Целью изобретения является достижение более жесткой и стабильной фиксации фрагментов при переломах в области угла нижней челюсти и как следствие снижение частоты послеоперационных осложнений, ускорение сращения переломов. Для этого разработан новый способ фиксации отломков. Лечение больных с переломами нижней челюсти в области угла при полной или частичной адентии складывается из следующих моментов: под местным или общим обезболиванием, после проведения ручной репозиции отломков по обе стороны от плоскости перелома на расстоянии не менее 1,5 см электродрелью вводят, учитывая плотность (компактность) костной ткани, металлические спицы на глубину высоты тела челюсти, причем в медиальный отломок спицу вводят вертикально или под углом, а в ретромолярный треугольник под углом к окклюзионной поверхности нижней челюсти на глубину 2-2,5 см. Направление спицы к углу челюсти. Затем спицы изгибают навстречу друг другу, а излишки скусывают. Металлические спицы фиксируют лигатурными проволоками, затем на них накладывают валик из быстротвердеющей пластмассы. Наступает стабильная фиксация фрагментов при условии, что репозиция и удержание отломков ручным способом продолжались до момента полной полимеризации пластмассы. Валик не должен препятствовать закрыванию рта. При необходимости производят его коррекцию фрезой или бором (фиг.1, 2). For this, a new method for fixing fragments has been developed. The treatment of patients with fractures of the lower jaw in the angle region with full or partial adentia consists of the following points: under local or general anesthesia, after manual reposition of the fragments on both sides of the fracture plane at a distance of at least 1.5 cm, an electric drill is introduced, taking into account the density ( compactness) of bone tissue, metal knitting needles to a depth of the body height of the jaw, moreover, the knitting needle is inserted vertically or at an angle into the medial fragment, and into the retro-molar triangle at an angle to the occlusal surface of the lower h Lust to a depth of 2-2.5 cm. Травма автодорожная, двухнедельной давности. В поликлинике по месту жительства наложены шины Васильева и проведен курс антибактериальной терапии. Общее состояние пациентки удовлетворительное. Местно определяется симптом "ступеньки" при пальпации угла нижней челюсти слева. Больная отмечает онемение кожи подбородка и нижней губы слева (симптом Венсана положительный). На рентгенограмме нижней челюсти в боковой проекции определяется нарушение целостности костной ткани в области угла нижней челюсти слева, смещение дистального отломка кверху. После общеклинического обследования проведена операция: остеосинтез нижней челюсти по методу клиники. Техника операции: под премедикацией и проводниковой анестезией по Берше-Дубову 2%-ным раствором новокаина 30,0 мл произведена ручная репозиция отломков, затем в медиальный отломок вертикально введена металлическая спица на глубину 1,5 см. В ретромолярный треугольник под углом к окклюзионной поверхности нижней челюсти введена вторая спица. Спицы изогнуты по направлению друг к другу, фиксированы лигатурной проволокой, концы скушены. Наложен валик из быстротвердеющей пластмассы. Достигнута стабильная фиксация отломков. Послеоперационный период протекал без осложнений. Места вкола спиц ежедневно обрабатывались раствором бриллиантовой зелени. After a general clinical examination, an operation was performed: osteosynthesis of the lower jaw according to the clinic method. Technique of operation: under premedication and conduction anesthesia according to Bershe-Dubov 2% solution of novocaine 30.0 ml, manual reposition of fragments was performed, then a metal needle was inserted vertically into the medial fragment to a depth of 1.5 cm. In the retro-molar triangle at an angle to the occlusal surface the lower jaw introduced a second spoke. The spokes are bent towards each other, fixed with a ligature wire, the ends are tapered. The roller is made of quick-hardening plastic. Stable fixation of fragments is achieved. The postoperative period was uneventful. Knitting needles were treated daily with a solution of diamond greens.

Аналогом и прототипом предложенного способа является способ фиксации отломков при переломах нижней челюсти по Блэку, суть которого заключается в следующем: после ручной репозиции отломков челюсти накладывается обвивной шов из проволоки с внутренней и наружной стороны челюсти с фиксацией в области заранее изготовленного пластмассового пелота на альвеолярный отросток по обе стороны перелома. Однако данный способ не лишен недостатков. Во-первых, со временем теряется прочность фиксации данной конструкции и необходимо дополнительное подтягивание лигатурной проволоки, а при выполнении данной манипуляции возможен излишний перекрут и отрыв проволоки с необходимостью повторного вмешательства. Во-вторых, под пелотом трудно проследить степень репозиции отломков. В-третьих, под плотно прилегающим пелотом неминуемо образование пролежней вследствие нарушения трофики подлежащих тканей, могут возникать вторичные кровотечения, воспаления с развитием травматического остеомиелита. Новизна предлагаемого способа состоит в том, что все указанные недостатки устраняются и появляется возможность осуществить:
а) более четкую репозицию отломков с возможностью последующего динамического наблюдения за их положением в процессе сращения, что позволяет предотвратить осложнение в виде смещения;
б) отсутствие пролежней и контактных стоматитов, так как пластмассовый валик не плотно прилегает к слизистой альвеолярного отростка, не нарушает трофику подлежащих тканей, а значит, предупреждает вторичные кровотечения, воспаления с развитием травматического остеомиелита и не имеет локального химического воздействия на слизистую;

The novelty of the proposed method lies in the fact that all these disadvantages are eliminated and it becomes possible to implement:
a) a clearer reposition of fragments with the possibility of subsequent dynamic monitoring of their position in the process of fusion, which helps prevent complications in the form of displacement;
b) the absence of pressure sores and contact stomatitis, since the plastic roller does not fit tightly on the mucosa of the alveolar process, does not violate the trophism of the underlying tissues, and therefore prevents secondary bleeding, inflammation with the development of traumatic osteomyelitis and does not have a local chemical effect on the mucosa;
c) stable fixation throughout the fragments.
