Целью изобретения является снижение травматичности вмешательства с достижением более жесткой и стабильной фиксации фрагментов при переломах верхней челюсти и, как следствие, уменьшение частоты возникновения послеоперационных осложнений и сроков сращения фрагментов при переломах. Для достижения этой цели разработан новый способ фиксации фрагментов при переломах верхней челюсти по верхнему типу. The aim of the invention is to reduce the invasiveness of the intervention with achieving stiffer and more stable fixation of the fragments during fractures of the upper jaw and, as a result, reducing the incidence of postoperative complications and the timing of the coalescence of fragments during fractures. To achieve this goal, a new method of fixation of fragments in the upper jaw fractures of the upper jaw was developed.
Суть способа заключается в следующем. Под местным или общим обезболиванием после предварительной ручной репозиции отломков под контролем прикуса на верхнюю и нижнюю челюсти накладывают назубные проволочные шины с зацепными петлями. Затем производят разрез кожи в области основания скулового отростка височной кости и скелетируют фрагменты. Металлическую спицу проводят из надскуловой области кнутри от скулового отростка височной кости, книзу через подвисочную ямку к бугру верхней челюсти с выходом в полость рта и изгибают ее по форме верхней зубочелюстной дуги. Наружный конец спицы изгибают крючком вокруг скулового отростка височной кости и во избежание соскальзывания крючка с отростка в проксимальный его отдел, насквозь, вводится с помощью электродрели вторая спица, концы которой скусываются. Общая длина второй спицы не должна превышать 1,5 см. Продолжая ручную репозицию отломков, спицу, изогнутую в полости рта по форме верхней зубочелюстной дуги, в состоянии натяжения фиксируют лигатурной проволокой к назубной проволочной шине. То же проделывают с противоположной стороны. При необходимости проводят коррекцию прикуса межчелюстной резиновой тягой. Наступает стабильная удовлетворительная фиксация. Производят ушивание раны в области основания скулового отростка височной кости. The essence of the method is as follows. The outer end of the knitting needle is crocheted around the zygomatic process of the temporal bone and, in order to prevent the hook from sliding off the process, the second spoke is inserted through the electric drill through the ends, the ends of which bite. The total length of the second knitting needle should not exceed 1.5 cm. Continuing the manual reposition of the fragments, the knitting needle, bent in the oral cavity in the form of the upper dentition, is fixed in the state of tension by the ligature wire to the tooth wire splint. Do the same from the opposite side. If necessary, bite correction is performed by intermaxillary rubber traction. There is a stable satisfactory fixation. The wound is sutured in the region of the base of the zygomatic process of the temporal bone.

После общеклинического обследования и наложения назубных проволочных шин с зацепными петлями на обе челюсти проведена операция: остеосинтез верхней челюсти металлическими спицами по новой методике. Суть операции. Под премедикацией, проводниковой (по Вайсблату с блокадой II ветви тройничного нерва) и инфильтрационной анестезией 2% раствором новокаина 20,0 мл отломки репонированы ручным способом и фиксированы металлическими спицами справа и слева, проведенными из надскуловой области кнутри от предварительно скелетированного скулового отростка височной кости книзу и внутрь через подвисочную ямку в полость рта, где после ручной репозиции отломков и удержания их в правильном положении спицу натягивают и изгибают по форме верхней зубочелюстной дуги. Затем избытки спиц скусывают и спицы фиксируют лигатурной проволокой к назубной проволочной шине. Наружный конец спицы изгибают крючком вокруг скулового отростка височной кости и во избежание соскальзывания крючка с отростка в проксимальный его отдел насквозь вводят вторую спицу, концы которой скусывают. Длина этой спицы не более 1,5 см. Отломки верхней челюсти прочно фиксированы. Наложена межчелюстная резиновая тяга. Затем рана в области скуловой дуги ушита, линия швов туширована раствором бриллиантовой зелени, наложена асептическая повязка (фиг.1, 2). Ранее известен способ фиксации фрагментов при переломах верхней челюсти по методу Фальтина-Адамса, заключающийся в следующем: после проведения ручной репозиции отломков и наложения гнутых проволочных шин с зацепными петлями на обе челюсти под анестезией остро и тупо выделяют скуловой отросток лобной кости чаще путем наружного доступа и после просверливания отверстий через них проводят проволочную петлю, концы которой с помощью толстой инъекционной иглы вводят в преддверие рта и крепят к назубной шине. То же проделывают с другой стороны. По истечении 3-4 недель после остеосинтеза лигатуры извлекают путем подтягивания. Новизна предлагаемого способа состоит в том, что все указанные недостатки устраняются и появляется возможность осуществить:
a) более стабильную фиксацию на всем протяжении отломков;
б) меньшую травматичность при проведении манипуляций, а значит, снижение вероятности инфицирования;
в) поддержание гигиены полости рта и возможность открывания и закрывания рта, что значительно улучшает трофику тканей и ускоряет репаративные процессы, если нет необходимости в наложении межчелюстной резиновой тяги.The novelty of the proposed method lies in the fact that all these disadvantages are eliminated and it becomes possible to implement:
a) more stable fixation throughout the fragments;
b) less trauma during the manipulation, and therefore, a decrease in the likelihood of infection;
c) maintaining oral hygiene and the ability to open and close the mouth, which significantly improves trophic tissue and accelerates reparative processes, if there is no need for the application of intermaxillary rubber traction.
