Наиболее близким аналогом изобретения является способ закрытия стомальной раны [Randomized clinical trial of short-term outcomes following purse-string versus conventional closure of ileostomy wounds. Существующий метод, заключающийся в закрытии стомальной раны кисетным швом, имеет существенные недостатки: большой размер дефекта, остающегося на брюшной стенке после наложения кисетного шва, значительно повышает риск инфицирования раны, удлиняет сроки ее заживления, служит предпосылкой для образования кожного рубца больших размеров. Больной Р., 69 лет, поступил в плановом порядке с жалобами на наличие колостомы. Из анамнеза: 3 месяца назад пациент был оперирован в хирургическом стационаре, из срединного доступа была выполнена резекция сигмовидной кишки с наложением двуствольной сигмостомы по поводу перфоративного дивертикулита. После стабилизации состояния и проведенного обследования пациенту выполнено восстановление кишечной непрерывности следующим путем: окаймляющим разрезом стома иссечена из передней брюшной стенки до брюшной полости, кишка подготовлена для наложения анастомоза. Край стомы резецирован, сформирован колоколоанастомоз конец в конец однорядным непрерывным швом монофиламентной нитью. После этого выполнено ушивание брюшины, апоневроза и подкожной клетчатки. Кожная стомальная рана круглой формы, размером 5 см в диаметре, была ушита следующим образом: с помощью резорбируемой монофиламентной нити 3-0 сформирован кисетный шов с захватом дермального слоя кожи с шагом стежка 7-10 мм. Кисетный шов затянут.Дополнительно наложены узловые стежки той же нитью между противоположными краями раны для сближения и затянуты. При этом образовался дефект неправильной звездчатой формы в диаметре до 1 см. Была вырезана коллагеновая пластина (например, из коллагеновой рассасывающейся пластины «Коллост») соответственно форме и размеру звездчатого дефекта. В центре пластины было сформировано отверстие размером 3-4 мм для обеспечения дренажа из раны. Коллагеновая пластина фиксирована к образовавшемуся дефекту узловыми швами резорбируемой нитью 4-0. Наложена асептическая повязка. В послеоперационном периоде проводились обезболивающая терапия и перевязки. Через дренажное отверстие в коллагеновой пластине осуществлялся самостоятельный дренаж сукровичного отделяемого из раны. Послеоперационный период протекал без осложнений. В послеоперационном периоде продолжено проводимое лечение, на фоне которого состояние пациента улучшилось. В последующем на месте ушитой стомальной раны образовался мягкий эластичный рубец размерами около 5-7 мм. Рубец имеет эстетичный вид, заживление произошло в короткие сроки, инфицирования не выявлено.Patient R., 69 years old, was admitted routinely with complaints of colostomy. The stoma edge was resected, end-to-end bell anastomosis was formed with a single-row continuous suture with a monofilament thread. After that, the suturing of the peritoneum, aponeurosis and subcutaneous tissue was performed. A round cutaneous stoma wound, 5 cm in diameter, was sutured as follows: a purse-string suture was formed using a resorbable monofilament suture 3-0, capturing the dermal layer of the skin with a stitch pitch of 7-10 mm. The purse string suture is tightened. In addition, knotted stitches with the same thread are applied between the opposite edges of the wound to bring together and tightened. In this case, a defect of irregular stellate shape was formed in diameter up to 1 cm. A collagen plate (for example, from a collagen absorbable plate "Collost") was cut out according to the shape and size of the stellate defect. A 3-4 mm hole was formed in the center of the plate to allow drainage from the wound. The collagen plate is fixed to the resulting defect with interrupted sutures with a resorbable 4-0 suture. An aseptic bandage was applied. In the postoperative period, anesthetic therapy and dressings were performed. Through the drainage hole in the collagen plate, an independent drainage of the secretion from the wound was carried out. The postoperative period was uneventful. In the postoperative period, the ongoing treatment was continued, against which the patient's condition improved. Subsequently, at the site of the sutured stomal wound, a soft elastic scar of about 5-7 mm in size was formed. The scar has an aesthetic appearance, healing took place in a short time, no infection was detected.
