Abstract
Description
The invention relates to medicine, namely to dentistry, and can be used for the treatment of periodontitis.
Currently, infectious and inflammatory diseases of the periodontium are one of the main reasons for patients to visit a dentist. There is a high incidence among all age groups. The progression of inflammation in periodontal tissues due to incorrect therapy or its absence leads to tooth loss. The presence of foci of chronic infection also has a systemic effect, contributing to microbial sensitization and a decrease in the reactivity of the immune system.
The main cause of the development of the disease in periodontal tissues are microorganisms of dental plaque. Pathogenic effects occur due to the ability of bacterial waste products to have a damaging effect on periodontal epithelial cells and trigger a cascade of immune responses. Periodontitis is an inflammation of the tissues surrounding the tooth, occurring with a violation of the periodontal connection and destruction of bone tissue. There are three clinical and morphological stages of the disease: mild, moderate and severe.
One of the components of the local therapy of infectious and inflammatory periodontal diseases is local antiseptic therapy. A key factor contributing to the decrease in the effectiveness of drugs used in medicine and in particular dentistry is the proven ability of bacteria to form a biofilm. Colonies of bacteria immersed in the extracellular matrix form the so-called biofilm or biofilm, which significantly reduces their susceptibility to drugs and allows for a long time to provide a chronic inflammatory process. Thus, there is a need to find a composition that can destroy the biofilm and affect microorganisms, and methods its application.
The prior art patent RU 2545800, publ. 04/10/2015, offering treatment in two stages, and first, at the first stage, the periodontal pockets are washed with a 1.5% hydrogen peroxide solution with a jet cannula, passing the cannula along the periodontal pockets around the teeth, and when the furcation of the premolars and molars is exposed, with the cannula entering under the furcation. Then the periodontal pockets are washed in the same way with an aqueous infusion of the herb of sage officinalis daily for 3-5 days, then with an aqueous infusion of chamomile flowers daily for 3-5 days, then with an aqueous infusion of psyllium leaves daily for the next 3-5 days, after each washing the mucous membranes of the gums are applied with cotton turundas with the corresponding water infusion of herbs introduced by the cannula with an exposure of 1 hour, in addition, mouth baths of the corresponding water infusion of herbs introduced by the cannula are prescribed daily 5-7 times a day. At the second stage, Normoflorin-B or Bifidumbacterin is administered by cannula instillation, passing the cannula along the periodontal pockets around the teeth, when the furcation of premolars and molars is exposed, with the cannula entering under the furcation, at the same time, the mucous membranes of the gums are covered with cotton turundas with this drug, with an exposure of 2 hours daily for 5 -10 days.
The disadvantages of this method are the abundance of manipulations, prolonged exposure of the drug, the inconvenience of home use due to the need for a large number of trays per day. Also, the risk of developing allergic reactions to plant components is not excluded.
A known method for the treatment of inflammatory periodontal diseases (RU 2372949, publ. 20.11.2009), which consists in washing the periodontal pockets with a 0.05% solution of miramistin. Then a gum dressing is applied containing a mixture of a 2% solution of calcium chloride, juice of kolanchoe and stinging nettle, glucosamine hydrochloride, dimethyl sulfoxide and, as a gel base, an alloy of polyethylene oxides PEO-400 and PEO-4000 with glycerin. Through a gum bandage, periodontal tissues are exposed to a low-frequency magnetic field with an intensity of 12-17 MT from the Polus-1 apparatus for 20 minutes.
The disadvantage of the described method of therapy is the use of a magnetic field, which has a number of limitations in use, for example, if patients have metal crowns, prostheses, implants. This does not allow the widespread use of this method of treatment in the population.
The closest analogue of the invention is a method for the treatment of periodontitis (RU 2480228, 04/27/2013), which consists in carrying out curettage of periodontal pockets and their antiseptic treatment, conducting local treatment with "Forest Balsam" - applications on the area of inflammation in the amount of 0.05-0.2 grams and insertion into periodontal pockets. The duration of the procedures is 15 minutes twice a day, the course of treatment is 5-6 days.
The disadvantage of this method is the weak activity of the herbal preparation against biofilms. Also, the technique is not convenient for patients due to the need for the procedure at the doctor twice a day.
The technical result of the present invention is to increase the effectiveness of the treatment of periodontitis through the use of an antiseptic composition that does not have a local irritating and toxic effect, minimal invasiveness and simplification of the procedure, reducing the healing time.
The technical result is achieved due to the fact that the method for treating periodontitis of mild and moderate severity provides for the use of an antiseptic composition in the form of a gel containing 0.1 wt.% polyaminopropyl biguanide. After removal of supra- and subgingival dental deposits using an ultrasonic scaler and a brush with paste, the said composition, heated to a temperature of 26-28 ° C, is introduced into each periodontal pocket in the amount of 1.5-2.0 g, while its removal not made out of pocket. Next, home use of the specified composition is prescribed in the form of applications using a universal mouthguard. Applications are carried out once a day, after eating and brushing your teeth with an exposure of 40 minutes, the course of application is 14 days.
From the prior art, various methods are known for removing supra- and subgingival dental deposits, including the use of ultrasonic scalers and various pastes, for example, Detarthrin paste. In the method according to the invention, pastes of any manufacturer suitable for removing dental deposits and not damaging the enamel can be used.
It should be noted that for the first time in dental practice, it is proposed to use a gel composition containing polyaminopropyl biguanide as an active component in an amount of 0.1 wt.%. The authors of the present invention unexpectedly found that the use of an antiseptic gel composition containing 0.1 wt.% polyaminopropyl biguanide, allows you to increase the effectiveness of the treatment of periodontitis, including by suppressing bacterial adhesion and destruction of biofilms.
13.02.2019.The composition used in the present invention, containing as an active ingredient polyaminopropyl biguanide in an amount of 0.1 wt.%, may include any carriers and excipients suitable for pharmaceutical use. The composition in the form of a gel may be prepared by any of the techniques known in the art. As such a composition can be used gel composition "Prontosan", which includes 0.1 wt.% polyaminopropyl biguanide. A similar composition is disclosed, for example, in EP 3439641 A1, publ. 02/13/2019.
In addition, it was unexpectedly found that the introduction of the specified composition into the periodontal pocket, heated to a temperature of 26-28°C, makes it possible to increase the antiseptic effect of the composition, which contributes to a more effective destruction of the biofilm and, as a result, an accelerated decrease in bacterial aggression and an accelerated regenerative process.
A further procedure for daily home use of the indicated composition in the form of applications using a universal mouthguard with an exposure of 40 minutes for 14 days made it possible to achieve a decrease in the depth of the pocket or its absence, depending on the severity, immobility of the teeth, good general condition of the patient (data of a general clinical and radiological examination ), the gum has acquired a uniformly pale pink color and is firmly attached to the surface of the tooth. Also, dynamic observation for 6-12 months showed the absence of relapses (no bleeding, swelling, redness of the gums), strengthening of the teeth, restoration of their function, the appearance of foci of bone tissue compaction, especially in those areas where osteoporosis was previously detected.
The present invention is illustrated by the following clinical examples of the use of a pharmaceutical composition.
р. Example 1. Patient S., born in 1973. complained of bleeding and pain when brushing teeth and eating, bad breath. Objectively: supra- and subgingival hard and soft plaque is noted, the gingival mucosa is hyperemic, edematous, bleeding is noted during probing, periodontal pockets up to 4 mm deep are determined, tooth mobility is 1.6, 1.7, 2.6, 2.7, 3.5, 3.6, 3.7, 4.6, 4.7 - I degree. Indexes: IG (hygiene index) OHI-S=2.5 (unsatisfactory), PMA (papillary-marginal-alveolar index)=68%, SBI (bleeding index)=100%. On OPTG - there is bone resorption up to ½ of the length of the roots of the teeth. Diagnosis - chronic generalized periodontitis of moderate severity in the acute stage.
A comprehensive treatment was carried out: training in individual oral hygiene, removal of supra- and subgingival hard and soft dental deposits, brushing with paste, a composition heated to 26-28°C (Prontosan gel) was introduced into each PC. Applications were prescribed with the above gel composition using universal mouthguards with an exposure of 40 minutes once a day after meals and brushing teeth, the duration of the course is 14 days. A general vitamin complex was prescribed. After the course of therapy, objectively: the mucous membrane of the gums is pale pink, the gingival papillae are more tightly attached to the surface of the teeth, there is a decrease in tooth mobility, as well as a decrease in the depth of periodontal pockets. IG=0.6 (good), PMA=25%, SBI=25%. When examined after 6 months and after 1 year, no recurrence of periodontitis was observed.
р. Example 2.
