Abstract
Description
The invention relates to medicine, in particular to ophthalmology, and can be used to prevent and treat excessive scarring in the area of newly formed outflow pathways of intraocular fluid after anti-glaucoma operations.
На сегодняшний день самый признанный способ решения проблемы избыточной репаративной активности тканей является трансконъюнктивальная ревизия фильтрационной подушки (ФП) - нидлинг (от английского слова needle - игла).The most unfavorable complication of the surgical treatment of glaucoma is obliteration of the area of anti-glaucoma surgery, which leads to an increase in intraocular pressure in the postoperative period. To date, the most recognized way to solve the problem of excessive reparative activity of tissues is the transconjunctival revision of the filtration cushion (AF) - Needling (from the English word needle - needle).
A known method of preventing scarring of the filtration zone using various medicinal substances. The use of antimetabolites and cytostatics: 5-fluorouracil (5-FU) and mitomycin C (MMS) in the postoperative period in the form of subconjunctival injections (Wallace L., 1996, Mardelli P., 1996; Shetty R., 2005; Gutierrez-Ortiz C, 2006). The use of these drugs cannot be widespread due to severe side effects associated with the toxicity of the drugs used: damage to the cornea and sclera, small anterior chamber, suprachoroidal hemorrhage, retinal detachment, late endophthalmitis, cataract progression, development of severe keratitis and endophthalmitis.
A known method of carrying out nidling using a solution of dexamethasone (Immortal A.M. et al. Nidling of a filter pad as a way to increase the effectiveness of fistulizing interventions // VII All-Russian School of Ophthalmologists. Collection of scientific articles. M, 2008, S. 48-50). Corticosteroids reduce the degree of autoimmune inflammatory process by reducing vascular permeability, reducing the number of macrophages and suppressing their functions such as phagocytosis, the release of a number of enzymes and growth factors that reduce the activity of fibroblasts and inhibit wound healing and fibrosis. Patients with non-functioning AFs against the background of compensated and uncompensated IOP perform a needling AF with the introduction of 0.2 ml of dexamethasone solution under the control of a slit lamp.
There is a method of destroying excess adhesions by means of a single AF needling with the introduction of 0.2 ml of a solution of collizin 50 KE / ml in the period from 14 to 30 days after surgical treatment and subsequent daily installations for 10-20 days of a solution of collalysine 50 KE / ml 6 times within 1 hour to maintain the result (patent RU 2465872, September 6, 2011). A prolonged positive result was obtained using this technique. However, a solution of collalysine must be prepared daily, which is difficult for elderly patients.
Ю., Бессмертный A.M. A known method of restoring the outflow of intraocular fluid (VGZ) using finger massage of the eyeball. (Chervyakov A.Yu., Immortal A.M. "Surgical tactics for hypertension after fistulizing operations", J. Glaucoma, 1, 2002, pp. 39-42). The same work speaks of a more complex technology - the use of an argon laser for transconjunctival dissection of sutures that strengthen the scleral flap. But these methods are not applicable at all times and are not always effective enough.
The closest analogue of the proposed method is to carry out the nidling of the filter cushion with the introduction of a photosensitizer into the cavity of the cushion, followed by exposure to coagulating radiation from an argon laser (patent RU 2233146, July 27, 2004). As a photosensitizer, porphyrins of the chlorine series are used, for example, Radachlorin. Under local epibulbar anesthesia, subcutaneous injection needles are injected subcutaneously at a distance of 3-5 mm from the area of the filter pad. Then the needle is advanced in the subconjunctival tissues in the cavity of the pillow and a photosensitizing drug, namely the Radachlorin photosensitizer, is introduced, a 0.35% solution for intravenous administration in an amount of 0.1-0.3 ml. Then, over the entire surface of the pillow, in random order from the periphery to the center, coagulation is performed by an argon laser operating in continuous mode with the following parameters: wavelength 488-514 nm, radiation power 0.3-0.5 W, exposure 0.15-0, 2 s, the diameter of the focal spot is 50-500 microns to obtain a cushion of a given size. Control over the effectiveness of manipulations is carried out visually during biomicroscopic examination. The correctness of the operation is judged by the reduction of the pillow to a given size and by the flattening of its wall.
The task of the invention is the development of the least traumatic method of carrying out a nidling after penetrating anti-glaucoma filter type operations.
The technical result of the proposed method is the elimination of excess interstitial adhesions, cystic degeneration of the filter pad, providing a prolonged hypotensive effect, as well as reducing the morbidity of treatment and reducing the need for repeated operations.
The technical result is achieved by puncture of the cystic wall of the filtration cushion using Nd-YAG laser exposure in the avascular zone on the periphery of the cyst in a certain mode.
It is known that in patients with non-functioning flat, cystic, injected AF and with clearly delimited AF, fluid flow through the newly formed outflow pathways is disturbed, which can cause ophthalmic hypertension in the early postoperative period. At the same time, the movement of chamber moisture prevents the adhesion of the created drainage paths and inhibits the development of scar tissue. Cystic degeneration of AF is the most unfavorable and difficult to treat outcome of scarring of the filtration paths. Carrying out AF nidling with the help of Nd-YAG laser exposure provides an opening of the cyst and separation of interstitial adhesions, restoration of fluid movement along the formed outflow paths, which significantly reduces trauma and the risk of complications from the course of treatment, while minimally mechanically injuring the tissue.
The method is as follows.
Patients with dysfunctional cystic AF against the background of compensated and uncompensated IOP perform a needling of AF using Nd-YAG laser treatment under local epibulbar anesthesia. The laser beam is directed into the avascular zone along the periphery of the cyst and puncture its wall. The radiation power is 2-10 mJ, the wavelength is 1064 nm, the number of pulses is 5-10. Within three days after the puncture, it is possible to additionally bougie up the puncture site, performed using the Nd-YAG laser treatment, to empty the cyst and thereby achieve a prolonged effect.

AF is cystic, the cornea is transparent, the anterior chamber is 3 mm, the moisture of the anterior chamber is transparent, the pupil is round in shape 3 mm in diameter, opacities in the nucleus and cortical layers of the lens. Optic nerve disc with a grayish tint, glaucoma excavation E / D = 1.0, shift of the vascular bundle to the nasal side. In the macular zone, redistribution of pigment. The patient was nidled by a filtration pad using Nd-YAG laser treatment under local epibulbar anesthesia - a three-time installation of a 0.5% solution of Proximetacaine (Alkain). The laser beam was directed into the avascular zone along the periphery of the cyst and punctured its wall. Power 10 mJ, wavelength 1064 nm, number of pulses 5. 5 days after the IOP procedure according to Maklakov 19 mm RT. Art. FP flat. When viewed after 3 months of IOP according to Maklakov 18 mm RT. Art.
2.Clinical example 2.
Patient K., 76 years old. Diagnosis: right eye - operated (twice) open-angle 3 "in" glaucoma, artifakia, left eye - open-angle 3 "a" glaucoma, immature complicated cataract. The first anti-glaucoma operation on the right eye (modified sinus strabeculectomy with prophylactic posterior trepanation of the sclera) was performed 1 year ago. Before IOP operation, according to Maklakov, 43 mmHg. Art. at maximum drug hypotensive regimen. In the early postoperative period, a ciliochoroidal detachment was revealed, in connection with which an operation was performed - posterior trepanation of the sclera in the lower inner quadrant. 30 days after IOP surgery according to Maklakov 25 mm RT. Art. Objectively, the right eye is calm, AF is cystic at 12 o’clock, the cornea is transparent, the anterior chamber is 3.0 mm, moisture is transparent, the pupil is round in shape, 3 mm in diameter. ZKL in the correct position. Optic nerve disc with a grayish tint, a shift of the vascular bundle to the nasal side, glaucomatous excavation E / D = 0.8. In the macular zone, redistribution of pigment. The patient was nidled by a filter cushion using an Nd-YAG laser destructor under local epibulbar anesthesia - three instillations of a 0.5% solution of Proximetacaine (Alkain). The laser beam was directed into the avascular zone along the periphery of the cyst and punctured its wall. Power 2 mJ, wavelength 1064 nM, number of pulses 10. Within 3 days after the procedure, a hole in the cyst wall was boughed once a day to achieve the best result. 5 days after the IOP procedure according to Maklakov 19 mm RT. Art. FP flat. When viewed after 3 months. IOP Maklakov 18 mm RT. Art.
Thus, the proposed method of filtering pad inflation for the prevention and treatment of excessive scarring after anti-glaucoma surgery allows the cyst to be emptied in the zone of newly created outflow pathways and to prevent further scarring without gross invasive intervention, thereby increasing the effectiveness of anti-glaucoma surgery and reducing exposure injuries and the need for repeated surgical interventions.
