(54) СПОСОБ БИЛИОДИГЕСТИВНОГО ШУНТИРОВАНИЯ(54) METHOD OF BILIODYSTICK BUNCHING
Изобретение относитс к области медицины и может быть использовано дл хирургического лечени больных с патологией желчных путей. Известен способ билиодигестивного шунтировани , включающий наруж ное дренирование желчных путей, отведение желчи в пищеварительный тракт l . Однако известный способ вл етс недостаточно эффективным и возможны послеоперационные осложнени . Цель изобретени - предупреждение послеоперационных осложнений и повышение эффективности способа. Эта цель достигаетс тем, что дополнительно дренируют двенадцатиперстную кишку, оба дренажа соедин ют и непрерывно отвод т желчь в кишечник . Способ осуществл ют следукадим о разом. При операции папиллосфинктеротомии у больного с узким холедохом через дуоденотомное отверстие, обр зованное дл рассечени большого ду денального сосочка, ввод т длинный изогнутый зажим с сомкнутыми браншами . Конец зажима провод т через привратник и вып чивают переднюю стенку антрального отдела желудка. В области вып чивани накладывают кисетный шов, в центре которого производ т гастротомию на прот женин 5 мм. Выведенные кончики зажима захватывают и провод т в двенадцатиперстную кишку резиновый дренаж с внутренним просветом 3 мм. Дистальный конец дренажа устанавливают у перехода вертикальной ветви двенадцатиперстной кишки в нижнюю горизонтальную . Кисетный шов зат тивают, формируют герметичный серозно-мускул рный канал вокруг дренажа длиной 2-2,5 см. Проксимальный конец дренажа вывод т наружу через отдельный прокол брюшной стенки. Основание свища фиксируют к передней брюшной стенке. Затем следуют приемы, направленные на ушивание дуоденотомного отверсти ,дренирование общего желчного протока по одному из известных методов. В первые 2-3 сут после операции дренажи открыты, наход тс в положении сифон. Вследс твие эвакуации содержимого желудка и двенадцатиперстной кишки наружу у больных отсутствуют тс анота, рвота, метеоризм, отпадает необходимость трансназального дренировани . Без каких-либо дополнитесьных меропри тий уже через 28-36 час восстанавливаетс перистальтика кишечника и самосто тельное отхождение ,газов. С момента восстановлени мо торно-эвакуаторной функции желудочнокишечного тракта дуоденальный дренаж временно перекрывают. На 4-5 сут п.осле операции, предварительно укоротив концы, производ т соединение дренажей общего желчного протока и двенадцатиперстной кишки.стекл нным тройником, один из просаетов которого служит дл холангио- и луоденоманодебитометрии, холангиографии, пробы с метиленовой синькой. Зажим Моро, наложенный на дуоденальный дренаж, позвол ет по мере улучшени проходимости терминаль ного отдела холедоха уменьшать сброс желчи по шунту. Возможна в определенной степени саморегул ци системы: при улучшении проходимости терминаль ного отдела холедоха большее количество желчи следует естественным пу тем, при ухудшении его проходимости увеличиваетс сброс желчи по шунту, предупрежда развитие желчной гипертензии и св занных с нею осложнений. Удаление дренажа производ т на следующий лень после удалени дренажа холедоха. Предложенный способ несложен Б осуществлении, позвол ет отводить желчь в пищеварительный тракт непрерывно , управл емо, контролируемо, предупрежда осложнени со стороны дыхательных путей, глотки, пищевода, экономит рабочее врем медицинского персонала и практически применим у любого больного с показани ми к этому вмешательству.
