В настоящее время развитие хирургической техники, анестезиологии и реанимации привели к улучшению непосредственных и отдаленных результатов хирургического лечения рака желудка. В том числе широко выполняются гастрэктомии при раке проксимального отдела желудка. За счет более агрессивной хирургии рака желудка во всех стадиях улучшились показатели 5-летней выживаемости, что и усилило интерес хирургов к поиску и разработке различных типов реконструкции после гастрэктомии, с точки зрения их физиологичности и качества жизни больных [1, 2]. Известны и широко используются различные способы анастомозирования пищевода и тонкой кишки при гастрэктомии по поводу рака желудка [3]. Задачей, решаемой настоящим изобретением, является разработка методики формирования эзофагоэнтероанастомоза с последующим восстановлением пассажа по 12-перстной кишке при выполнении гастрэктомии.The problem solved by the present invention is the development of methods for the formation of esophagoenteroanastomosis with subsequent restoration of the passage through the duodenum 12 when performing gastrectomy.
Достигаемым техническим результатом является: обеспечение удовлетворительного результата хирургического лечения рака желудка путем использования предложенной методики формирования аппаратного эзофагоэнтероанастомоза с последующим восстановлением пассажа по 12-перстной кишке.Achievable technical result is: providing a satisfactory result of surgical treatment of gastric cancer by using the proposed methodology for the formation of hardware esophagoenteroanastomosis with subsequent restoration of the passage through the duodenum.
Благодаря предложенным техническим особенностям оперативное вмешательство является технически простым (учитывая использование сшивающего аппарата), выполняется с соблюдением принципов онкорадикализма и выполняется из лапаротомного доступа даже при необходимости резекции нижнегрудного отдела пищевода, тем самым достигается уменьшение операционной травмы и сокращается время оперативного вмешательства и анестезиологического пособия, что приводит к благоприятному протеканию послеоперационного периода, низкому числу послеоперационных осложнений и сокращению сроков лечения данной группы пациентов, а также к сокращению сроков их социально-трудовой реабилитации.Thanks to the proposed technical features, surgical intervention is technically simple (considering the use of a stapler), performed in compliance with the principles of oncoradicalism and performed from laparotomic access, even if the lower thoracic esophagus is resected, thereby reducing surgical trauma and reducing the time of surgical intervention and anesthetic management, which leads to a favorable course of the postoperative period, a low number of postoperative nnyh complications and reduce the duration of treatment in this group of patients, as well as to a reduction in terms of their social and occupational rehabilitation.

Предложенная методика выполняется у больных, которым установлены показания к гастрэктомии и резекции нижнегрудного отдела пищевода.The proposed technique is performed in patients who have established indications for gastrectomy and resection of the lower thoracic esophagus.
Выполняется верхне-среднесрединная лапаротомия. Далее выполняется гастрэктомия, резекция нижнегрудного отдела пищевода с лимфаденэктомией D2-D3 по общепринятой методике. После этого формируется отключенная по Ру петля тонкой кишки. В «свободный» конец кишки вводят циркулярный сшивающий аппарат, например, ЕЕА DST 25. «Шляпку» аппарата фиксируют в дистальном конце пищевода кисетным швом. Далее формируется аппаратный эзофагоеюноанастомоз «конец в бок». После этого сшивающий циркулярный аппарат извлекают из «свободного» конца петли тонкой кишки и формируется однорядный еюнодуоденоанастомоз «конец в конец» (с целью восстановления пассажа по 12-перстной кишке). Далее выполняется санация, дренирование и ушивание брюшной полости по общепринятой методике.An upper-midline laparotomy is performed. Клинический пример: Больная Б., 36 лет, поступила в НИИ СП с диагнозом «желудочно-кишечное кровотечение». При обследовании: по данным ЭГДС имеется опухоль верхней трети желудка с переходом на кардиальный отдел пищевода, признаки состоявшегося желудочно-кишечного кровотечения. После дообследования и предоперационной подготовки под общей анестезией больной выполнена верхне-среднесрединная лапаротомия. В брюшной полости метастазов и увеличенных регионарных лимфатических узлов не обнаружено. Учитывая наличие опухоли верхней трети желудка с частичным поражением нижнегрудного отдела пищевода, решено выполнить гастрэктомию с лимфааденэктомией D3 по предложенной методике. После выполнения гастрэктомии и резекции нижнегрудного отдела пищевода сформирована отключенная по Ру петля тонкой кишки. В отводящий конец кишки введен циркулярный сшивающий аппарат ЕЕА DST 25, «шляпка» которого предварительно фиксирована в дистальном конце пищевода кисетным швом. Далее был сформирован аппаратный эзофагоеюноанастомоз «конец в бок». После этого сшивающий циркулярный аппарат извлечен из «свободного» конца петли тонкой кишки и сформирован однорядный еюнодуоденоанастомоз «конец в конец». Далее выполнена санация и дренирование брюшной полости, ушивание передней брюшной стенки.Clinical example: Patient B., 36 years old, was admitted to the SRI SR with a diagnosis of gastrointestinal bleeding. After gastrectomy and resection of the lower thoracic esophagus, a loop of the small intestine is disconnected along Ru. A circular stapler EEA DST 25 is inserted into the outlet end of the intestine, the “cap” of which is previously fixed in the distal end of the esophagus with a purse string suture. Next, a hardware end-to-side esophagoejunoanastomosis was formed. After this, the cross-linking circular apparatus is removed from the "free" end of the loop of the small intestine and a single-row end-to-end single-duodenoanastomosis is formed. Next, rehabilitation and drainage of the abdominal cavity, suturing of the anterior abdominal wall were performed.
