iavman seklinde lokal bir
sunum da uygulanabilir. Su içinde tam olarak çözünebilen tetrahidropirimidinin
olaganüstü iyi biyo uyumlulugu nedeniyle bu tür Iavman çözeltileri %25'e kadar
tetrahidropirimidin içerebilirler. Ancak içerik genel olarak %O,5 - 5 arasindadir.
Bulusa konu olan tetrahidropirimidinler, transplantlarin da Iavmani ve korunmasi
için kullanilan Doorzand Medical Innovations firmasinin polysol çözeltisi ile birlikte
verilmesine ve kullanilmasina da özellikle uygundur. Polysol çözeltisine agirlikça
Tetrahidropirimidinler pratikte vücudun hemen hemen her alaninda ve pratikte her
cerrahi operasyon türünde müdahalelerde genis kapsamda kullanilabilirler. Bu, dis
çekimlerine, çene cerrahisi ve dislerin, yapay eklemlerin implantasyonlarina, göz
operasyonlarina kadar uygulanir. Bulusa göre transplantasyonlar bunun disindadir.
Daha ayrintili bir arastirma için siçanlardan olusan bir hayvan modelinde 10 mg/kg
trinitrobenzensulfon asidinin %50 etanol içinde hafif eter anestezisi etkisinde
verilmesiyle bir bagirsak iltihabi indüklenmistir. Dört gün sonra makroskopik olarak
arastirilabilen lezyonlar olusmus idi. TNBS verilmeden bir hafta önce ve dört gün
sonra agizdan farkli dozlarda (30 - 300 mg/kg) ectoin verilmistir. Ectoin lezyonlarin
derecesini düsürecek durumda olup, 100 mg/kg dozu ile azami etki gözlenebilmis
Biyokimyasal açidan ectoin miyeloperoksidaz etkinlikteki degisiklikleri engellemis
ve kalin bagirsaktaki glutatyon düzeyini düsürmüs idi. Ectoin ayrica lKAM-1,DNFo,
dokusunda çesitli medyatörlerin seviyesindeki degisiklerden korumustur. Etki,
referans ilaç olarak kullanilan 300 mg/kg'lik sülfasalazin ile benzer niteliktedir.
Ectoin'in koruyucu etkisi kalin bagirsagin histopatolojik olarak incelenmesi
temelinde teyit edilebilmistir.
Örnek (bulusun konusu olmaksizin)
Siçanlardan standart sartlar altinda ince bagirsak bölümleri alinmis ve hemen
soguk tuzlu çözelti ile yikanmis, ardindan polysol çözeltisi (Fa. Doorzand Medical
iskemik olarak 18 saat 50 ml besin çözeltisinde 4°C derecede saklanip, islemlerin
yarisinda polysol çözeltisine ectoin (5 mg/kg) eklenmistir.
Ince bagirsak bölümleri 4°C derecede saklandiktan sonra test tüpünde 37°C
derecede modifiye KHB ortami ile reperfore edilmistir (%5 dekstran 78, %0,95 KHB,
sodyum bikarbonat). Oksijen kaynagi olarak %95 02 ve %5 CO2 karisimi
kullanilmistir. Oksijen kismi basinci sürekli olarak 500 mmHg üzerinde tutulmustur.
Reperfüzyon sonunda tüm doku örnekleri -80°C derecede sivi azotta dondurulmus
ve Iaktat dehidrogenaz (LDH) salimi ve NO ölçütü olarak nitrit salimi durumu
arastirilmistir.
Doku kalitesinin bir ölçütü olarak LDH salimi için polysol içinde muhafaza edilen
ölçülmüstür. Ectoin ile modifiye edilmis polysol için 15 dak. Sonra 8,8 +/- 3,42 U/I
ve 30 dakika sonra 25,4 +/- 8,2 sonucu ortaya çikmistir. Ectoin kullanimi böylelikle
doku hasarinin derecesini azaltmistir, bu ise cerrahi operasyonlar ve
transplantasyonlar sonrasi iyilesme süreci üzerinde olumlu etkide bulunmustur.
NOX olusumu yaygin olarak doku örneginin maruz kaldigi (oksidatif) stres Için
gösterge kabul edilir.
Polysol ile tedavi gören test grubunda 0,78 +/- 0,063 mmol/L NOX olusumu elde
edilmis iken, ectoin katkili test gruplarinda ölçülen deger 0,325 +/-1,05 mmol/L
olmustur. NOx'in ikiye bölünmesi NO olusumunun ikiye bölünmesine neden olur ve
böylelikle oksidatif stresin önemli miktarda azalmasini saglar.
Mikroskopik incelemeler, ectoin uygulanan doku örneklerinde, sadece polysol
uygulanan örneklere kiyasla mikrovillüslerin durumunda önemli bir iyilesme
oldugunu göstermektedir.

Claims (10)
1. Asagidaki formüle sahip olan tetrahidropirimidin olup, özelligi: Rtnin bir hidrojen atomu veya azami sekiz karbon atomlu bir hidrokarbon kismi olmasi, R” degiskeninin hidrojen, OH veya OR” olmasi ve R” degiskeninin R veya COR olmasi, Rlnin yukarida belirtilen anlama sahip olmasi ve cerrahi müdahalelerde mekanik etkiler nedeniyle ortaya çikan transplant ile ilgili stres ve agrilar hariç, ameliyat sonrasi inflamatuar stres ve agrilarin önlenmesi veya tedavisi amaçli bir yöntemde kullanima yönelik olmasidir.
2. Istem 1'e göre tetrahidropirimidin olup, özelligi; karin boslugunda, karacigerde veya böbreklerde müdahalelerin neden oldugu inflamatuar stres ve/veya agrinin tedavisinde kullanima yönelik olmasidir.
3. istem 2'ye göre tetrahidropirimidin olup, özelligi; mide/bagirsak sisteminde enflamasyon ile ilgili agrilarin tedavisinde kullanima yönelik olmasidir.
4. Istem 1'e göre tetrahidropirimidin olup, özelligi; endoskopik uygulama, eklem nahiyesinde yapilan endoskopiler veya laparoskopik cerrahi müdahaleler sonrasi enflamasyon ile ilgili stres ve/veya agrilarin tedavisinde kullanima yönelik olmasidir.
5. Istem 1'e göre tetrahidropirimidin olup, özelligi; dis çekimleri, çene cerrahisi, kiriklar ile ilgili ameliyatlar, implantasyonlar veya göz ameliyatlarindan kaynaklanan enflamasyon ile ilgili stres ve/veya agrilarin tedavisinde kullanima yönelik olmasidir.
6. Istem 1 - 5'ten birine göre kullanim amaçli tetrahidropirimidin olup, özelligi; R degiskeninin metil olmasi ile karakterize edilir.
7. Istem 1 - 6'dan birine göre kullanim amaçli tetrahidropirimidin olup, özelligi; R' degiskeninin hidroksi olmasi ile karakterize edilir.
8. Istem 1 - 7'den birine göre kullanim amaçli tetrahidropirimidin olup, özelligi; tetrahidropirimidinin ectoin veya hidroksiectoin olmasi ile karakterize edilir.
9. Bir madde olup, özelligi; Istem 1 - 8iden birine göre kullanim amaçli, 10 - tetrahidropirimidin içermesidir.
10. Topikal uygulama için kullanim amaçli bir çözelti olup, özelligi; Istem 1 - 9'dan birine göre, agirlikça %0,1 - 25, tercihen agirlikça %0,5 - 5 miktarda fizyolojik açidan kabul edilebilir bir tetrahidropirimidin içermesidir.
