Известен способ лечения врожденного высокого стояния лопатки по методу Фищенко /методические рекомендации «Клиника и лечение болезни Шпренгеля», авт. Ю.А.Веселовский. Л., 1981 г., с.7-9, рис.2-3/. Способ лечения врожденного высокого стояния лопатки включает: полуциркулярный разрез кожи, подкожной клетчатки и поверхностной фасции от верхнего до нижнего угла лопатки, выделение внутреннего края лопатки, ее поднадкостничное выделение по передней и задней поверхностям, удаление добавочной кости или фиброзного тяжа, фиксирующего лопатку к позвоночнику. Далее у основания пересекают клювовидный отросток, лопатку низводят до уровня здоровой и фиксируют подшиванием нижнего угла к подлежащему ребру.A known method of treating congenital high standing of the scapula according to the method of Fischenko / guidelines "Clinic and treatment of Sprengel disease", ed. Задачей предлагаемого изобретения является повышение эффективности оперативного лечения, путем исключения возможности развития рецидива заболевания за счет функционально выгодной фиксации лопатки - подшивание подвздошно-реберной мышцы к нижнему углу низведенной лопатки.The objective of the invention is to increase the effectiveness of surgical treatment, by eliminating the possibility of developing a relapse of the disease due to the functionally beneficial fixation of the scapula - suturing the ileo-rib muscle to the lower corner of the reduced scapula.
Технический результат поставленной задачи достигается тем, что в способе лечения врожденного высокого стояния лопатки, заключающемся в мобилизации ее нижнего угла, медиального края, остеотомии клювовидного отростка и низведении лопатки; предлагается выполнить миотомию подвздошно-реберной мышцы на уровне 5, или 6, или 7, или 8 ребра и мобилизацию ее нижней части на протяжении 1-5 см (в зависимости от возраста пациента и тяжести заболевания). Затем проксимальный конец нижней порции подвздошно-реберной мышцы подшивают к нижнему углу низведенной лопатки, что обеспечивает ее надежную фиксацию в достигнутом положении.The technical result of the task is achieved by the fact that in the method of treatment of congenital high standing of the scapula, which consists in mobilizing its lower corner, medial edge, osteotomy of the coracoid process and bringing down the scapula; It is proposed to perform myotomy of the ileo-costal muscle at the level of 5, or 6, or 7, or 8 ribs and mobilization of its lower part for 1-5 cm (depending on the age of the patient and the severity of the disease). Способ осуществляется следующим образом.The method is as follows.
Производят дугообразный разрез вдоль медиального края лопатки 1, из окружающих тканей выделяют ее нижний угол и медиальный край. Мобилизуют переднюю поверхность лопатки 1; выполняют остеотомию клювовидного отростка. Выполняют миотомию подвздошно-реберной мышцы 2 на уровне 5, 6, 7 или 8 ребра и мобилизуют лоскут 1-5 см из нижней порции указанной мышцы. Лопатка 1 низводится до уровня здоровой и к ее нижнему углу подшивают сформированный лоскут 3 из нижней порции подвздошно-реберной мышцы.
