Варикозная болезнь вен нижних конечностей (ВБВНК) является самым распространенным заболеванием периферических вен, встречающимся у 26-38% женщин и 10-20% мужчин. Самым частым осложнением ВБВНК является острый тромбофлебит подкожных вен, частота развития которого достигает до 30% [Кириенко А.И., Матюшенко А.А., Андрияшкин В.В. Острый тромбофлебит // М.: Литтерра. - 2006. - Т. 108. - С.П.]. Ключевым принципом лечения ВБВНК является устранение вертикального рефлюкса по подкожным венам [Савельев В.С. и др. Флебология: руководство для врачей / под ред. В.С. Савельева // Медицина. - 2001. - Т. 175]. Впервые А.А. Трояновым (1888 г.) и Ф. Тренделенбургом (1890 г.) был описан хирургический метод лечения варикозно расширенной большой подкожной вены (БПВ), заключающийся в перевязке большой подкожной вены на уровне верхней трети бедра. Далее в 1907 г. W. Babcock предложил удалять ствол БПВ с помощью жесткого зонда, проведенного через вену, с оливой на одном конце с режущими краями, предназначенными для срезания вены от окружающих тканей. Данный способ комбинированной флебэктомии с различными модификациями используется и в настоящее время как самый эффективный и надежный способ лечения ВБВНК. Недостатками комбинированной флебэктомии являются наличие разрезов, вероятность повреждения глубоких вен и артерий, гематомы, лимфорея, неврологические нарушения и возможность инфицирования операционных ран [Золотухин И.А. При эндовазальных термических методах под ультразвуковым контролем производится пункция вены, в просвет вены вводится радиочастотный катетер или лазерный световод, который устанавливается дистальнее проксимального приустьевого притока или на 1-2 см дистальнее сафенофеморального соустья (СФС), далее после проведения тумесцентной анестезии производится коагуляция вены, приводящая к облитерации и трансформации ее в соединительнотканный тяж. Вопрос о перевязке приустьевых притоков при эндовазальных термических методах лечения варикозной болезни все еще остается дискутабельным, однако частота рецидивов варикозной болезни при ЭВЛК без перевязки притоков соустья выявляется реже по сравнению с кроссэктомией с перевязкой притоков. В случае возникновения рецидива варикозной болезни возможно проведение пенной склерооблитерации под ультразвуковым контролем [М. Kalteis et al. Предлагаемый способ чрескожного лигирования подкожной вены осуществляется следующим образом. Под ультразвуковым контролем с использованием ультразвукового аппарата 5 для навигации в паховой области дистальнее на 10-30 мм от СФС проводят местную инфильтрационную и тумесцентную анестезию в проекции предполагаемой перевязки вены, затем производят два прокола по 2-3 мм: инъекционной иглой G18 производят первый прокол 6 кожи над веной и латеральнее на 20 мм от проекции вены производят второй прокол 7 устройством 1, заряженным шовным материалом 4, через второй прокол проводят шовный материал 4 под вену, выдвигая на 10 мм медиальнее, крючком 3 для минифлебэктомии через первый прокол 6 захватывают шовный материал и выводят в прокол 6, таким же образом крючком захватывают шовный материал латеральнее вены и выводят в первый прокол 6, далее нить завязывают в узел, а узел погружают в прокол 6 при помощи устройства 2, при этом захватывают узел в петлю через боковую прорезь, производят затягивание узла и лигирование вены.The proposed method for percutaneous ligation of the saphenous vein is as follows. Пациентка М., 1982 года рождения, обратилась в «Клинику современной флебологии» с жалобами на тяжесть, усталость в правой ноге к концу дня, а также на наличие варикозно расширенных вен на правой ноге. На основании клинической картины и данных УЗДС был выставлен диагноз: Варикозная болезнь C2s, Ер, As, Рr 2, 3 LII с поражением БПВ правой нижней конечности. По данным УЗДС было выявлено варикозное расширение БПВ правой нижней конечности до 1,9 см в приустьевом отделе БПВ, до 0,8-0,9 см на уровне бедра, несостоятельность клапанного аппарата БПВ с выраженным стволовым рефлюксом. При помощи УЗДС было определено оптимальное место для перевязки БПВ - 10 мм дистальнее СФС. После инфильтрационной анестезии кожи над предполагаемым местом перевязки вены была произведена тумесцентная анестезия БПВ. Перевязка БПВ проводилась с применением предлагаемого способа: инъекционной иглой G18 произвели первый прокол кожи над веной и латеральнее на 20 мм от проекции вены произвели второй прокол устройством, содержащим металлическую ручку, прикрепленную к металлическому стержню с петлей на конце, через которую проходит шовный материал. Через второй прокол провели шовный материал под вену, выдвигая на 10 мм медиальнее, крючком для минифлебэктомии через первый прокол захватили шовный материал и вывели в первый прокол, таким же образом крючком захватили шовный материал латеральнее вены и вывели в первый прокол. Нить завязали в узел, а узел погрузили в первый прокол при помощи устройства, содержащего металлическую ручку, прикрепленную к металлическому стержню с петлей на конце, которая имеет боковую прорезь. Для этого захватили узел в петлю через боковую прорезь, произвели затягивание узла и лигирование вены. Дистальная часть ствола БПВ была облитерирована стандартным способом ЭВЛК. Непосредственно после процедуры на ультразвуковом контроле отмечается состоятельная культя БПВ без наличия тромботических масс.
