Abstract
Description
The invention relates to medicine, namely to reconstructive surgery of the face, and can be used in the treatment of patients with lower macrognatia.
The objective of the invention is to maintain the function of the lower jaw in the early postoperative period and reduce the load on the temporomandibular joint during the establishment of a physiological bite.
Known methods for eliminating this malocclusion are: the method of "stepwise" osteotomy, "spike" connection of fragments of the lower jaw. The fixation of fragments of the lower jaw in the postoperative period in all methods is carried out using tooth splints.
значительная трудность и сложность выпиливания "шипов", и "ступеней";
выключение нижней челюсти из функции при фиксации фрагментов нижней челюсти назубными шинами, что приводит к удлинению сроков консолидации фрагментов, вследствие чего удлиняются сроки лечения в стационаре и реабилитации в послеоперационном периоде;
ухудшение условий и полноценности питания при фиксации фрагментов нижней челюсти назубными шинами;
инфицирование послеоперационной зоны микрофлорой полости рта из-за недостаточного ухода за полостью рта при фиксации фрагментов нижней челюсти назубными шинами;
The disadvantages of these methods are:
significant difficulty and difficulty sawing "spikes", and "steps";
switching off the lower jaw from the function when fixing fragments of the lower jaw with dental splints, which leads to an extension of the periods of consolidation of the fragments, as a result of which the treatment time in the hospital and rehabilitation in the postoperative period is extended;
the deterioration of conditions and nutrition in the fixation of fragments of the lower jaw with dental splints;
infection of the postoperative zone with microflora of the oral cavity due to insufficient care for the oral cavity when fixing fragments of the lower jaw with splint;
lack of a period of adaptation to a new type of bite.
Прикус при использовании указанных методов устанавливают на операционном столе и в дальнейшем его адаптировать нельзя, это ведет к перегрузке височно-нижнечелюстного сустава из-за нового вынужденного положения в суставе. Перегрузка сустава ведет к возникновению острых артритов в послеоперационном периоде;
возникновение ложных суставов и рецидивов при недостаточной фиксации в послеоперационном периоде (назубными шинами) приводит к необходимости повторного оперативного вмешательства.When using these methods, the bite is installed on the operating table and cannot be adapted in the future, this leads to an overload of the temporomandibular joint due to a new forced position in the joint. Overload of the joint leads to the occurrence of acute arthritis in the postoperative period;
the occurrence of false joints and relapses with insufficient fixation in the postoperative period (splint) leads to the need for repeated surgery.
These shortcomings are caused by insufficiently rigid fixation in the postoperative period by the splint, overload of the temporomandibular joint, the inability to adequately hygienic care of the postoperative wound, shutting down the lower jaw from the function.
The goal is to prevent postoperative complications by reducing the load on the temporomandibular joint and preserving the functions of the lower jaw in the early postoperative period.
To achieve this goal, the claimed invention "Surgical method for the treatment of lower macrognathia" includes the following essential features, the totality of which is aimed at solving only one problem associated with the purpose of the invention.
Radiography of the lower jaw is performed in two lateral and direct projections, tele-radiography (TRG), gypsum models are made, and the sizes of the resected areas are calculated. The final course of the operation is set on models gypsum plated in the okludator.
A typical lower jaw osteotomy is performed. Fragments of it are fixed using Kirschner spokes with a compression-distraction apparatus. Wounds are sutured with catgut. In this way, relatively rigid fixation of fragments of the lower jaw is achieved.
In relation to the prototype, the claimed invention has the following distinctive features: the use of a compression-distraction apparatus for fixing fragments of the lower jaw (a.p. N 167352), the installation of a physiological bite gradually in the early postoperative period by activation of the screws of the compression-distraction apparatus according to the examination and x-ray -control, the activation limit of the screws is the appearance of mild soreness in the temporomandibular joint; creating a period of adaptation to a new type of occlusion and the position of the temporomandibular joint.
Between the distinguishing feature and the purpose of the invention there is the following causal relationship: the gradual establishment of a physiological bite and, therefore, dosing the load on the temporomandibular joint by increasing the adaptation period, which prevents the occurrence of complications in the form of arthritis. The use of a compression-distraction apparatus preserves the function of the lower jaw, which allows for adequate hygienic care of the wound and proper nutrition of the patient, prevents the occurrence of false joints and relapses.
Example: Patient S., 33, was admitted to the hospital with a diagnosis of lower macrognathia. A general clinical and x-ray examination was carried out, the size of the resected areas was calculated.
Under endotracheal anesthesia with intubation through the nose, an incision is made in the mucosa and periosteum along the transitional fold in the area From the vestibular and lingual sides, the body part of the lower jaw is skeletonized, and then resected. Kirchner spokes were inserted percutaneously into each fragment of the lower jaw. Using a compression-distraction apparatus, fragments are set in the position of a physiological bite. Wounds were sutured by catgut. Hemostasis during the operation.
A similar operation was performed on the right side.
After surgery, X-ray control of the lower jaw in a direct and two lateral projections.
Активация винтов производилась до установления физиологического прикуса (25 сут на стадии фиброзной мозоли). 3-5 days after surgery, the screws were activated according to the results of examination and x-ray control and according to the individual sensitivity of the patient until mild pain in the temporomandibular joint appeared. X-ray control performed again. Based on the x-ray, the screws of the apparatus are again activated after 3-5 days. The activation of the screws was carried out until a physiological bite was established (25 days at the stage of fibrous callus).
There were no complications in the postoperative period. Wound healing by primary intention. In the early postoperative period, a course of anti-inflammatory, restorative, stimulating therapy.
The described method was applied in 9 patients with lower macrognatia with a good aesthetic and functional result. There were no complications in the near and long term.
The method allows to eliminate lower macrognathia, while providing a full-fledged nutrition function and preserving the function of the lower jaw in the early postoperative period, while ensuring a significant reduction in time, optimal aesthetic anatomical functional results.
In this case, postoperative functional disorders in the temporomandibular joints, the occurrence of false joints, relapses are prevented.
сохранением функции нижней челюсти, полноценностью функции питания в раннем послеоперационном периоде в отличии от фиксации назубными шинами, что значительно сокращает сроки лечения больных в стационаре и реабилитации в послеоперационном периоде;
адекватным гигиеническим уходом за полостью рта и послеоперационной раной, что невозможно осуществить при фиксации наружными шинами;
