Наиболее близким решением является способ диагностики и лечения механической желтухи, описанный О.Д.Лукичевым, В.Г.Ившиным, Г.А.Старченко и др. (Хирургия им. Н.И.Пирогова, №1, М.: Медицина, 1990 г., с.10-14), который предусматривает поэтапное выполнение ультразвукового исследования органов брюшной полости, гастроскопии, эндоскопической ретроградной панкреатохолангиографии (ЭРПХГ), лапароскопии и чрескожной чреспеченочной холангиографии (ЧЧХГ) для диагностики причины механической желтухи, а так же дает возможность осуществить дренирование билиарного тракта. Поставленная задача решается тем, что при ультразвуковом исследовании гепатопанкреатобилиарной зоны выявляются предлагаемые типы желчной гипертензии, которые определяют дальнейшую хирургическую тактику у этой группы больных. I тип желчной гипертензии предполагает ретенцию по данным УЗИ желчных путей и желчного пузыря вплоть до конфлюенса; при II типе желчной гипертензии присоединяется расширение внутрипеченочных желчных ходов вплоть до III порядка.The problem is solved in that the ultrasound examination of the hepatopancreatobiliary zone reveals the proposed types of biliary hypertension, which determine the further surgical tactics in this group of patients. Больная Н-ая, 22 лет поступила в хирургическое отделение областной клинической больницы г. Курска 26.11.2003 г. с жалобами на коликообразные боли в правом подреберье, тошноту, рвоту, пожелтение кожных покровов. Вышеописанные симптомы появились 3 дня назад. За медицинской помощью не обращалась. В день обращения интенсивность болевого синдрома резко возрасла, рвота приобрела многократный характер. После выполнения клинико-биохимического лабораторного исследования крови и мочи, выполнения УЗИ органов брюшной полости поставлен диагноз: ЖКБ, холецистохоледохолитиаз, дистальный блок холедоха, желчная гипертензия I типа. Проведена инфузионная терапия, включающая спазмолитики, аналгетики. 27.11.2003 г. фиброгастродуоденоскопия с осмотром БСДК, ЭРПХГ - заключение: контрастирован гепатикохоледох до 15 см в диаметре, дефектов наполнения не отмечается, имеется коническое сужение дистального отдела на протяжении 1 см. Выполнена эндоскопическая папиллосфинктеротомия, литоэкстракция. Послеоперационный период без осложнений. Получала инфузионную терапию кристаллоидными растворами. 29.11.2003 г. - ЭРПХГ - контроль: стеноз БСДК ликвидирован, отмечается свободный сброс контраста в двенадцатиперстную кишку. 01.12.2003 г. в плановом порядке под эндотрахеальным наркозом выполнена ВЛХЭ. Послеоперационные период протекал без осложнений. Швы сняты 06.11.2003 г. Заживление per prima. The postoperative period was uneventful. Sutures were removed on November 6, 2003. Healing per prima. In satisfactory condition, she was discharged under the supervision of a surgeon in the clinic.

Больной З-а, 47 лет, поступил в хирургическое отделение областной клинической больницы г. Курска 29.04.2003 г. с жалобами на тупые ноющие боли в правом подреберье, тошноту, рвоту, желтушность кожных покровов в течение 2-х недель. За медицинской помощью не обращался. На кануне отмечает многократную рвоту, приступообразный характер болей, появление кожного зуда. После выполнения клинико-биохимического лабораторного исследования крови и мочи, выполнения УЗИ органов брюшной полости поставлен диагноз: ЖКБ, холецистохоледохолитиаз, дистальный блок холедоха, желчная гипертензия II типа. Назначена инфузионная, спазмолитическая, аналгетическая терапия. 30.04.2003 г. выполнена ЧЧХС под ультразвуковым контролем; антеградная холангиография - заключение: контрастирован гепатикохоледох до 2,5 см в диаметре, в дистальном отделе дефект наполнения до 2,5 см в диаметре, коническое сужение дистального отдела протяженностью до 1,5 см, сброса контраста в двенадцатиперстую кишку нет. Получал инфузионную, спазмолитическую, аналгетическаую и антибиотикотерапию. 05.05. 2003 г. выполнена фиброгастродуоденоскопия с осмотром БСДК, эндоскопическая папиллосфинктеротомия, литотрипсия и литоэкстракция. Послеоперационный период без осложнений. 08.05.2003 г. - контрольное УЗИ - заключение: признаков желчной гипертензии не визуализировано; ЭРПХГ - контроль: стеноз БСДК ликвидирован, отмечается свободный сброс контраста в двенадцатиперстную кишку. 13.05.2003 г. в плановом порядке под эндотрахеальным наркозом выполнена ВЛХЭ. Послеоперационный период протекал без осложнений. Швы сняты 18.05.2003 г. Заживление per prima. The postoperative period without complications. 05/08/2003 - control ultrasound - conclusion: signs of biliary hypertension are not visualized; ERPC - control: stenosis of BSDC is eliminated, a free discharge of contrast into the duodenum is noted. 05/13/2003, in a planned manner under endotracheal anesthesia performed VLHE.
