Известны способы хирургических операций, при которых осуществляют закрытие дефекта барабанной перепонки трансплантатом (мирингопластика). При осуществлении таких операций трансплантат изготавливают либо в виде кожно-слизистого лоскута из слизистой оболочки щеки, либо из хряща перегородки носа, либо из среза декальцинированной кости [1]. Недостатками известных способов являются значительная травматичность, относительная техническая сложность выполнения операции, а также относительно высокая вероятность возникновения различных осложнений в послеоперационном периоде, понижающих функциональный результат.Known methods of surgical operations, in which the closure of the defect of the tympanic membrane with a graft (myringoplasty). Известен способ мирингопластики [2], при котором закрытие дефекта барабанной перепонки осуществляют трансплантатом, изготовленным из аутохряща, который берут из козелка или ушной раковины и подвергают обработке путем медленного раздавливания зажимом Кохера в продольном и поперечном направлении до нужной толщины. Изготовленный таким образом трансплантат после раздавливания становиться тонким, подвижным, цельным, сохраняет предназначенную ему форму, сгибается во всех направлениях; под микроскопом представляет треугольники, связанные между собой тонкими волокнами. После хирургической санации среднего уха аутохрящ накладывается на сохраненные структуры косточек и на стенки наружного слухового прохода, на него помещается височная фасция, затем укладывается меатотимпанальный лоскут с барабанной перепонки или ее остатки. Недостатком известного способа мирингопластики является относительная сложность механической обработки аутохряща, что требует значительного времени и удлиняет время операции, а также возникновение осложнений, связанных с регидностью барабанной перепонки в послеоперационном периоде.A known method of myringoplasty [2], in which the closure of the defect of the tympanic membrane is carried out by a graft made of auto-cartilage, which is taken from a tragus or auricle and subjected to processing by slow crushing with a Kocher clamp in the longitudinal and transverse directions to the desired thickness. Эта задача решается тем, что в известном способе мирингопластики, включающем изготовление трансплантата из полоски аутохряща козелка или ушной раковины, наложение полученного трансплантата на сохраненные структуры косточек и остатки барабанной перепонки, покрытие трансплантата височной фасцией и укладку на нее остатков меатотимпанального лоскута, при заготовке трансплантата после отделения полоски хряща от козелка или ушной раковины ее перфорируют и в результате получают трансплантат из хряща с отверстиями в виде сетки. При этом экспериментально обосновано, что размеры отверстий сетки должны быть не более 2 мм. Заготовленный таким образом трансплантат обладает необходимыми качествами для замещения перфорированной барабанной перепонки, а именно упругостью, мягкостью, легко поддается любому моделированию, хорошо адаптируется, кроме того, такой трансплантат создает хорошие условия для вентиляции барабанной полости, способствует улучшению микроциркуляции тканевой жидкости барабанной полости в послеоперационном периоде. Больная А., 42 года, поступила в ЛОР отделение с диагнозом: правосторонний тотальный мезотипанит, стадия ремиссии. При отоскопии установлено почти полное отсутствие барабанной перепонки с сохранением ободочка, слизистая бледная, в барабанную полость выступает рукоятка молоточка, на аудиограмме имеется кондуктивная форма тугоухости, костно-воздушный разрыв в среднем 35-40 дБ, слух справа - 0,5 м шепотной речи, разговорная речь - 1,5 м, слева слух в пределах нормы. Под местной анестезией осуществлена операция мирингопластики с аттиноантротомией заушным подходом, где обнаружен вялотекущий воспалительный процесс. Произведена диэпидермизация остатков барабанной перегородки, ревизия цепи слуховых косточек, которые функционировали. На деэпидермизацированные остатки барабанной перепонки и рукоятку молоточка уложена полоска аутохряща сетчатого вида, сверху помещена височная фасция, затем уложены остатки меатотимпанального лоскута. Послеоперационный период протекал гладко. На 16-й день выписана домой с улучшением слуха 5-6 метров шепотной речи. Осмотрена больная через 6 месяцев. Слух в пределах 6 метров. Барабанная перепонка подвижна, регидность отсутствует. Больной П., 48 лет, поступил в ЛОР отделение с диагнозом: левосторонний хронический гнойный отит. Полость после радикальной операции, правосторонний адгезивный отит. Жалобы на понижение слуха на левое ухо. При микроскопии в левом ухе обнаружена полость после радикальной операции, в барабанной полости слизистая оболочка сохранена, бледная, имеются остатки ножки стремени, подножная пластинка функционирует, в переднем отделе барабанной полости сохранены частично остатки барабанной перепонки, зашпорная область эпидермизирована. Шепотную речь воспринимает около ушной раковины, разговорную - 1 метр. Проходимость слуховой трубы сохранена. На правой барабанной перепонке рубцы. Шепотная речь - 3 метра. На аудиограмме определяется костно-воздушный разрыв на левое ухо до 40-45 дБ, на правое ухо 25-30 дБ, кондуктивная форма тугоухости. Под местной анестезией произведена тимпанопластика с мастоидопластикой с мышечно-периостальным лоскутом с восстановлением задней стенки наружного слухового прохода. Операция произведена заушным подходом, снята эпидермально-рубцовая ткань зашпорной области. Произведена деэпидермизация остатков барабанной перепонки, установлена калюмелла по патенту №2159095 на подножную пластинку стремени. Аутохрящ сетчатого вида наложен первым слоем на остатки барабанной перепонки и калюмеллу, зашпорная область заполнена мышечно-периостальным лоскутом, задняя стенка восстановлена аутохрящем носовой перегородки, сверху покрыта фасцией височной мышцы, а заушная область заполнена эпидермально-рубцовой тканью. Послеоперационный период протекал гладко, эпидерммизация закончилась к 25 дню, то есть ко дню выписки. Шепотная речь при выписке составила 4 метра. Больной осмотрен через 6 месяцев, барабанная перепонка бледно-серого цвета, подвижная, задняя стенка восстановлена, шепотная речь до 5 метров. Under local anesthesia, tympanoplasty with mastoidoplasty with a muscular-periosteal flap with restoration of the posterior wall of the external auditory canal was performed. The operation was performed by the behind-the-ear approach; the epidermal-scar tissue of the locking region was removed. The epidermisation of the remnants of the tympanic membrane was carried out, the calyumella according to patent No. 2159095 was installed on the foot plate of the stirrup. The reticular auto-cartilage is superimposed with the first layer on the remnants of the tympanic membrane and the calyumella, the locking region is filled with a muscular-periosteal flap, the posterior wall is restored by the auto-cartilage of the nasal septum, the fascia of the temporal muscle is covered from above, and the behind-the-ear region is filled with epidermal-scar tissue. The postoperative period was uneventful, epidermization ended by day 25, that is, by the day of discharge. Whispering speech at discharge was 4 meters. The patient was examined after 6 months, the eardrum was pale gray, mobile, the back wall was restored, whispering up to 5 meters. There is no bone-air gap on the audiogram.

При отоскопии выявлено наличие перфорации барабанной перепонки округлой формы в натянутой части размерами 4х4 мм. Слизистая оболочка бледная, проходимость слуховой трубы 1-й степени. На аудиограмме выявлена кондуктивная форма тугоухости с костно-воздушным интервалом 30-35 дБ. Шепотная речь на правое ухо - 1 метр, разговорная речь - 3 метра. Ватно-марлевый протез, уложенный на диффект барабанной перепонки, улучшает слух до 6 метров. Под местной инфильтративной анастезией заушным подходом произведена мирингопластика. После снятия эпидермиса с остатков барабанной перепонки освежения краев перфорации, ревизии аттика первым слоем уложен сетчатый аутохрящ из ушной раковины, сверху покрыта фасцией височной мышцы. Послеоперационный период протекал гладко. Больная выписана на 14 день с улучшением слуха до 6 метров шепотной речи. Больная осмотрена через полгода, барабанная перепонка бледно-серого цвета, подвижная, признаков рубцевания, втяжения не обнаружено. На аудиограмме костно-воздушный разрыв отсутствует. After removing the epidermis from the remnants of the tympanic membrane, refreshing the edges of the perforation, revision of the attic, the first layer is the mesh auto-cartilage from the auricle, the fascia of the temporal muscle is covered from above. The postoperative period was uneventful. The patient was discharged on day 14 with improved hearing to 6 meters of whispering speech. The patient was examined after six months, the eardrum is pale gray, mobile, signs of scarring, retraction were not found. There is no bone-air gap on the audiogram. Whispering speech - 6 meters.

Больной И., 35 лет, поступил в ЛОР отделение с диагнозом: левосторонний посттравматический средний отит с жалобами на наличие шума в ухе и понижение слуха. При отоскопии установлено наличие субтотального дефекта барабанной перепонки, слизистая бледная, проходимость слуховой трубы 1-й степени, ватно-марлевый протез улучает слух до 6 метров, а без него - 0,5 метров. На аудиограмме определяется костно-воздушный разрыв 35 дБ. Больному произведена мирингопластика заушным подходом. Снят эпидермис с остатков барабанной перепонки, после снятия костного навеса осмотрена цепь слуховых косточек, связь которых не нарушена с подножной пластинкой стремяни. На дефект барабанной перепонки уложена полоска аутохряща из ушной раковины сетчатого вида, сверху покрыта фасцией височной мышцы, слух улучшился на операционном столе. Послеоперационный период протекал гладко. Выписан домой на 14-й день с улучшением слуха до 6 метров. Повторный осмотр через 6 месяцев: барабанная перепонка бледно-серого цвета, подвижная, аудиограмма показывает отсутствие костно-воздушного разрыва, шепотная речь составляет 6 метров.Patient I., 35 years old, was admitted to the ENT department with a diagnosis of left-sided post-traumatic otitis media with complaints of noise in the ear and hearing loss. Re-examination after 6 months: the eardrum is pale gray, mobile, the audiogram shows the absence of a bone-air gap, whisper speech is 6 meters.
Предложенный способ апробирован на 25 больных. Результаты мирингопластики положительные, что позволило восстановить у пациентов целостность барабанной перепонки и слуховую функцию.The proposed method is tested on 25 patients. В сравнении с прототипом предложенный способ мирингопластики прост в выполнении, уменьшается время операции. При этом предложенный способ предупреждает возникновение осложнений в послеоперационном периоде, а именно регидности барабанной перепонки. Достигаются эти результаты благодаря тому, что заготовленный вышеописанным способом трансплантат успешно выполняет функцию естественной барабанной перепонки и способствует образованию воздушной подушки под барабанной перепонкой, тем самым создаются условия для ускорения эпидермизации барабанной перепонки. Таким образом, предложенный способ обеспечивает восстановление структуры барабанной перепонки и слуховой функции.In comparison with the prototype, the proposed method of myringoplasty is simple to perform, the operation time is reduced.
