Abstract
Description
The invention relates to medicine, in particular to neurosurgery, and can be used in the neurosurgical treatment of patients with complex skull defects, combined with cicatricial changes in the scalp.
- №1. - С. 72-76).Closest to the claimed is a method of transcutaneous cranioplasty for complex defects of the skull bones, which consists in using titanium perforated plates (Reconstructive alloplasty of skull bones defects with titanium implants. - Kubrakov K.M., Karpuk I.Yu., Fedukovich A.Yu., 2011 Surgery News . - No. 1. - S. 72-76).
The disadvantage of the method selected as a prototype is a large injury during surgery, since it is necessary to expose the defect of the skull, as well as the surrounding surface of the bone for attaching the plate.
Another disadvantage is that an increase in access size increases the time of surgery, the amount of intraoperative blood loss and increases the risk of flap ischemia with subsequent necrosis. The overall hospital stay and the risks of postoperative complications such as bleeding, flap necrosis, divergence of the edges of the postoperative wound with the formation of fistulas also increase.
A very important disadvantage of this method is the complexity, and sometimes the inability to use with severe scarring of the scalp with its thinning.
The objective of the invention is the low invasiveness of the intervention while reducing postoperative complications and the creation of conditions for intervention in cicatricial deformity of the scalp.
The technical result of the task is achieved by the fact that in the method of cranioplasty for complex defects of the skull bones, based on the incision and fixing the titanium plate with screws, the incision is made according to the size of the titanium plate, the titanium plate is inserted through this section, the skin is punctured over the titanium plate and inserted through punctures fixing screws.
Performing a cut according to the size of the titanium plate allows you to reduce the size of the cut, i.e. operating wound surface, which reduces the amount of blood loss, the risks of postoperative complications and reduces the healing time of the wound.
Performing skin punctures over the titanium plate and introducing fixing screws through the punctures reduces the traumatic effect on the skin flap.
The method is as follows. In the area of the skull defect, 2 cm away from its edge, an arcuate skin incision is made, of such a size that through it it is possible to insert an implant made of titanium in the form of a spherical network.
The bone is skeletonized through an incision made along the edge of the defect, then the institution is made through the incision of the implant, and screws are fixed to the edge of the bone defect. In the region of the implant edge closed by the skin, a puncture percutaneous screw insert is performed with fixation of the titanium implant.
Example 1. Patient L, 50 L., was admitted to the Research Institute of Emergency Medicine. I.I. Janelidze 09/20/2017 with a diagnosis of a condition after decompressive trepanation of the skull, post-trepanation defect of the left temporal and parietal bones. History: performing decompressive trepanation of the skull, removal of subdural hematoma with complications in the postoperative period in the form of suppuration of the wound with the formation of a fistula and further cicatricial deformation of the skin in the area of the postoperative scar.
Due to the presence of a post-trepanation defect, it was necessary to perform cranioplasty, however, due to the presence of cicatricial deformity of the scalp in the area of upcoming implantation, it was not possible to use standard types of cranioplasty.
Performed cranioplasty by the claimed method. The operation was performed without intraoperative complications. The wound healed by primary intention. Screw entry points - no inflammation. The sutures were removed on the eighth day, and the patient was discharged in satisfactory condition.
Example 2. Patient M, 60 l. entered the Research Institute of Ambulance them. I.I. Janelidze 04/11/2018 with a diagnosis of a condition after decompressive craniotomy, post-trepanation defect of the right temporal bone. In the anamnesis: performing decompressive trepanation of the skull, removal of a volumetric formation of the brain with subsequent radiation therapy, which resulted in post-radiation necrosis of the scalp in the area of surgery with cicatricial deformity.
The patient received a planned operation in the amount of cranioplasty with a titanium mesh implant. Performing cranioplasty by an open method was undesirable, due to the presence of extremely thinned and scarred skin (postradiation changes), therefore, it was decided to perform cranioplasty by the claimed method. The operation was performed without intraoperative complications. The wound healed by primary intention. Screw entry points - no inflammation. Sutures were removed on time. The patient was discharged in satisfactory condition.
The inventive method allows cranioplasty with minimal trauma to the skin flap, minimal risk of developing intraoperative complications in the form of bleeding, and also reduces the risks of such complications in the postoperative period as: necrosis of the skin flap, inconsistency of sutures, the formation of fistulas, suppuration of the postoperative wound, the healing time of the wound is reduced and hospitalization of the patient.
The method allows performing surgical treatment in patients with complex defects of the skull, scarred skin of the head, in which the implementation of cranioplasty completely open way is impossible.
