Abstract
The invention relates to medicine, namely to surgery, and can be used when applying a pancreatojejunal anastomosis after pancreatoduodenal resection. The aim of the invention is to prevent the occurrence of anastomotic inconsistency. Pancreatoduodenal resection is performed so that the level of resection is located proximal to the extreme pancreatic branch of the splenic artery. The pancreatic stump is mobilized 1.5–2 cm distal to the site of entry into the parenchyma of the extreme pancreatic branch of the splenic artery by dissecting the transitional fold of the parietal peritoneum along the upper and lower margins. Then the stump is invaginated into the lumen of the prepared jejunum, placing the first row of sutures distal to the outermost the preserved pancreatic branch of the splenic artery, and the second between the indicated artery and the subsequent branches of the splenic artery. The proposed method was applied for pancreatoduodenal resection in 6 patients. The method allows to increase the reliability of the anastomosis.
Description

Vol. T., 38 years old, was operated on for a disease of the pancreatic head. Produced pancreatoduodenal resection with preservation of the extreme pancreatic net splenic artery. Formed invaginated pancreatojejunostomy by type end to end. At the same time, the first row of stitches was placed distapnee | g of the extreme pancreatic branch of the splenic artery, and the second between the indicated artery and the next pancreas with the splenic artery. The postoperative period is smooth. Discharged in satisfactory condition.
