В качестве прототипа изобретения служит способ получения функционального оттиска после резекции верхней челюсти [RU 2423955, опубл. 20.07.2011], в котором получают анатомический оттиск альгинатной массой стандартной ложкой, по которому изготавливают гипсовую модель. Моделируют на ней обтуратор путем выстилки позитивного отображения дефекта неба воском с одновременным моделированием резецированного альвеолярного гребня. Устанавливают полученную композицию в полости рта и припасовывают путем восполнения возможных зазоров воском. Получают оттиск, по которому изготавливают индивидуальную ложку. Адаптируют полученную ложку в полости рта, используя известные функциональные пробы. Устанавливают ретенционные металлические скобы в восковой обтуратор, фиксированный в дефекте челюсти. Поставленная задача решается благодаря тому, что в способе получения функционального комбинированного оттиска у больных с послеоперационными дефектами верхней челюсти, включающем припасовку индивидуальной ложки в полости рта, коррекцию ее краев при помощи воска, отличающегося созданием дополнительной опоры мягких тканей на стороне дефекта, уточнением границ дефекта базисным воском, создающим дополнительное давление на оттискную массу, заполняющую дефект и получением двухэтапного оттиска силиконовой оттискной массой с проведением функциональных проб. Сущность предложенного способа заключается в следующем. Получают анатомический оттиск, отливают модель и изготавливают индивидуальную ложку из фотополимерного материала, после обработки ложка припасовывается в полости рта, необходимо добиться плотного наложения на челюсть. На щечной поверхности ложки, на резецированной стороне формируется поддержка для мягких тканей щеки. Для этого путем сошлифовывания отдельных участков ложки или добавления разогретого базисного воска формируется щечная поверхность индивидуальной ложки. Большое значение имеет оформление задней границы протеза. Для формирования задней границы протеза и улучшения его фиксации на небную поверхность индивидуальной ложки, соответственно податливой зоне мягких тканей твердого неба, на здоровой стороне и соответственно тканям мягкого неба на границе дефекта, накладывается размягченный базисный воск толщиной 0,5-1 мм. Затем проверяемая конструкция вводится в полость рта больного, прижимается к небу. Излишки воска, вытесненные за пределы ложки, удаляются. После этого индивидуальная ложка перфорируется с помощью фрезы для фиксации оттискного материала. Используется С-силиконовый оттискной материал (Zeta+; Спидекс и т.п.). Далее снимается первый слой функционального оттиска базовым материалом, основная часть которого укладывается в области дефекта верхней челюсти и по краям протеза для того, чтобы сформировать краевой замыкающий клапан. При этом голову больного немного наклоняем вниз, при медленном выдохе он произносит долгое "А". Эта проба повторяется в течение 1,5-2 минут. Мягкое небо при этом несколько выпрямляется и поднимается. В этот момент ложку с оттискным материалом усилием большого и указательного пальцев прижимают к небу. При этом кончики пальцев должны надавливать на утолщенные участки индивидуальной ложки. На оттиске, полученном таким образом, происходит оптимальное сжатие не только тканей линии "А", но и податливых тканей, мягких по всей поверхности твердого неба. Расположенный на ложке неравномерно, участками, базовый оттискной материал также выполняет роль стоппера, позволяющий получить в будущем разгружающий, некомпрессионный функциональный оттиск. После этого корригирующим слепочным материалом снимается функциональный оттиск. Индивидуальная ложка выводится из полости рта, покрывается ровным слоем силиконовой корригирующей массы средней вязкости ("Stomaflex cream", Zeta+; Спидекс и т.п.), не вызывающей компрессии слизистой оболочки более 50%, толщиной около 2 мм и снова размещается в полости рта. Получение оттиска в данных клинических ситуациях часто затруднено вследствие ограничения открывания рта. Однако необходимость создания высокой обтурирующей части, играющей в последующем роль лечебного аппарата для механотерапии, ставит задачу получения оттиска послеоперационной полости на всю глубину, принимая во внимание все ретенционные пункты. В этой связи при получении рабочего оттиска, в дополнение к нанесению слепочной массы на индивидуальную ложку, проводится предварительное промазывание стенок и дна дефекта, используя корригирующую массу средней вязкости. На этапе получения функционального оттиска у челюстно-лицевого больного для обеспечения герметичности разобщения носовой и ротовой полостей и получения удовлетворительного краевого замыкающего клапана обтурирующего протеза больному необходимо плотно прижать внешние носовые ходы и попросить больного сделать попытку выдоха воздуха через нос. Операцию повторить несколько раз. Привести в движение мимическую мускулатуру, составляющую модилюс преддверия полости рта, то есть при сомкнутых губах больной делает движения губ вперед, назад, пытается, поднять углы рта несколько раз. После этого проверяем чистоту произношения гласных и согласных букв. Особое внимание уделяется чистоте произношения сонорных букв типа Н, М, Б, В, Г, Д и др., а также «шумных» типа Л, П, К, Ф, Ш, X, Ц. Чистота их произношения свидетельствует о качественном герметичном разобщении ротовой и носовой полостей и хорошем краевом замыкающем клапане. Таким образом, можно оценить результаты функционального оттиска, что в последующем обеспечивает качество изготовленного челюстно-лицевого протеза для реабилитации больного. После отверждения оттискной массы индивидуальная ложка извлекается из полости рта пациента. Снятие оттиска таким образом позволяет получить максимально точное отображение протезного поля и уменьшить консольный эффект будущего обтурирующего протеза.The essence of the proposed method is as follows. Of great importance is the design of the posterior border of the prosthesis. To form the posterior border of the prosthesis and improve its fixation, a softened base wax with a thickness of 0.5-1 mm is applied to the palatal surface of the individual spoon, respectively the compliant zone of soft tissues of the hard palate, on the healthy side and, accordingly, to the tissues of the soft palate at the border of the defect. Then, the tested design is introduced into the patient's oral cavity, pressed against the sky. Excess wax displaced outside the spoon is removed. After that, the individual spoon is perforated with a cutter to fix the impression material. C-silicone impression material is used (Zeta +; Spidex, etc.). Next, the first layer of the functional impression is removed with the base material, the main part of which is laid in the region of the defect of the upper jaw and along the edges of the prosthesis in order to form an edge closing valve. At the same time, we tilt the patient’s head down slightly, while exhaling slowly, he pronounces a long “A”. This test is repeated for 1.5-2 minutes. The soft palate thus straightens and rises somewhat. At this moment, a spoon with impression material is pressed against the sky by the force of the thumb and forefinger. In this case, the fingertips should press on the thickened sections of the individual spoon. On the print obtained in this way, not only the tissues of line “A” are optimally compressed, but also compliant tissues that are soft over the entire surface of the hard palate. Situated unevenly on the spoon, in sections, the base impression material also acts as a stopper, which allows for the future unloading, non-compression functional impression. After this, a corrective impression is removed by corrective impression material. An individual spoon is removed from the oral cavity, covered with an even layer of medium viscosity silicone corrective mass ("Stomaflex cream", Zeta +; Spidex, etc.), which does not cause compression of the mucous membrane more than 50%, about 2 mm thick and is again placed in the oral cavity . Imprinting in these clinical situations is often difficult due to limited opening of the mouth. However, the need to create a high obstructive part, which subsequently plays the role of a medical apparatus for mechanotherapy, poses the task of obtaining an impression of the postoperative cavity to the full depth, taking into account all the retention points. In this regard, upon receipt of a working impression, in addition to applying the impression mass on an individual spoon, a preliminary smearing of the walls and the bottom of the defect is carried out using a corrective mass of medium viscosity. At the stage of obtaining a functional impression in the maxillofacial patient, in order to ensure the tightness of separation of the nasal and oral cavities and to obtain a satisfactory regional closing valve of the obstructing prosthesis, the patient must tightly press the external nasal passages and ask the patient to try to exhale through the nose. Repeat the operation several times. To set in motion the facial muscles that make up the model of the vestibule of the oral cavity, that is, with closed lips, the patient makes lip movements forward, backward, tries to raise the corners of the mouth several times. After that, we check the pronunciation of vowels and consonants. Particular attention is paid to the cleanliness of pronunciation of sonorous letters such as H, M, B, H, D, D, etc., as well as "noisy" types of L, P, K, F, W, X, C. The cleanliness of their pronunciation indicates a high-quality airtight dissociation of the oral and nasal cavities and a good marginal closure valve. Thus, it is possible to evaluate the results of a functional impression, which subsequently ensures the quality of the manufactured maxillofacial prosthesis for the rehabilitation of the patient. After the impression mass has cured, the individual spoon is removed from the patient's oral cavity. The removal of the impression in this way allows you to get the most accurate representation of the prosthetic field and reduce the cantilever effect of the future obstructive prosthesis.
