Abstract
Description
Изобретение относится к области медицины, а именно к вопросу лечения больных эпителиальным копчиковым ходом и его осложнений.The invention relates to medicine, namely to the treatment of patients with epithelial coccygeal passage and its complications.
Целью изобретения является разработка новых оригинальных способов ушивания раны после удаления эпителиального копчикового хода с рубцовыми изменениями клетчатки в пределах нормальной ткани, располагающегося в межъягодичной складке копчиковой области.The aim of the invention is the development of new original methods of suturing a wound after removal of the epithelial coccygeal passage with cicatricial changes in fiber within normal tissue located in the intergluteal fold of the coccygeal region.
Известно, что оперативное лечение эпителиального копчикового хода, особенно при его гнойных осложнениях до сих пор, несмотря на кажущуюся простоту, вызывает споры среди проктологов и хирургов: следует ли в стадии абсцесса просто санировать гнойник путем широкого дренирования его и надеяться на то, что воспаление больше не повторится или же сразу попытаться ликвидировать источник инфекции - эпителиальный копчиковый ход.It is known that surgical treatment of the epithelial coccygeal passage, especially with its purulent complications, still, despite its apparent simplicity, causes controversy among proctologists and surgeons: should the abscess simply be sanitized by extensive drainage and hope that the inflammation is greater it does not happen again or immediately try to eliminate the source of infection - the epithelial coccygeal passage.

Все методы оперативного вмешательства можно разделить на 6 групп:All surgical methods can be divided into 6 groups:

1. Метод открытого ведения раны1. The method of open wound management
После ликвидации воспалительного процесса в отсроченном порядке во время операции после иссечения эпителиального копчикового хода с множественными затеками с целью адекватного дренирования и очищения рану в послеоперационном периоде заживляют открытым способом. Осложнения в ближайшем послеоперационном периоде составляют 7,4%, в отдаленном периоде 8%.After the elimination of the inflammatory process in a delayed order during the operation after excision of the epithelial coccygeal passage with multiple streaks in order to adequately drain and clean the wound in the postoperative period, heal in an open way. После ликвидации очага инфекции при небольшом абсцессе с наличием незначительной инфильтрации операция заканчивается частичным ушиванием краев раны. Этот метод более рациональный, но число осложнений в ближайшем послеоперационном (21%) и в отдаленном периоде (10%) и сроки заживления раны (в среднем 35 дней) также остаются значительными и формируется грубый рубец.After the elimination of the focus of infection with a small abscess with a slight infiltration, the operation ends with partial suturing of the edges of the wound. После иссечения эпителиального копчикового хода и тщательного гемостаза с целью уменьшения раневой поверхности проводится подшивание кожных краев раны ко дну в «шахматном» порядке. Метод - более прогрессивный и уменьшает сроки заживления раны, но отмечается деформация послеоперационного рубца в области копчика. При этом методе число осложнений в ближайшем послеоперационном периоде составляет 7,9%, в отдаленном периоде 4,9%, а сроки временной нетрудоспособности больных составили 26 дней.After excision of the epithelial coccygeal passage and thorough hemostasis in order to reduce the wound surface, the skin edges of the wound are sutured to the bottom in a checkerboard pattern. 5. Метод кожной пластики5. Method of skin grafting
Применяется для закрытия послеоперационных ран обширных размеров. Но часто возникающий некроз кожи значительно продлевает сроки заживления раны и оставляет неровные грубые послеоперационные рубцы. Однако в работе некоторых авторов приживление лоскутов отмечается в 90-100% наблюдений.It is used to close extensive postoperative wounds. 6. Метод ушивания раны П-образными швами6. The method of suturing wounds with U-shaped sutures
Этот метод дает возможность проконтролировать процесс заживления, предупредить возникновение осложнений и тем ускорить сроки выздоровления. На этот метод нами получен патент на изобретение №2189182 от 20 сентября 2002 года. При этом методе нам удалось снизить число послеоперационных осложнений в ближайшем послеоперационном периоде до 3,5%, в отдаленном периоде до 3,1%, а сроки временной нетрудоспособности больных сократились до 16 дней. Однако при глубокой ране технически неудобно накладывать П-образные швы и иногда происходит прорезывание нитей в глубине раны, что приводит к расхождению краев раны и тем самим увеличиваются сроки заживления.This method makes it possible to control the healing process, prevent the occurrence of complications and thereby speed up the healing time. Предлагаемый нами метод оперативного лечения эпителиального копчикового хода, а именно: наложение двух параллельных вертикальных швов для сближения краев раны, дающих возможности контролировать и управлять процессом заживления раны, является оригинальным и более эффективным методом радикального лечения этого заболевания.Our proposed method of surgical treatment of the epithelial coccygeal passage, namely: the application of two parallel vertical sutures to approximate the edges of the wound, which make it possible to control and manage the healing process of the wound, is an original and more effective method of radical treatment of this disease.
Описание методикиDescription of the technique
В положении больного по Депажу через первичное свищевое отверстие проводим прокрашивание эпителиального копчикового хода метилиновой синью. Далее проводим ревизию полости зондом для уточнения границ удаления копчикового хода. Двумя полуовальными кожными разрезами проводим экономное иссечение эпителиального копчикового хода в пределах нормальной ткани. Выполняем тщательный гемостаз. Затем производим завершающий этап по новой методике: отступя 0,6 см от края раны, делаем вкол иглы с антибактериальной нитью в кожу и подкожную клетчатку и иглу проводим с дна раны, захватывая фасцию крестца и копчика, не повреждая надкостницу. Выкол через кожу делается в 0,6 см от края раны на противоположной стороне (фиг.1). На расстоянии 0,3-0,4 см от первой нити так же параллельно проводится вторая нить (фиг.2). Следующую вторую пару параллельных швов накладывают в 2 см от первой пары. Далее аналогичным образом накладывают 3 или 4 пары швов. Затем первую пару параллельных швов одновременно завязывают на один узел и затягивают с одной и другой стороны раны до соприкосновения края раны и окончательно завязывают двумя узловыми швами с каждой стороны раны (фиг.3). Таким же образом завязываются все последующие пары параллельных швов, что обеспечивает полное соприкосновение краев операционной раны (фиг.4).In the position of the patient according to Depage through the primary fistulous opening, we stain the epithelial coccygeal passage with methyline blue. A puncture through the skin is made 0.6 cm from the edge of the wound on the opposite side (Fig. 1). At a distance of 0.3-0.4 cm from the first thread, the second thread is also held in parallel (Fig. 2). The next second pair of parallel seams is placed 2 cm from the first pair. Next, in a similar manner impose 3 or 4 pairs of seams. Then the first pair of parallel sutures is simultaneously tied to one knot and tightened on one and the other side of the wound until the wound edge comes into contact and finally tied with two interrupted sutures on each side of the wound (Fig. 3). In the same way, all subsequent pairs of parallel sutures are tied, which ensures complete contact of the edges of the surgical wound (Fig. 4).

Для контроля на второй день после операции мы раздвигаем края раны без усилий и проводим ее осмотр. При необходимости можно промывать рану антисептическим раствором или ввести мазь на водорастворимой основе. При неосложненном течении рана заживает по типу первичного натяжения на 7-8 день.
