Воспалительные заболевания слизистой оболочки рта могут развиваться под влиянием как местных причин, так и сочетанного воздействия местных и общих факторов на фоне измененной реактивности организма. У лиц с сахарным диабетом сложность патогенеза заболеваний слизистой рта и сопутствующих при этом нарушений происходит в результате нарушения региональной геодинамики и тканевого кровотока. Причем инициальным патогенетическим фактором являются не морфологические, а функциональные изменения сосудов слизистой оболочки рта, микроангиопатии, что приводит к развитию тканевой гипоксии [Вербовой А.Ф. и др. Изменения кожи и слизистой полости рта при сахарном диабете и их профилактика //Медицинский совет. - 2017. - №.3. - С. 54-57]. При морфологическом исследовании слизистой оболочки отмечены атрофия и склероз, хроническое продуктивное воспаление, развитие круглоклеточной инфильтрации, увеличение количества тучных, плазматических клеток, эозинофилов, макрофагов, развитие микроангиопатии, иммунная недостаточность. Травмирующим фактором могут быть острые края зубов, мостовидные и съемные протезы, нарушения расположения зубов, зубной камень, острая и горячая пища и др. Обычно изменения слизистой оболочки при хронической механической травме могут длительное время не беспокоить больного, вызывая лишь чувство неловкости, дискомфорта, незначительную болезненность, припухлость. Однако, учитывая общесоматическое заболевание - сахарный диабет типа 1, хроническая механическая травма полости рта может протекать достаточно тяжело для пациента. Ранний симптом сахарного диабета в полости рта - ксеростомия (сухость в полости рта), поэтому появляется мучительная сухость. С годами состояние и регенерация тканей полости рта после травмы ухудшается, также развиваются с нарастающей прогрессией признаки хронического генерализованного пародонтита средней и тяжелой степени, трещинки, эрозии, также очень болезненные, часто присоединяется грибковая инфекция, ухудшающая самочувствие больного и усугубляющая проявления диабета в полости рта, слизистая становится сухой, истонченной, ярко-красного цвета. При резкой болезненности язвы показаны аппликации обезболивающими средствами. Некротические ткани со дна язвы тщательно удаляют механически под анестезией или с помощью протеолитических ферментов. Назначают аппликации кератопластических средств (витамины А и Е, масло шиповника, линимент тезана, бальзам Шостаковского, каротолин, облепиховое масло, 5% метилурациловую мазь, солкосерил и др.). Прижигания категорически противопоказаны. Производится тщательная санация полости рта. При повреждениях съемными протезами необходима их коррекция.
