Abstract
Description
The invention relates to the pharmaceutical industry and is a combined pharmaceutical composition of antiseptic action for the treatment of infected wounds of various origins.
The problem of the healing of purulent wounds is a serious, urgent and unresolved problem in medicine. The number of postoperative purulent complications is about 30% after planned and emergency surgical interventions and has no tendency to decrease. One of the areas of search for an effective method of treating infected wounds was the development of local multi-functional agents containing several medicinal components with complex therapeutic activity in relation to the main substrates of a complex, nonhealing wound. In the light of the above, it is relevant and necessary to develop a drug that can clear the wound surface from purulent necrotic masses, provide an antimicrobial effect, minimize pain syndrome and, ultimately, heal the wound as soon as possible.
There is a patent №2323748 (from 21.02.2006) "Medical bandage containing a complex of enzymes from crab hepatopacreas, and the method of its production." The dressing consists of cellulose, modified by periodate oxidation and treated with acid-soluble chitosan, with an immobilized proteolytic complex based on hepatopancreas crab.
Известен «Материал, обладающий биологической активностью, способ его получения и терапевтическое средство на его основе» (патент №2357753 от 24.03.2005), который представляет собой модифицированную целлюлозу с соиммобилизованными на ней тремя лекарственными средствами (трипсин, лизоцим, инсулин).The “Material with biological activity, a method for its preparation and a therapeutic agent based on it” (patent No. 2357753 of March 24, 2005), which is a modified cellulose with three drugs co-mobilized on it (trypsin, lysozyme, insulin), is known.
Известен «Способ получения аппликации атравматической одноразовой», в котором раскрывается способ получения раневого покрытия, содержащего сухую гелевую пластину на основе полиакриламидного геля (ПААГ) с гемостатическим порошком (патент №2448738 от 24.11.2009).The “Method of obtaining an atraumatic single-use application” is known, which discloses a method for obtaining a wound cover containing a dry gel plate based on polyacrylamide gel (PAAG) with a hemostatic powder (patent No. 2448738 of November 24, 2009).
The direct analogs of the proposed drug are unknown to the authors. To indirect analogues, i.e. Analogues for their intended purpose include Levomekol ointment, Baneocin powder, Solcoseryl ointment and gel. However, the above preparations do not contain proteolytics, without which a complete cleansing of the wound from non-viable tissues and exudate is impossible, therefore, there are no favorable conditions for the formation of granulations and regeneration. There are also no anesthetics. Thus, a new wound-healing agent will significantly complement today's arsenal of tools for the care of complex wounds.
As already noted, in the modern pharmaceutical market of Russia there are drugs created for the treatment and healing of infected wounds, including combinations.
For example, in patent RU2115418 ("Combined chemotherapeutic agent" butol "for the treatment of local wound infection and inflammatory diseases" dated January 23, 1998) a combined chemotherapeutic agent for the treatment of local wound infection and inflammatory diseases containing the antiseptic-quinosol, characterized in which additionally contains the antibiotic Streptomycin. However, this pharmaceutical composition is made in the form of a solution, which significantly affects the prolongation of the action of the composition, as well as its bioavailability. In addition, the tool does not meet the criteria described above.
Another drug that is actively used in the treatment of infected wounds is Levomekol (Produced by OAO Nizhfarm, Russia). This combined preparation for topical use has anti-inflammatory and antimicrobial action, is active against gram-positive and gram-negative microorganisms (staphylococcus, pseudomonas and intestinal sticks). Easily penetrates deep into tissues without damaging biological membranes, stimulates regeneration processes. In the presence of pus and necrotic masses, the antibacterial effect is maintained. However, the spectrum of action on the pathogenic microflora of the active substances contained in the preparation is not wide enough. With the use of ointment "Levomekol" possible allergic reactions in cases of hypersensitivity to the drug.
Known from the prior art means are not without drawbacks and have a fairly high adhesion to the wound surface, which in some cases leads to injury of the damaged surface, bleeding and pain.
The aim of the present invention is to eliminate these disadvantages.
The claimed composition with complex therapeutic activity based on derivatives of glucosamine and acrylic polymers for the treatment of infected wounds of various origins is a translucent gel based on hydroxypropylmethylcellulose and polyacrylamide with the inclusion of chitosan, himopsin, miramistin and lidocaine hydrochloride as pharmaceutical agents. The mechanism of action of the developed drug is multifunctional and includes the following therapeutic effects: enzymatic biochemical cleansing of the wound due to lysis of denatured proteins without damaging healthy tissues, indirect antimicrobial action (himopsin) by reducing the number of pathogenic microorganisms due to the lysis of their nutrient medium; full antimicrobial action due to miramistin and pain relief not only due to the cooling effect of the gel, but also due to the inclusion of lidocaine in the composition of the drug.
The therapeutic effect is achieved through a combination of various types of biological activity of the composite components of the gel. Each medicinal and biologically active substance contributes to the acceleration of the process of purification and healing of wounds:
Молекулы препарата воздействуют на наружную оболочку микробной клетки, что приводит к ее разрушению и гибели.- Miramistin has a pronounced bactericidal action against aerobic and anaerobic bacteria, gram-positive and gram-negative organisms, both in the form of monocultures and in the form of associations, including hospital strains with multi-resistance to antibiotics. The drug molecules affect the outer shell of the microbial cell, which leads to its destruction and death.
Химопсин восстанавливает микроциркуляцию в стенках раны, улучшает обменные процессы, что клинически проявляется уменьшением местного воспаления.- The components of himopsin cleave various peptide bonds in the protein molecule: trypsin hydrolyzes peptide bonds containing arginine and lysine residues; chymotrypsin cleaves peptide bonds with aromatic amino acid residues - tyrosine and tryptophan. Himopsin restores microcirculation in the wound walls, improves metabolic processes, which is clinically manifested by a decrease in local inflammation.
- Хитозан является носителем ферментного комплекса и мирамистина, обеспечивает пролонгированное терапевтическое действие фермента и антисептика, обладает бактерицидными свойствами и оказывает ранозаживляющее действие.- Chitosan is a carrier of the enzyme complex and Miramistina, provides a prolonged therapeutic effect of the enzyme and antiseptic, has bactericidal properties and has a wound-healing effect.
Механизм местноанестезирующего действия проявляется в угнетении нервной проводимости за счет блокады каналов натрия в нервных волокнах и окончаниях.- Lidocaine hydrochloride is a local anesthetic used for conduction, infiltration, terminal anesthesia. The mechanism of local anesthetic action is manifested in the suppression of nerve conduction due to the blockade of sodium channels in the nerve fibers and endings.
Thus, the pharmaceutical composition based on two substances for the treatment of infected wounds has four types of pharmacological effect - necrolytic, antimicrobial, wound healing and anesthetic. The gel form of the drug has an indirect analgesic effect due to the cooling effect when applied to the damaged surface.
In the case of developing an external medicinal product for treating wounds, an important role is played by the choice of both the dosage form itself and the excipients forming it, based on the ultimate goal — the maximum bioavailability of the biologically active ingredient in the medicinal product when applied to the damaged surface — a wound defect.
The means of local treatment of wounds should have complex multi-aspect effects, which are realized both at the expense of the active components (the active principle), as well as at the expense of the basis of the preparation.
Taking into account the peculiarities of the injection site (wound surface), the following requirements are imposed on the intended complex: biocompatibility with human tissues, the absence of allergic reactions, pyrogenic and toxic effects on healthy tissues.
Based on the specifics of the course of the wound process, the composition of drugs used in the first stage of the wound process - the stage of inflammation, it can be assumed that gels of complex composition, having proteolytic, anti-inflammatory, antimicrobial, anesthetic, wound healing, will contribute to cleansing and wound healing.
Summarizing the data on the means used to treat infected wounds, the following components were selected as the main components for the development of the wound healing gel: chromosin proteolytic in combination with chitosan, Miramistin antiseptic in combination with chitosan, lidocaine anesthetic and auxiliary substances — hydroxypropylmethylcellulose, polyacrylate, polyacrylate, polyacrylate, polyacrylate, and
Wet air is optimal for the processes of reparative regeneration of integumentary tissues of humans and animals. The introduction of specific dosage forms into the gel base provides a complex and multifaceted effect on the wound process, while healing takes place under conditions of moderately moist air that is favorable for repair.
Detailed Description of the Invention
Further, the invention is described by examples of its implementation.
Example 1. The technology of obtaining the gel healing
3.On an analytical balance, we weigh 16.0 g of the lyophilized powder of the substance of the chitosan-himopsin enzyme complex (CXX); 14.0 g of lyophilized powder of the chitosan-miramistin complex (CHM); 1.0 g lidocaine; 20.0 g hydroxypropyl methylcellulose (GGTMC); 1.0 g of polyacrylamide (PAA); measure 50,0 ml (62,5 g) of glycerin by the measured cylinder; relative density at 20 ° С is 1.25 g / cm 3 .
The manufacture of the gel begins with the preparation of medicinal substances and bases. The operation of preparing the base includes the process of dissolution and swelling.
In the production tank measure the calculated amount of purified water, preheated to 60-65 ° C. The HPMC powder in an amount weighed on an analytical balance is layered on the surface of the water and left for 2-3 hours to swell and disappear into lumps, then the system is mixed using a propeller mixer until a homogeneous gel is obtained. After that, with constant stirring, add the calculated amount of polyacrylamide with lidocaine anesthetic previously dissolved in it. The introduction of another gelling agent thickens the system and increases the viscosity level.
Pharmaceutical substances KXX and KXM based on acid-soluble chitosan are prepared in advance.
To this end, prepare a 1% solution of chitosan with the addition of acetic acid, the solution is left to swell and structuring for one day. Then add a certain amount of himopsin, at the rate of 2.0 g per 100 g of the drug. In order to incorporate the proteolytic into the chitosan structure, the gel solution is left for two hours at room temperature. Similarly, prepare the substance KHM, adding to the chitosan solution antiseptic Miramistin, at the rate of 0.05 g per 100 g of the drug. The solution is left for two hours at room temperature.
The prepared gel-like solutions are subjected to freeze drying. The lyophilization of gel solutions of enzymes and antiseptics in chitosan is carried out in a frozen state under vacuum, while the water is removed from the frozen substance by sublimation of ice, i.e. converting it to steam, bypassing the liquid phase. Water vapor is pumped out of the process chamber by a vacuum pump and is condensed on the coils of a low-temperature condenser. The dried material retains its structural integrity and biological activity to a greater extent than air-dried films. When moistened, the material restores its original properties. In addition to the listed advantages of this method, there is the removal of excess acetic acid used to dissolve chitosan. Lyophilisates are prepared in advance.
Lyophilisates KXX and KXM in the required quantities, previously dissolved in a small amount of water, are added to the base. The ingredients are thoroughly homogenized using a high speed mixer.
Полученный гель оставляют до полного завершения структурообразования в течение 24 часов для удаления пузырьков воздуха (дегазация).The resulting gel is left to complete the structuring within 24 hours to remove air bubbles (degassing).
In in vivo tests on a flat purulent wound model, Miramistin is active against various pathogens. Already on the third day, the contamination of the wound in the studied groups significantly lagged behind the control group.
In vitro tests, Miramistin has a pronounced biocidal action against both gram-positive and gram-negative bacteria, as well as fungi that cause infectious agents.
Chitosan promotes faster healing of wounds compared to control. In in vitro tests, chitosan contributes to the prolonged action of chemopsin and stabilizes the ego through the formation of a complex with the formation of non-covalent bonds.
The results of the kinetics of inactivation of the native enzyme (inactivation was carried out in the absence of autolysis) and the enzyme-chitosan system at different temperatures show that chitosan is able to stabilize the enzyme, as evidenced by the obtained values of the effective inactivation rate constants of the systems (0 depending on the temperature from 1.2 to 2.0 ). This can occur mainly due to the multi-point binding of the protein molecule with the polysaccharide molecule, which does not affect the active center of the enzyme
Tail Flick. The lidocaine concentration used in the investigational drug has a pronounced antinociceptive effect in the hot plate and Tail Flick tests.
Animals under anesthesia in a sterile environment simulate a purulent wound by the method of P.I. Tolstoy (1976). To do this, the skin that has been shaved from wool is treated with an antiseptic, and skin is cut from the subcutaneous tissue with a diameter of 25 mm. A gauze ball containing 1.2 × 10 9 microbial bodies of a S. Aureus daily culture or 2.6 × 10 9 E. Coli is introduced into the resulting wound, after which the wound is sutured. The next day (24 hours) after modeling, all animals form an abscess with all the characteristic signs of inflammation. After the sutures are removed, the upper skin flap is removed, the gauze swab is removed, the pus is evacuated. The area of the original wound is determined by applying a contour to a transparent film, then the wound is treated with the investigational drug or the comparator Levomekol. To study the healing ability of the drug and control samples applied 24 hours after the modeling of a purulent wound. Preparations are applied for 18 days. During the treatment, the diameter of the wound is measured every three days.
The course of the wound process in experimental animals is evaluated by the planimetric method.
After simulating a purulent wound, rats in all groups of the investigational medicinal product, the comparison drug Levomekol, and wounds without drugs were sluggish during the first two days of the study.
1, 2).Differences in wound healing rate between doses of 0.2 g / 200 g alive. and 1 g / 200 g is alive. as in the group "Investigated drug", and in the group "Levomekol" was not. That is, increasing the dose of drugs more than 0.2 g per 200 g alive. did not have a greater healing effect as with the infusion of S. aureus, and with E. coli (Fig. 1, 2).
3).When a wound is infected with a culture of S. Aureus, the healing rate in the group with the test drug is higher than in the group with the comparator Levomekol (Fig. 3).
4).When a wound is infected with an E.Coli culture, the rate of healing in the group with the test drug is higher than in the group with the comparator Levomekol (Fig. 4).
The data obtained demonstrate the specific action of a drug with a complex therapeutic activity based on derivatives of glucosamine and acrylic polymers for the treatment of infected wounds of various origins on a flat wound model. When S. Aureus and E. Coli are infected, the test drug shows the best result in wound healing compared with the comparator drug.
The studied dosage form has a wide range of antimicrobial action, helps to clean the wound from necrotic tissue and a scab, has an effective wound healing and antinociceptive effect. Thanks to the original composition, the overall effectiveness of the studied drug is higher than the effectiveness of each component separately.
Thus, as a result of research, the high efficiency of the proposed pharmaceutical composition with complex therapeutic activity was shown. The introduction of a drug developed on the basis of glucosamine derivatives and acrylic polymers into practice will allow optimizing the results of treating patients with infected wounds of various etiologies, significantly reducing treatment time, minimizing the drug burden, and reducing the total costs of the state, which is of great social importance.
