Способ фиксации фрагментов при переломах верхней челюсти по верхнему типу с помощью металлических спиц заключается в следующем: после предварительного обезболивания, проведения ручной репозиции отломков и положения на обе челюсти проволочных шин с зацепными петлями производят разрез кожи в области основания скулового отростка височной кости и скелетируют фрагменты; затем проводят металлическую спицу из надскуловой области, кнутри от скулового отростка височной кости, книзу, через подвисочную ямку к бугру верхней челюсти с выходом в полость рта и изгибают ее по форме верхней зубо-челюстной дуги, а наружный конец спицы изгибают крючком вокруг скулового отростка височной кости и, во избежание соскальзывания крючка с отростка в проксимальный его отдел насквозь вводится вторая спица длиной не более 1,5 см; рану ушивают, спицы фиксируют к назубной шине; от приоритетного способа фиксации по методу Фальтина-Адамса новый способ существенно отличается тем, что позволяет осуществить более стабильную фиксацию на всем протяжении отломков, меньшую травматизацию, а значит снижение вероятности инфицирования, поддержание гигиены полости рта и возможность открывания и закрывания рта, что значительно улучшает трофику тканей и ускоряет репаративные процессы.A method for fixing fragments in upper jaw fractures of the upper type using metal knitting needles is as follows: after preliminary anesthesia, manual reposition of fragments and position on both jaws of wire splints with hook loops, a skin incision is made in the region of the base of the zygomatic process of the temporal bone and fragments are skeletonized; then a metal knitting needle is drawn from the supra-zygomatic region, inward from the zygomatic process of the temporal bone, downward, through the infratemporal fossa to the tubercle of the upper jaw with access to the oral cavity and it is bent in the shape of the upper dento-maxillary arch, and the outer end of the spoke is bent with a hook around the zygomatic process of the temporal bone bones and, in order to avoid slipping of the hook from the process, a second spoke of not more than 1.5 cm in length is inserted through the proximal section; the wound is sutured, the needles are fixed to the splint; The new method differs from the priority fixation method according to the Faltin-Adams method in that it allows for more stable fixation along the entire length of the fragments, less trauma, and therefore a reduction in the likelihood of infection, maintaining oral hygiene and the ability to open and close the mouth, which greatly improves trophism tissues and accelerates reparative processes.
