Изобретение относится к области медицины, а именно к диагностике, онкологии и урологии, и может быть использовано для периоперационной оценки уровня стриктуры мочеточника у пациентов с нефростомическим дренажом. The invention relates to the field of medicine, namely to diagnostics, oncology and urology, and can be used for perioperative assessment of the level of ureteral stricture in patients with nephrostomy drainage.
Стандартным способом визуализации дефекта мочеточника является антеградная (нисходящая) пиелоуретерография. Данный метод исследования основывается на визуализации верхних мочевых путей за счет введения рентген-контрастного вещества в почечную лоханку через нефростомический дренаж, либо с помощью чрескожной пункции. Антеградная пиелоуретерография позволяет визуализировать просвет исследуемого мочеточника, тем самым давая возможность судить о наличии дефекта мочеточника (стриктура, обтурация) и его уровне относительно отделов мочеточника.The standard way to visualize a ureteral defect is antegrade (descending) pyeloureterography. Несмотря на возможность проведения непосредственно рентгеноскопии, т.е. выполнение процедуры в режиме реального времени, целесообразность интраоперационной антеградной пиелоуретерографии представляется сомнительной, ввиду необходимости использования дополнительного оборудования, привлечения технического персонала, трудности в соблюдении стерильности операционного поля и участников оперативного вмешательства, а также значимых временных затрат во время проведения операции в целом и лучевого воздействия на пациента и операционную бригаду. Противопоказаниями к проведению данной методики являются инфекции кожи и мягких тканей в поясничной области; состояния, сопровождающиеся нарушением свертываемости крови [1].However, antegrade pyeloureterography has a number of significant disadvantages and limitations. Известны другие способы визуализации дефектов мочеточников -компьютерная томография (КТ) в режиме внутривенной урографии. Данный способ визуализации основывается на способности почек захватывать из крови рентген-контрастное вещество (KB), концентрировать и выделять с мочой. Тем самым позволяя судить о размере и форме почечных лоханок и мочеточников, а также идентифицировать конкременты и участки сужения мочевых путей. Пациентам медленно, в течение 2 минут, вводят 20 мл раствора KB в одну из периферических вен, чаще всего в вену локтевого сгиба [2]. Очевидным преимуществом метода является относительная простота и доступность исследования. Противопоказаниями к проведению экскреторной урографии являются аллергическая реакция на контрастное вещество, шок, коллапс, тяжелые заболевания печени и почек с выраженной азотемией, гипертиреоидизм, сахарный диабет, гипертоническая болезнь в стадии декомпенсации и беременность. Недостатками способа является ограничение применения у пациентов с хронической почечной недостаточностью [1] и невозможность периоперационной визуализации.There are other known methods for visualizing ureteral defects - computed tomography (CT) in intravenous urography mode. Contraindications to excretory urography are an allergic reaction to a contrast agent, shock, collapse, severe liver and kidney diseases with severe azotemia, hyperthyroidism, diabetes mellitus, hypertension in the stage of decompensation and pregnancy. The disadvantages of this method are the limited use in patients with chronic renal failure [1] and the impossibility of perioperative visualization.

Указанный технический результат достигается в способе периоперационной оценки уровня стриктуры мочеточника у пациентов с нефростомическим дренажом, включающем визуализацию с контрастированием, в котором после мобилизации одного или двух пораженных мочеточников осуществляют внутрипросветное введение 15 мл раствора красителя индоцианина зеленого (IGG) в концентрации 2 мг/мл через имеющийся нефростомический дренаж, во флуоресцентном режиме краситель визуализируют в просвете мочеточника, верифицируют уровень стриктуры мочеточника, мочеточник клипируют на 2-3 мм проксимальнее уровня стриктуры. Проведена интраоперационная визуализация уровня стриктуры левого мочеточника по заявляемому способу ICG-визуализации с выполнением оперативного вмешательства в объеме иссечения стриктуры мочеточника с формированием уретероцистоанастомоза по методике Демеля:Intraoperative visualization of the level of the stricture of the left ureter was carried out using the proposed ICG visualization method with surgical intervention in the scope of excision of the ureteral stricture with the formation of ureterocystoanastomosis according to the Demel method:
В положении больной на спине под общей анестезией, после обработки операционного поля, в мочевой пузырь введен уретральный катетер Фолея №20 с 10 мл NaCl в баллоне. Выше пупка на 1 см вскрыта брюшная полость, установлен троакар. Наложен пневмоперитонеум (Р=14 мм.рт.ст). Дополнительно установлены два троакара слева и один справа. Отмечается спаечный процесс, локализованный преимущественно в области малого таза -следствие проведенной химиолучевой терапии по поводу рака шейки матки. Выполнена мобилизация левого мочеточника, пересечена круглая связка матки слева. Визуализирована нижняя треть мочеточника - вовлечена в спаечный процесс (Фиг. 2). Принято решение о проведении флуоресцентной визуализации с ICG-красителем. Через имеющийся у пациентки нефростомический дренаж слева введено 15 мл раствора индоцианина зеленого ICG в концентрации 2 мг/мл: во флуоресцентном режиме краситель визуализируется в просвете левого мочеточника, не доходя до мочевого пузыря около 2,5 см (Фиг. 3-4). Принято решение о клипировании левого мочеточника на 2-3 мм проксимальнее уровня стриктуры, и выполнении уретеронеоцистоанастомоза по Демелю. Краситель извлечен из левого мочеточника. Экстраперитонизирован мочевой пузырь на всем протяжении. Мочевой пузырь рассечен в поперечном направлении, вытянут в продольном направлении. Проксимальнее стриктуры левого мочеточника наложена клипса Hem-O-Lok (Фиг. 5), с помощью холодных ножниц мочеточник пресечен, выполнена спатуляция дистального края. Сформирован уретеронеоцистоанастомоз 10-ю узловыми швами с использованием 3 нитей Vicryl 4-0 по методике Демеля. В левый мочеточник установлен стент Ch7. Оставшийся дефект стенки мочевого пузыря продольно ушит непрерывным швом с использованием 1 нити Vicryl 3-0. An adhesive process is noted, localized mainly in the pelvic area - a consequence of chemoradiotherapy for cervical cancer. The left ureter was mobilized and the round ligament of the uterus was transected on the left. The lower third of the ureter is visualized - it is involved in the adhesive process (Fig. 2). A decision was made to perform fluorescence imaging with ICG dye. Through the patient’s existing nephrostomy drainage on the left, 15 ml of indocyanine green ICG solution at a concentration of 2 mg/ml was injected: in fluorescent mode, the dye is visualized in the lumen of the left ureter, not reaching the bladder about 2.5 cm (Fig. 3-4). A decision was made to clip the left ureter 2-3 mm proximal to the level of the stricture and perform ureteroneocystoanastomosis according to Demel. The dye was extracted from the left ureter. The bladder is extraperitoneated throughout. The bladder is dissected in the transverse direction and extended in the longitudinal direction. A Hem-O-Lok clip was applied proximal to the stricture of the left ureter (Fig. 5), the ureter was cut off using cold scissors, and the distal edge was spatulated. A ureteroneocystoanastomosis was formed with 10 interrupted sutures using 3 Vicryl 4-0 threads according to the Demel technique. A Ch7 stent was installed in the left ureter. The remaining bladder wall defect is closed longitudinally with a running suture using 1 3-0 Vicryl suture. Completing the operation.
Заявляемый способ периоперационной оценки стриктур мочеточников с использованием раствора индоцианина зеленого (ICG) для проведения внутрипросветной флуоресцентной визуализации позволяет достоверно визуализировать уровень стриктуры мочеточника в режиме реального времени непосредственно в операционной ране с сохранением изначальных условий стерильности, не прибегая к задействованию дополнительного оборудования и персонала, в отсутствии источников излучения, без излишнего иссечения здоровых тканей, в том числе у пациентов после операций в области малого таза и забрюшинного пространства, а также со сниженной почечной функцией.The inventive method for perioperative assessment of ureteral strictures using indocyanine green (ICG) solution for intraluminal fluorescence imaging allows reliably visualizing the level of ureteral stricture in real time directly in the surgical wound while maintaining the original sterile conditions, without resorting to the use of additional equipment and personnel, in the absence radiation sources, without unnecessary excision of healthy tissue, including in patients after operations in the pelvic area and retroperitoneal space, as well as with reduced renal function.
