Description
Изобретение относится к медицине, а именно к колопроктологии, и может быть использовано для лечения больных с острым и хроническим воспалением эпителиального копчикового хода. Целью изобретения является разработка методики укрепления крестцово-копчиковой фасции, а также разработка новых оригинальных способов ушивания раны после иссечения ЭКХ единым блоком с кожей и измененными тканями.The invention relates to medicine, namely to coloproctology, and can be used to treat patients with acute and chronic inflammation of the epithelial coccygeal passage. Наличие обширной послеоперационной раны представляет особую сложность при сведении ее краев, поэтому на сегодняшний день учеными предложено значительное количество способов закрытия послеоперационной раны. Одни авторы после иссечения копчикового хода и патологически измененных тканей предлагают вести рану открыто (В.К.Ан, 2003), другие ушивают ее частично в области углов или только нижнего угла (М.С.Алексеев, 1990; Г.А.Серопян, 2002), некоторые подшивают кожные края раны ко дну (В.А.Врублевский, 1976; С.Б.Дубов, 1993; Г.И.Воробьев, 2001) или ушивают рану «наглухо» (O.K.Скобелкин, 1989; Т.З.Данилов, 2000; Н.А.Башанкаев, 2003; Р.С.Баширов, 2005; А.А.Воробей, 2005; А.А.Карташев, 2011). В последнее время в литературе значительное внимание уделяется пластическим способам закрытия раны после иссечения ЭКХ (А.П.Старостин, 1987; В.Л.Денисенко, 2004; Darwish A.M, 2009). Recently, considerable attention has been paid in the literature to plastic methods of closing a wound after excision of an ECC (A.P. Starostin, 1987; V.L. Denisenko, 2004; Darwish A.M, 2009). However, to date, there is no single approach to the choice of tactics and method of surgical benefits for this disease (V.L.Denisenko, 2008).
Многие исследователи считают оптимальной методику иссечения ЭКХ единым блоком с кожей и измененными тканями с последующим ушиванием раны «наглухо». По мнению ряда авторов, данный способ значительно сокращает сроки госпитализации и предупреждает развитие грубых деформирующих рубцов (М.М.Каншин, 1981; А.В.Куляпин, 1989; В.Е.Хренов, 1995; Н.А.Башанкаев, 2003; Nahas S.C., 1997). Наиболее часто для ушивания раны используют адаптационный шов по Донати и его модификации. Основным недостатком данного метода является чрезмерное натяжение сшиваемых тканей, что ведет к дальнейшему прорезыванию швов либо на коже, либо на крестцово-копчиковой фасции. Известен способ оперативного лечения нагноившегося эпителиального копчикового хода (патент РФ №2277864, МПК A61B 17/00, 2006), по которому после иссечения эпителиального копчикового хода и его разветвлений закрывают операционную рану наложением шва, при этом при первом вкалывании нить проводят через крестцово-копчиковую связку, при втором вкалывании нить проводят также по дну раны, через крестцово-копчиковую связку, как при первом вколе. За рядом швов в верхней и нижней частях раны в коже, отступя от шва на 2-3 см, делают разрезы-проколы до полости, сформировавшейся при иссечении копчикового хода. В сформированные разрезы вводят силиконовые дренажные трубки вдоль швов. Укладывают между краями раны до дна марлевую салфетку. Далее через дренажи проводят санацию очага воспаления. Салфетки систематически меняют до прекращения экссудации из раны. После чего салфетку и дренажи удаляют. Описание методикиDescription of the technique
После обработки операционного поля 70%-ным раствором этилового спирта, в положении больного по Депажу, через первичное свищевое отверстие проводим прокрашивание эпителиального копчикового хода раствором бриллиантового зеленого с раствором перекиси водорода 3%-ным. Далее проводим ревизию полости зондом для уточнения границ удаления копчикового хода. Двумя полуовальными кожными разрезами проводим экономное иссечение эпителиального копчикового хода с патологически измененными тканями в пределах здоровых. Выполняем тщательный гемостаз. Далее производим укрепление крестцово-копчиковой фасции по предложенной методике: по средней линии дна раны, отступя по 0,2 см в каждую сторону от средней линии, накладываем простой непрерывный обвивной шов 2 нитью «Викрил-рапид» 3/0 (ПГА-рапид, Сафил-квик) с атравматической иглой 3 на крестцово-копчиковую фасцию 1 (фиг.1). Контроль на гемостаз. Затем производим ушивание раны по новой методике: отступя 1,2 см от края раны, делаем вкол иглы с нитью в кожу и подкожную клетчатку (фиг.2), проводим через дно раны под волокнами крестцово-копчиковой фасции с наложенным на нее непрерывным швом на другую сторону раны (фиг.3) и выкалываем иглу через кожу на противоположной стороне на расстоянии 1,2 см от края раны, затем делаем вкол на расстоянии 1,5 см вбок от выкола и на 0,3 см от края раны (фиг.2), и проводим иглу с нитью через кожу, подкожную жировую клетчатку, волокна фасции под наложенным на них непрерывным швом, и через дно раны, подкожную клетчатку другой стороны (фиг.3), выкалываем через кожу на расстоянии 0,3 см от края раны и 1,5 см вбок от первого вкола. Далее вкалываем иглу с нитью на расстоянии 1,5 см вбок от выкола и 0,3 см от края раны так, чтобы этот вкол был на одной перпендикулярной краю раны линии с первым (фиг.2), проводим иглу обратно через подкожную клетчатку, под фасцией с наложенным на ней непрерывным швом на противоположную сторону (фиг.3), и выкол делается на расстоянии 0,3 см от края раны и на 1,5 см от точки вкола параллельно краю раны. Далее вкол делается на расстоянии 1,2 см от края раны и 1,5 см вбок от точек выкола (фиг.2), проводится игла с нитью через кожу и подкожную клетчатку, через дно раны под волокнами крестцово-копчиковой фасции с наложенным на нее непрерывным швом на другую сторону раны (фиг.3) и выкалываем иглу через кожу на противоположной стороне на расстоянии 1,2 см от края раны и 1,5 см от точек вкола (фиг.2). Далее, затягивая швы, сближаем края раны до середины и завязываем.
