Лигирование подкожных сосудов путем прошивания чаще осуществляется при варикозном расширении вен (Волобуев Н.Н. Клиническая хирургия - 1976. - N 6. -с. 71; Червяков И.В. Вестник хирургии - 1985.- N 10. - с. 111) как вспомогательный метод лечения варикозной болезни, направленный на суммарное улучшение результатов хирургической коррекции. Большое значение при данном хирургическом вмешательстве имеет необходимость достижения косметического эффекта. Известные методы накожного прошивания подкожных сосудов, например вен, имеют существенные недостатки. Так, описанный метод венорафии (Поляков П.И., Трофименко С.С. Вестник хирургии - 1988. - N 7. - с. 76-78), взятый нами за прототип, предусматривает множественное чрескожное лигирование вен отдельными швами с завязыванием и оставлением узлов над кожей. Прототип имеет недостатки:
- значительное увеличение времени операции;
- развитие выраженного косметического дефекта вследствие высокого удельного давления нити шва на кожу с образованием множественных странгуляционных бороздок;
- неизбежная воспалительная реакция и местные аллергические изменения из-за отсутствия биологической инертности у шовного материала без дополнительной его обработки;
- неравномерное распределение степени сдавления вены не обеспечивает гарантированного полного прекращения кровотока по всей длине вены;
- ненадежность фиксации сосуда ввиду возможного развязывания одинарной нити или ее разрыва;
- ограничение использования монофиламентной синтетической нити из-за необходимости трехкратного завязывания узла, что приводит к большой затрате времени;
- ввиду высокого риска образования обезображивающих странгуляционных борозд на поверхности кожи и возможного прорезывания швов возникает необходимость раннего их снятия, что приводит к реканализации вен, сводящей на нет успех операции. Спешка хирурга в удалении швов в сроки, когда после компрессии вен, прерывания или замедления кровотока по ним происходит формирование лишь тромба красного типа, приводит к тому, что наступает резорбция рыхлого сгустка крови, что заканчивается восстановлением кровотока по вене. При раннем снятии лигатур всегда отмечается умеренное кровотечение из мест проколов иглой, из раневого шовного канала.Known methods for skin flashing of saphenous vessels, such as veins, have significant disadvantages. - the inevitable inflammatory reaction and local allergic changes due to the lack of biological inertness in the suture material without additional processing;
- uneven distribution of the degree of compression of the vein does not guarantee a complete cessation of blood flow along the entire length of the vein;
- unreliability of fixation of the vessel due to the possible untying of a single thread or its rupture;
- the restriction of the use of monofilament synthetic yarn due to the need for three times knotting, which leads to a large waste of time;
- due to the high risk of the formation of disfiguring strangulation grooves on the surface of the skin and the possible eruption of sutures, it is necessary to remove them early, which leads to recanalization of the veins, which negates the success of the operation. The surgeon’s rush to remove the sutures at a time when only a red blood clot forms on them after compression of the veins, interruption or slowdown of blood flow, leads to the fact that there is a resorption of a loose blood clot, which ends with the restoration of blood flow through the vein. With early removal of ligatures, moderate bleeding is always observed from places of puncture with a needle, from a wound suture canal.
С целью надежной фиксации захваченного участка сосуда к коже, создания гарантированного гемостаза, повышения косметического эффекта операции и сокращения времени ее проведения нами предложен чрескожно-подкожный непрерывный шов "взахлест" для лигирования подкожных сосудов. Накануне операции после принятия гигиенической ванны на оперируемую конечность накладывают марлевую повязку, смоченную предложенным нами ксимедоно-гидрокортизоно-борным водным раствором. Повязку снимают непосредственно перед операцией. После окончания этапов операции Троянова - Тренделенбурга и Бэбкокка производят прошивание поверхностно расположенных боковых ветвей большой и малой подкожных вен многожильными нитями, предварительно импрегнированными ксимедоно-гидрокортизоно-борным водным раствором при Тo 0+2oС в течение суток. Около отмеченной красителем проекции венозного ствола (см. чертеж) вкалывают иглу 1 с двойной нитью 2 через кожу 3 на глубину необходимого захвата боковых ветвей основного ствола вены 4. Выкол производят с противоположной стороны вены. Завязывают узел 5 на коже 3 одновременно двойной разноцветной нитью. Отступая 1,5-2,0 см от первого узлового шва, по ходу вены делают второй аналогичный вкол иглы 1 с двойной нитью 2 и проводят ее под веной перпендикулярно последней, после чего выкалывают с противоположной стороны вены 4. Вытянув всю двойную нить до отказа, захватывают взахлест одну жилу одноименного цвета нити предыдущего стежка за среднюю часть и стягивают ее к точке выкола. При этом на поверхности кожи образуется прямоугольный треугольник из одинарной нити 6, а под веной проходит по одной перпендикулярной линии двойная нить 2. Основание треугольника находится параллельно венозному стволу, а вершина - на противоположной стороне вены. Затем таким же образом накладывают необходимое количество накожных витков на пораженную вену. После прошивания вены на всем протяжении, на заключительном этапе, накладывают обвивной шов аналогично первому с двойным захлестыванием так, чтобы образовался узел. По всей линии швов накладываются салфетки, пропитанные ксимедоно-гидрокортизоно-борным водным раствором с укреплением их классической марлевой циркулярной повязкой. В послеоперационном периоде такие повязки-аппликации с ксимедоно-гидрокортизоно-борным водным раствором применяли ежедневно до снятия швов (на 10-12 день), всего на протяжении 3-х недель. Клинической опыт использования предложенного способа накожного лигирования подкожных кровеносных сосудов шовным материалом, предварительно импрегнированным ксимедоно-гидрокортизоно-борным водным раствором у 18 больных с варикозной болезнью показал явные преимущества перед прототипом. Сокращалось время операции, осуществлялась надежность гемостаза, значительно повышался косметический эффект, несмотря на более поздние сроки (10-12 день) снятия накожного непрерывного шва, чем в контроле (4-6 день). Относительно позднее снятие сдавливающих накожных швов приводило к образованию внутрисосудистого тромба белого типа, не подверженного рассасыванию. Ложный рецидив варикозной болезни зарегистрирован у одной больной. Больная А., 45 лет, поступила 7.10.94 г. в хирургическую клинику с выраженным варикозным расширением подкожных вен нижних конечностей в фазе компенсации. За 12 ч до операции наложена повязка с предложенным нами ксимедоно-гидрокортизоно-борным водным раствором. Под перидуральной анестезией из прецизионных разрезов в области лодыжек и паха произведено выделение большой и малой подкожных вен. Магистральные стволы их пересечены и в антеградном направлении в просвет введены сафенэкстракторы с последующей фиксацией их к вене. Предварительно размеченные до операции красителем боковые ветви большой и малой подкожных вен прошиты путем накожного лигирования двойной нитью, импрегнированной ксимедоно-гидрокортизоно-борным водным раствором по предложенному способу. Осуществлен стрипинг основных стволов большой и малой подкожных вен по Бэбкокку. Повязки с указанным раствором. Послеоперационный период протекал без осложнений. Швы на ранах сняты на 8-й день, накожные непрерывные - на 12-е сутки. Странгуляционные борозды на коже в местах нахождения нити не выражены, отсутствуют и признаки воспаления кожи в области швов. Кровотечения после снятия лигатур из мест проколов не отмечено. Домой выписана на 14-е сутки в удовлетворительном состоянии. Осмотрена через год. Рецидива заболевания нет. Косметический эффект хороший. The stripping of the main trunks of the major and minor saphenous veins by Babcock was carried out. Dressings with the specified solution. The postoperative period was uneventful. The wound sutures were removed on the 8th day, continuous cutaneous sutures on the 12th day. Strangulation furrows on the skin at the locations of the thread are not expressed, there are no signs of inflammation of the skin in the suture area. Bleeding after removing ligatures from puncture sites was not noted. Home was discharged on the 14th day in a satisfactory condition. Inspected after a year. There is no relapse of the disease. The cosmetic effect is good.
Контрольную группу составили 20 больных с аналогичным диагнозом, которым наряду с сафенэктомией осуществлялось традиционное накожное лигирование боковых ветвей. В послеоперационном периоде отмечалась более длительная болевая реакция в области швов с отеком и гиперемией. При снятии швов имелось умеренное кровотечение из мест кожных проколов, что свидетельствовало об отсутствии стабильного и фиксированного формирования внутрисосудистого тромба. В местах нахождения лигатур оставались небольшие продольные углубления, главным образом вследствие реакции кожи на шовный материал. Рецидив заболевания в отдаленном периоде выявлен у 4 оперированных.
