Abstract
Description
The invention relates to medicine and can be used in the treatment of patients with epithelial coccygeal passage.
It is known that the surgical treatment of the epithelial coccygeal tract, especially with its purulent complications, still, despite its apparent simplicity, causes controversy among proctologists and surgeons, for example, whether it is necessary to simply sanitize the abscess in the abscess stage, by draining it widely and hoping for that the inflammation will no longer recur or immediately try to eliminate the source of infection - the epithelial coccygeal passage. There is a particularly great debate about the last step in completing a radical operation, in particular, how to complete the operation and leave the wound open or sutured.
In recent years, the method of excision of the epithelial coccygeal passage with suturing the edges of the wound to the bottom and its modifications, such as the method of open wound management, the method of partial suturing of the wound, the method of suturing the edges of the wound to the bottom, the method of deaf suturing of the wound, the method of skin plasty and the method of suturing the wound with two cruciform continuous sutures.
In particular, there is a method for suturing a wound during a radical operation for an epithelial coccygeal tract [RU 2305499, C1, A61B 17/00, 10.09.2017], which includes the application of parallel sutures with the capture of the bottom of the wound at a distance of 2 cm from each other, followed by tightening and tying the ends of each pair of parallel sutures on one and the other side of the wound until its edges are in full contact.
The disadvantage of this method is the relatively low efficiency of suturing due to non-guaranteed completeness of contact of the edges of the wound.
The closest in technical essence to the proposed one is a method for the surgical treatment of chronic inflammation of the epithelial coccygeal tract [RU 2736688, C1, A61B 17/00, 11/19/2020], including preoperative sanitation of fistulas, excision of the epithelial coccygeal tract, purulent fistulas, infiltrates and wound closure, at the same time, the skin and subcutaneous tissue are dissected by two semi-oval incisions within healthy tissues and 2-3 cm above and below the primary opening of the passage with the capture of secondary fistulous passages without opening their lumen, moreover, the pathological focus is excised within healthy tissues in such a way that in its middle part, the maximum excision depth, in the upper and lower edges of the incision - the minimum, in the form of a navicular-shaped wound, in the upper and lower corners of the wound in the perpendicular direction, additional incisions are made on the gluteal regions on the affected side with a length equal to 1/3 of the buttocks standing height plus width from the midline to the edge of the wound, for this length the skin from under skin tissue is mobilized, the subcutaneous fat is not wedge-shaped, the resulting wound is sutured with skin plasty with a displaced flap from the bottom up: the subcutaneous tissue is sutured to 2/3 of the depth of the wound with a double absorbable thread, interrupted sutures are tied, then the ends of the first and second threads on both sides intradermally they capture the edges of the wound, which are brought down and fixed to the bottom of the partially sutured wound by tying the ends of each thread, obtaining two interrupted intradermal sutures at a distance of 1.5 cm from each other, followed by repetition depending on the length of the wound, additional incisions of the postoperative wound are sutured with an intradermal suture .
The disadvantage of the closest technical solution is the relatively high complexity due to additional incisions in the gluteal region, the relatively low suturing efficiency due to the non-guaranteed completeness of contact of the wound edges, which can lead to complications and increase the healing time.
The problem that is solved in the invention is to develop a fairly simple method of wound closure after removal of the epithelial coccygeal tract with cicatricial changes in cellular tissue within normal tissue and providing high closure efficiency with guaranteed complete contact of the wound edges, accelerated wound healing, reducing the possibility of complications.
The required technical result is to increase the efficiency of wound closure in the surgical treatment of the epithelial coccygeal tract while expanding the arsenal of technical means that can be used in the surgical treatment of the epithelial coccygeal tract.
The problem is solved, and the required technical result is achieved in a method for suturing a wound during a radical operation for an epithelial coccygeal tract, which consists in the fact that in the patient's position according to Depage through the primary fistulous opening, the epithelial coccygeal tract is stained with methylin blue (in order to determine the size and there are or streaks), an audit of the cavity is carried out with a probe to clarify the boundaries of the removal of the coccygeal passage, two semi-oval skin incisions are made to economically excise the epithelial coccygeal passage within normal tissue, thorough hemostasis is performed, then, retreating 0.6 cm from the edge of the wound, a needle with an antibacterial thread is injected into skin and subcutaneous tissue and deepen the needle to the bottom of the wound, capturing the fascia of the sacrum and coccyx without damaging the periosteum, while the needle is punctured through the skin 0.6 cm from the edge of the wound on the opposite side and then a skin suture 1.0 cm long is applied, then injection through the skin edge of the wound, then, through the bottom of the wound, capturing the skin and subcutaneous tissue, the needle is passed to the opposite side of the wound and punctured, i.e. suturing the wound goes like lacing shoes, but only with a skin jumper and when these threads cross at the bottom of the wound, and when sutured, they gradually tighten. Then a skin suture 1.0 cm long is again applied, after which sutures are sutured in the same way on the other side of the wound along the bottom of the wound, the sutures are crossed and both threads are tied at the distal end and 3 or 4 pairs of continuous sutures are applied and the wound defect is gradually tightened, and on the proximal end of the thread is tied with a separate interrupted suture.
Three pairs of continuous sutures are applied for an uncomplicated epithelial coccygeal passage with three primary fistulous openings, when the size of the cyst along the length is up to 7 cm, and four pairs of continuous sutures are applied for an uncomplicated coccygeal passage with four primary openings, when the size of the cyst along the length is up to 10 cm.
This method makes it possible to ensure complete contact of the edges of the surgical wound, using two threads that cross at the bottom of the wound, while skin sutures 1.0 cm long are formed on the skin.
Thus, two continuous cruciform sutures in the postoperative period make it possible to control the healing process, reduce the depth, length and width of the wound, prevent the occurrence of complications and thereby speed up the recovery time.
For control on the second day after the operation, the edges of the wound are moved apart effortlessly and examined. If necessary, the wound is washed with an antiseptic solution or a water-soluble ointment is administered. With an uncomplicated course, the wound heals according to the type of primary intention on the seventh or eighth day. As a rule, the stitches are removed on the eighth day.
