Description

Причиной развития КЦТ могут быть врожденные нарушения, травмы глаза, воспалительные и дегенеративные процессы в сосудистом тракте, внутриглазные паразиты, длительное применение сильнодействующих миотических препаратов. Пример 1. В клинику обратился пациент Д., 16 лет, с жалобами на постепенное снижение зрения правого глаза в течение последних 4-5 месяцев. При обследовании Vis OD 0.6 sph-1,25 cyl-0.5 ах 165°=0.85, OS 1.0. ВГД OD 14 мм рт.ст., OS 15 мм рт.ст. При биомикроскопии OD при максимальном медикаментозном мидриазе ретролентально на 9 часах визуализируется край объемного образования с гладкой пигментированной поверхностью, стенка полупрозрачна, соприкасается с задней капсулой хрусталика, где визуализируется контактное помутнение. На радужке в прикорневой зоне соответственно определяемому образованию за хрусталиком визуализируется щелевидный пигментированный передний край образования. При ультрабиомикроскопии (УБМ) правого глаза в области отростчатой части цилиарного тела по меридиану 9 часов определяется объемное тонкостенное образование, контактирующее с хрусталиком, размеры образования - высота 5,0 мм, протяженность 6,5 мм. Диагноз: OD - киста цилиарного тела и радужки, вторичная катаракта. Проведено лечение: после инсталляционной анестезии с помощью Nd-YAG-лазера (1064 нм) с использованием трехзеркальной линзы Гольдмана выполнили транспупиллярную дисцизию стенки КЦТ со стороны стекловидного тела, нанося в 3-х точках на стенку КЦТ пунктуры до появления отверстий и уменьшения размеров КЦТ, при этом длительность лазерного излучения составила 0,3 с, диаметр лазерного пятна на поверхности КЦТ 10 мкм, энергия излучения 5,0 мДж. Затем после ретробульбарной анестезии с помощью наконечника диодного лазера провели контактную транссклеральную фотокоагуляцию в проекции основания КЦТ: отступя от лимба на 3 мм, нанесли 6 коагулятов при выходной мощности излучения 1100 мВт, диаметре пятна на поверхности склеры 500 мкм и экспозиции лазерного излучения 7 с. Операция прошла без осложнений. В послеоперационном периоде назначены инсталляции нестероидных противовоспалительных капель на 10 дней. При контрольном осмотре через 3 месяца по данным УБМ правого глаза размеры КЦТ уменьшились, клинически при медикаментозном мидриазе в заинтересованном квадранте образование не визуализируется, Vis OD 0.7 без коррекции. Через 1 год после операции по данным УБМ правого глаза стенка КЦТ уплотнилась, размеры КЦТ уменьшились - высота 1,0 мм, протяженность основания - 3,2 мм; при биомикроскопии в прикорневой зоне радужки новообразованной ткани не визуализируется, помутнение задней капсулы хрусталика не прогрессирует; зрительные функции правого глаза стабильны.Example 1. Then, after retrobulbar anesthesia using the tip of a diode laser, we performed contact transscleral photocoagulation in the projection of the CCT base: retreating from the limb by 3 mm, applied 6 coagulates with an output radiation power of 1100 mW, a spot diameter on the sclera surface of 500 μm, and laser radiation exposure of 7 s. The operation went without complications. In the postoperative period, installations of non-steroidal anti-inflammatory drops for 10 days are prescribed. При биомикроскопии левого глаза: передний отрезок - передняя камера 2,5 мм, базальная колобома радужки на 13 часах, помутнение хрусталика в кортексе, диск зрительного нерва (ДЗН) бледный, отношение диаметра экскавации к диаметру ДЗН равно 0.5. При циклоскопии левого глаза на 5 часах визуализировано два объемных пигментированных образования с гладкой поверхностью, округлой формы, плотно соприкасающихся друг с другом. УБМ левого глаза: в области отростков цилиарного тела по меридиану 5 часов определяется объемное двухкамерное кистовидное образование, его размеры - высота 2.0 мм, протяженность основания 3.5 мм. Диагноз: OS - киста цилиарного тела, оперированная первичная узкоугольная глаукома. При динамическом наблюдении пациентки в течение 4 месяцев отметили ультразвуковые признаки увеличения размеров КЦТ. Проведено лечение: после максимального медикаментозного расширения зрачка и инсталляционной анестезии с помощью Nd-YAG - лазера (1064 нм) с использованием трехзеркальной линзы Гольдмана на стенку КЦТ со стороны стекловидного тела нанесли лазерные импульсы в 3-х точках до появления отверстий и уменьшения размеров КЦТ, при этом длительность лазерного излучения составила 0,5 с, диаметр лазерного пятна на поверхности КЦТ 10 мкм, энергия излучения 7,0 мДж. Затем после ретробульбарной анестезии с помощью наконечника диодного лазера провели контактную диод - лазерную фотокоагуляцию в проекции основания КЦТ - отступя от лимба на 4 мм, нанесли 4 коагулята при выходной мощности излучения 800 мВт, диаметре пятна на поверхности склеры 500 мкм и экспозиции лазерного излучения 5 с. Операция прошла без осложнений. В послеоперационном периоде назначены инсталляции нестероидных противовоспалительных капель на 10 дней. Через 6 месяцев наблюдений по клиническим данным и УБМ левого глаза признаков объемного образования в заинтересованном отделе сосудистой оболочки нет.
