Abstract
Description
The invention relates to medicine, namely to pediatric dentistry, and is intended for the prevention of dental caries in children during a single-session sanitation of the oral cavity through a single use of an antifungal drug.
Dental caries is the most common disease in children. It is known that the cause of caries in children is dental plaque (dental plaque). Dental plaque is the most complex and multicomponent biotype of the oral cavity, which includes almost all representatives of the oral cavity. Dental plaque itself is a kind of biofilm formed by accumulations of bacteria in a conglomerate of proteins and polysaccharides. Plaque formation begins with the interaction of acidic glycoproteins of saliva with Ca2+ ions of tooth enamel, while the main groups of glycoproteins react with hydroxyapatite phosphates to form pellicles on the tooth surface. The biofilm facilitates microbial colonization of the tooth surface and streptococci appear first, and then other representatives of the microflora. Soft plaque does not adhere tightly to the cells of the oral cavity, so it is relatively easy to remove. If this is not done, plaque contributes to the formation of plaque. Streptococci promote the decomposition of carbohydrates to the formation of dextran, the main adhesive component of dental plaque, and levan, which further decomposes into acids. The rate of formation and cariogenicity of dental plaque depends on its microbial composition.
Mutans и Сandida alb. Установлено, что наличие в полости рта грибов рода Сandida способствует увеличению кариесогенности зубного налета и увеличивает скорость его образования (Falsetta et al., 2014; Hwang et al., 2017, Yang et al., 2012; Xiao et al., 2016; Xiao et al., 2018).Many researchers have established a direct connection between the concentration of Str. Mutans with the development of dental caries (Kneist S., Maslak E., Tsare R., Berzina S., Skrivele S., Terekhova T., Shakovets N., Wagner M., de Moura-Sieber V., de Moura R., Berutta A. Biological and social risk factors for early childhood caries // Modern Dentistry. 2011. No. 1. P. 62-65).. Recently, evidence has appeared of the influence of symbiotic microflora Str. Mutans and Candida alb. in the etiopathogenesis of dental caries. It has been established that the presence of fungi of the genus Candida in the oral cavity contributes to an increase in the cariogenicity of dental plaque and increases the rate of its formation (Falsetta et al., 2014; Hwang et al., 2017, Yang et al., 2012; Xiao et al., 2016; Xiao et al., 2018).
2017;16(4):28-31).The effectiveness of one-session (one-step) sanitation of the oral cavity on the risk of developing dental caries in children has been shown (Satygo E., Reutskaya K. The influence of one-step sanitation of the oral cavity on the risk of developing caries of primary teeth in children 3-5 years old. Pediatric Dentistry and Prevention. 2017; 16(4):28-31).
However, there is still a need to search for means and methods in the pathogenetic prevention of caries.
The closest to the claimed invention is a method of nonspecific prenosological prevention and treatment of dental caries in young children (Method of nonspecific prenosological prevention and treatment of dental caries in young children: patent RU2661612, Russian Federation, application RU2017128961, application 2017.08.14, publ. 2018.07 .17), including a preliminary complex of dental examinations and microbiological diagnostics of contamination with bacteria Streptococcus mutans on the lingual surface of temporary teeth of the lower jaw and the mucous membrane of the middle third of the back of the tongue in children, the use of colloidal silver applications with a concentration of silver nanoparticles Ag+30 ppm in the form of a gel, different the fact that in the group with acute and chronic caries, the application of gel with nanosilver is carried out directly into biotopes - sources of Streptococcus mutans, with a viscose swab applicator, each time in the amount of two drops: the first drop of gel with a diameter of 2.5-3 mm is squeezed out of the bottle, sequentially applied to the lingual surfaces of the teeth of the lower jaw on the left and then on the right, the second drop is applied to the middle third of the dorsum of the tongue in a circular motion from the center to the periphery, from left to right, capturing the entire surface of the mucous membrane of the middle third of the dorsum of the tongue, while during the basic course the colloid is applied silver 2 times a day in the morning on an empty stomach and in the evening before bedtime for 7 days, the next 7 days break, a total of 3 months, in the morning they first hygienically brush their teeth, give the child 30.0-50.0 ml of clean water and after 5-7 minutes sequentially apply a gel with Ag+ nanoparticles to the lingual surface of the teeth and dorsum of the tongue, eat food after 20-30 minutes; in the evening before bed, no earlier than an hour after eating and/or drinking, hygienic brushing of teeth is carried out, the child is given 30.0-50.0 ml of clean water and after 5-7 minutes, sequential treatment of the lingual surfaces of the teeth of the lower jaw and the back of the tongue, apply a gel with Ag+ nanoparticles; The maintenance course lasts 3 months, for 14 days, colloidal silver is applied once a day before bedtime, the next 14 days are a break, this allows you to achieve no increase in caries at the stages of dispensary observation.
д.The disadvantages of this method include its complexity in execution, duration, as well as the large dependence of effectiveness on the compliance of patients, in particular parents and children. This is due to the large number of actions required from the patient, as well as their complexity, characterized by the need to take into account a large number of time ranges, etc.
The technical problem is the need to develop an easy-to-implement and effective method for the prevention of dental caries during one-session sanitation of the oral cavity in children.
The technical result consists in simplifying the process of preventing dental caries during one-session sanitation of the oral cavity in children while ensuring high efficiency.
The technical result is achieved by the fact that in the method of preventing dental caries during one-session sanitation of the oral cavity in children, during which medical dental care is provided to children with sanitation of the oral cavity, according to the invention, a bite block is installed between the teeth of the upper and lower jaw, a mouth retractor is installed, and also install a cotton roller on the side of the tongue between the lateral surface of the tongue and the teeth of the lower jaw, after which the teeth, gums and tongue are irrigated on one side with a 1% Candide solution and the remaining solution is removed using an aspiration system, then, if present, on the side on which treatment is carried out, teeth with complicated forms of caries are treated with the installation of fillings or crowns if necessary, and then after treatment, the bite block, mouth expander and cotton roller are removed, the installed fillings and crowns are checked for occlusion when installing them, after which the same is carried out manipulations, starting with the installation of a bite block on the other side, after all manipulations, the oral cavity is washed with the water-air system of the dental unit, and then water is collected from the oral cavity with a saliva ejector.
In the most preferred embodiment of the invention, an Optragate mouth opener is used. The use of this type of mouth retractor has an additional positive effect on simplifying the process of preventing dental caries during one-session sanitation of the oral cavity in children, while ensuring high efficiency by ensuring correct fixation and a good overview for the doctor. Also, in one of the preferred embodiments of the invention, before installing the bite block, the patient is placed in a state of deep sedation. This makes it possible to use the proposed method in the presence of complicated forms of dental caries, for example, pulpitis.
The proposed technique improves the hygiene index, reduces the cariogenicity of dental plaque, thereby reducing the increase in the intensity of dental caries in children. In addition, it is important that this technique does not depend on patient compliance, which significantly increases the effectiveness of dental care using the proposed method as a whole. With a single irrigation of the child's oral cavity before a single-session sanitation of the oral cavity, an etiopathogenetic preventive mechanism for the prevention of caries is implemented - reducing the cariogenicity of dental plaque: firstly, the number of foci infected with Str.mutans decreases due to sanitation of the oral cavity; secondly, the number of Candida alb. colonies is reduced due to treatment of the oral cavity with an antifungal agent. The influence of the symbiotic flora Str.mutans and Candida alb. is known. in the etiopathogenesis of dental caries.
Such a method as a single irrigation of the oral cavity with an antifungal agent before a one-session sanitation of the oral cavity is well controlled by the doctor; as part of the implementation of the method, ingestion of the drug is excluded, but sufficient exposure is realized during dental procedures. This allows you to obtain a sufficient therapeutic effect without depending on the patient’s competence. This method can be implemented in the treatment of children both under conditions of deep sedation and without it.
The inventive method is carried out as follows.
During the implementation of the method, medical dental care is provided to children with oral sanitation. To do this, install a bite block between the teeth of the upper and lower jaws, install a mouth opener, and also install a cotton roller on the side of the tongue between the lateral surface of the tongue and the teeth of the lower jaw. Preferably, an Optragate mouth opener is used. If a bite block is necessary, the patient is placed into a state of deep sedation. Next, the teeth, gums and tongue are irrigated on one side with a 1% Candide solution and the remaining solution is removed using an aspiration system. If there are teeth with complicated forms of caries on the side on which the treatment was carried out, dental treatment is carried out with the installation of fillings or crowns if necessary. After treatment, the bite block, mouth expander and cotton roller are removed, and the installed fillings and crowns are checked for occlusion when installing them. Next, similar manipulations are carried out, starting with the installation of a bite block on the other side. After all manipulations, the oral cavity is washed with the water-air system of the dental unit, and then water is collected from the oral cavity with a saliva ejector.
The inventive method is illustrated with examples.

They were divided into three groups. In the first group, children were treated with dental caries prevention according to the proposed method; in the second group, children had their oral cavity sanitized in one session under conditions of deep sedation without pre-treatment with an antimycotic drug. In the third group, children were sanitized in multiple sessions in an outpatient setting, that is, 1 tooth per session.
After three months, in the first group of children only 11% of children had a positive test, in the second group - in 54% of children, and only in the third group did 100% of children maintain the same test results.
Test results on Str. Mutans after 1 year were distributed as follows:
В первой группе показатель теста был положительный у 13 % детей, во второй группе - у 62% детей, а в третьей - у 100% (таблица 1).In the first group, the test was positive in 13% of children, in the second group - in 62% of children, and in the third - in 100% (Table 1).
Candida albicans. In addition, the children in all groups were assessed for the presence of Candida albicans.
У 100% детей трех групп до лечения была выявлена концентрация Candida albicans в слюне свыше 1,54±0,23 lgКОЕ/мл (%)In 100% of children of three groups, before treatment, a concentration of Candida albicans in saliva was detected above 1.54±0.23 lgCFU/ml (%)
После лечения такая же концентрация сохранилась в течение года у детей второй и третьей групп, тогда как у большинства детей первой группы через 3 месяца и через год концентрация Candida albicans в слюне была ниже 1,54±0,23 lgКОЕ/мл(%)( табл.2)After treatment, the same concentration remained for a year in children of the second and third groups, while in the majority of children in the first group, after 3 months and a year, the concentration of Candida albicans in saliva was below 1.54 ± 0.23 lgCFU/ml (%) ( table 2)
Additionally, the hygiene index was studied.
У детей третьей группы индекс гигиены за время исследования не изменился, у детей второй группы после одноэтапной санации полости рта индекс гигиены улучшился, тогда как у детей первой группы индекс гигиены улучшился значительно (таблица 3).In children of the third group, the hygiene index did not change during the study; in children of the second group, after one-stage sanitation of the oral cavity, the hygiene index improved, while in children of the first group, the hygiene index improved significantly (Table 3).
The increase in caries intensity was also analyzed.
Прирост интенсивности кариеса у детей первой группы через год составил 0,54, у детей второй группы 1,98, у детей третей группы - 4,89.The increase in the intensity of caries in children of the first group after a year was 0.54, in children of the second group 1.98, in children of the third group - 4.89.
2. Example 2.
Patient M, 3.5 years old, came to the pediatric dentistry clinic with his parents. 8 carious cavities were found in the oral cavity, the oral hygiene index was 68%, the concentration of Candida albicans in saliva was over 1.54±0.23 lgCFU/ml(%), test for critical content of Str. Mutans in saliva over 5×10 5 CFU/ml (%).
A one-session sanitation of the oral cavity was performed under sedation according to the claimed method. This involved placing a bite block between the maxillary and mandibular teeth, an Optragate mouth spreader and a cotton roll on the side of the tongue between the side of the tongue and the mandibular teeth. Afterwards, the teeth, gums and tongue on the left side were irrigated with a 1% Candide solution and the remaining solution was removed using an aspiration system. Next, treatment of advanced caries was carried out on teeth 6.4, 6.5, 7.5, 7.4. Moreover, fillings were installed on teeth 6.5 and 7.5, and crowns were installed on teeth 7.4 and 6.4. After treatment, the bite block, mouth expander and cotton roller were removed, and the installed fillings and crowns were checked for occlusion. Next, similar manipulations were performed on the right side and fillings were installed on teeth 5.5,5.4,8.4,8.5. Next, the oral cavity was washed with the water-air system of the dental unit, then the water was collected from the oral cavity with a saliva ejector.
Mutans в слюне ниже 5×105 КОЕ/мл (%).After 1 year, the increase in caries was 0, the hygiene index was 38%, the concentration of Candida albicans in saliva was below 1.54 ± 0.23 lg CFU/ml (%), test for the critical content of Str.
