Известно множество методик укрытия перфоративного отверстия, которые основываются на использовании собственных тканей (аутопластика). Все они включают разрез передней брюшной стенки, ревизию и выведение органа с местом перфорации в рану, наложение первого ряда сквозных узловых швов для сближения краев язвенного дефекта и наложение в последующем второго ряда узловых швов. В качестве прототипа выбран способ ушивания прободного отверстия при язвах изолированным сальником, А.А.Шалимов, В.Ф.Саенко, включающий сближение краев перфоративного отверстия первым рядом швов /без вворачивания/, затем наложение второго ряда серозно-мышечных швов с участком сальника, подшивая его по окружности [1]. Способ осуществляют следующим образом. После выведения перфоративного отверстия в рану на него накладывают лоскут 1 сетчатого аллотрансплантата, по размеру превышающий размер язвенного инфильтрата, для надежной фиксации в пределах здоровых тканей. Затем одним узловым швом 2 (фиг.1) ушивают края язвенного дефекта и фиксируют центр лоскута 1, после чего углы лоскута 1 фиксируют отдельными узловыми швами 3. Затем прошивают края лоскута непрерывным швом (на фиг.1 не показано). Сверху лоскута 1 аллотрансплантата укладывают прядь 4 большого сальника (фиг.2), которую фиксируют двумя-тремя шовными нитями 5 по углам. После санации и дренирования брюшной полости производят ушивание лапаратомной раны.The method is as follows. Результаты эксперимента подтвердили, что в экспериментальной группе, так же как и в контрольной, рубцевание язв происходило на 30 сутки, т.е. наличие сетки не оказывало замедления регенеративных процессов. При этом не наблюдалось сужения просвета полого органа, т.к. соединительная ткань, прорастающая на всем протяжении через сетчатый аллотрансплантат, замещала подслизистый слой, выполняя функцию каркаса стенки, а прядь большого сальника формировала слизистый слой (серозный покров) органа, отграничивая его от других органов брюшной полости.The experimental results confirmed that in the experimental group, as well as in the control group, ulcer scarring occurred on day 30, i.e. the presence of the grid did not slow down the regenerative processes.
