Abstract
Description
The invention relates to medicine, namely to dentistry. The operation of vestibuloplasty is to increase the area of the motionless mucous membrane of the alveolar process at the level of the vestibule of the oral cavity (1). Surgical intervention increases the width of the attached gums in order to eliminate mechanical trauma to the regional periodontal cords by muscles of the oral cavity: labial, chin, buccal, lingual and facial. One of the main indications is the prevention of gum recession or the cessation of its progression.
Vestibuloplasty is performed with a small vestibule of the oral cavity, which also contributes to the development of periodontal, bone pockets, exposure of the roots, and also eliminates the reliable fixation of removable dentures in the oral cavity. When prosthetics on dental implants, this leads to the occurrence of peri-implantitis.
(2).Various methods have been developed for deepening the vestibule of the oral cavity (2).
Vestibuloplasty is performed after anesthesia, antiseptic treatment of the oral cavity, displacement of the lips and cheek, determination of the mucogingival border at the level of surgical intervention. Then, a cut is made with a scalpel along the mucous-gingival border to the periosteum. The mucous membrane from the region of the transitional fold is separated, moved to the region of the deeper arch of the vestibule of the oral cavity and is sutured to the periosteum. The resulting wound surface is healed by secondary intention (2) or covered with a free autograft (1), a non-free flap from the area of the pterygo-maxillary fold (3) or an allogeneic peritoneum (4), etc.
The disadvantages of such interventions are their significant morbidity, the duration of the surgical intervention, the high possibility of microbial infection, flap rejection, long-term disability, the inability to take normal food and oral hygiene, impaired speech function, and a significant risk of relapse.
The aim of the invention is to develop a method of vestibuloplasty, which can significantly reduce the invasiveness of surgery, reduce the duration of the operation and the postoperative period and significantly improve the quality of life of the patient.
This goal is achieved by the fact that in the submucosal layer of the movable part of the mucous membrane of the alveolar process is introduced in parallel and retreating 1-2 mm from the transition folds of the hair-insulated electrode of the molecular resonance generator, after which the electrode is slowly removed, this manipulation is repeated 6-12 times depending on the location of the transitional fold and the thickness of the submucosal layer in the area of operation; during the intervention, no mucosal incisions are made and soft tissues are not excised. The method is implemented as follows. After bilateral mandibular anesthesia, antiseptic treatment of the oral cavity, the lip or cheek is displaced so that the mucogingival border is clearly visible in advance of the oral cavity at the level of surgical intervention. Then, in the submucous layer of the movable part of the mucous membrane of the alveolar ridge, a Italian insulated molecular-resonance generator Vesalius LX80 is introduced 1-2 mm from the transitional fold, creating currents in the range from 4 to 16 MHz after switching on to “coagulation” mode ”Of which the electrode is slowly removed, this manipulation is repeated 6-12 times depending on the location of the transitional fold and the thickness of the submucosal layer in the area of operation.
During the intervention, no mucosal incisions are made and soft tissues are not excised.
(5, 6, 7).The proposed method of vestibuloplasty is based on the use of molecular resonance technology based on the use of high-frequency currents that cause the formation of molecular resonance in tissues (5, 6, 7).
The duration of surgery, if you do not take into account the antiseptic treatment of the oral cavity and anesthesia, is only 0.5-3.0 minutes.
The physical basis of the method is that the energy of the generator is transmitted by quanta, the energy of which is absolutely equal to the energy of intermolecular bonds of biological cells. In this case, the amplitude of vibrations of individual molecules in cells increases sharply, which leads to rupture of cell membranes. To achieve a coagulating effect when applying the molecular resonance technique, the frequency regime of the generator is changed so as not to provide 100% resonance in molecular bonds. As a result, there is a change in the spatial structure of protein molecules caused by the breaking of hydrogen bonds (denaturation), which leads to their “sticking together” and provides a coagulating effect. The coagulating effect of the molecular resonance apparatus is “soft” in nature and does not affect the adjacent tissues: periosteum and mucous membrane. In this case, the movable mucous membrane of the alveolar process is rigidly fixed to the periosteum and becomes immobile, which is clinically manifested by an increase in the area of the kerotized mucosa.
During the operation, the temperature of the cells rises slightly and reaches 45-50 ° C, which does not lead to necrosis of biological structures. The coagulating effect, achieved not by coarse thermal exposure, but by changing the spatial configuration of protein molecules allows molecular resonance effects in the immediate vicinity of blood vessels, nerve trunks and their endings. Moreover, with insignificant heat exposure and minimal soreness, blood loss does not occur, the operation takes place in a practically “dry” wound. Molecular resonance surgery eliminates carbonization of tissues, inflammation and practically does not cause pain during the operation and in the postoperative period.
Upon reaching the coagulating effect, the mucous membrane turns pale, which is the basis for termination of exposure.
The method is used in the treatment of 12 patients, immediate and long-term results are good.
Clinical example
Patient R., 51 g. Diagnosis: periodontitis generalized moderate severity; shallow vestibule of the oral cavity in the lower jaw. The distance from the crest of the alveolar ridge to the transitional fold at a level of 34 to 44 was 4 mm.
44.After bilateral mandibular anesthesia, the lower lip and the corners of the mouth were displaced, and the mucogingival border was determined in the area of surgical intervention. A hairy insulated electrode was introduced into the submucosal layer in parallel and stepping 1 mm from the transition fold at a level of 31 to 34, then the apparatus was turned on, and the electrode was slowly removed. In this case, the blanching of the mucous membrane was a guideline. This manipulation was repeated four more times, the distance between the punctures of the mucous membrane was 1.0 mm. Similarly performed the operation in areas 41 and 44.
The duration of the operation, if you do not take into account the antiseptic treatment of the oral cavity and anesthesia, was three minutes. Slight pain, swelling was noted during the first days after surgery. The immediate and long-term results are good, the depth of the vestibule of the oral cavity at the level of 34 and 44 a year later was 9 mm. The patient notes a significant improvement in the quality of life.
The proposed method can be widely used both in outpatient and inpatient units of dental institutions.
