Изобретение относится к медицине и касается способов хирургического лечения заболеваний опорно-двигательной системы, в частности больных с поперечной распластанностью стопы и отклонением первого пальца стопы кнаружи. Известны способы лечения поперечной распластанности стопы с отклонением первого пальца стопы кнаружи:
1. Способ А. Ф. Краснова (А.Ф. Краснов, С.Г. Герасимов. Сухожильно-мышечная пластика при лечении поперечного плоскостопия: состояние сухожильно-мышечного аппарата при травмах и ортопедических заболеваниях. Куйбышев, 1980 - С. 108-112). Цель изобретения - одномоментное исправление деформации стопы и первого пальца, сокращение срока лечения, уменьшение числа осложнений и рецидивов. Сущность заявленного способа заключается в следующем. Мобилизуют первую плюсневую кость путем рассечения капсулы первого плюснепредплюсневого сочленения, удаляют экзостоз плюсневой кости, рассекают наружную порцию капсулы плюснефалангового сочленения, производят стяжку аллотрансплантатом первой и второй плюсневых костей. При этом исправляется распластанность стопы и деформация первого пальца. Первая плюсневая кость прочно фиксируется в заданном положении, что гарантирует от рецидива деформации. К тому же высокая мобильность плюсневой кости снижает напряжение трансплантата при стяжке, это уменьшает возможность перетирания плюсневых костей при нагрузке, что наблюдается в известных способах. Принципиальное отличие предлагаемого способа от известного заключается в том, что мобилизуют первую плюсневую кость путем рассечения капсулы первого плюснепредплюсневого сочленения, производят стяжку первой и второй плюсневых костей аллотрансплантатом, проходящим спереди и сзади указанных костей тотчас проксимальнее их головок. Способ выполняют следующим образом. Производят первый разрез кожи по передней поверхности стопы в проекции первого предплюснеплюсневого сочленения. Под сустав подводят лопаточки Буяльского. Скальпелем рассекают фиброзную капсулу сустава с наружной, передней и задней сторон оставляя участок без рассечения с внутренней стороны (фиг.1). Рану послойно ушивают. The method is as follows. Языкообразный лоскут капсулы отводят дистально и удерживают крючком. Вскрывают сустава. Остеотомом отсекают экзостоз (фиг.1). Ахиллотомом рассекают фиброзную капсулу с наружной стороны в поперечной плоскости. Первый палец устанавливают в правильное положение. Один конец трансплантата проходит по задней, второй - по передней поверхности плюсневой кости. Первую плюсневую кость сдвигают наружи, устанавливая в правильное положение. При необходимости (тонкая капсула) лоскут удлиняют за счет одного из концов аллотрансплантата. Раны послойно ушивают. Накладывают гипсовую повязку на 1 месяц. Первая плюсневая кость легко, без напряжения устанавливается и надежно удерживается в заданном положении трансплантататом, проведенным по передней и задней поверхностям первой и второй плюсневых костей. Незначительное натяжение трансплантата обеспечивает своеобразный биомеханический комплекс: улучшается рессорность стопы, снижается перетирающий эффект трансплантата на плюсневые кости. Пример 1. Больная Ч. 18 лет поступила в клинику травматологии и ортопедии Самарского медуниверситета с диагнозом: двустороннее поперечное плоскостопие, отклонение пальцев стопы кнаружи 10 февраля 2000 года. 14 февраля произведены операции по описанному способу. Больная уложена на спину. Обезболивание - спинно-мозговая анстезия. Первый разрез кожи по передней поверхности правой стопы в проекции первого предплюснеплюсневого сочленения. Распатором отсепарованы мягкие ткани от сустава. Под сустав подведены лопаточки Буяльского. Скальпелем рассечена фиброзная капсула с оставлением мостика с внутренней стороны сустава. Рана послойно ушита. Второй разрез полуовальной формы по передней внутренней поверхности стопы в проекции первого плюснефалангового сочленения. Языкообразным разрезом рассечена капсула сустава. Сустав вскрыт. Под головку плюсневой кости подведена лопаточка Буяльского. Остеотомом сбит экзостоз. Ахиллотомом рассечена капсула с наружной стороны в поперечном направлении. Распатором отслоена надкостница от первой плюсневой кости. Третий разрез кожи по передней поверхности стопы в проекции второй плюсневой кости. Распатором отслоена надкостница от кости. Третья операционная рана соединена со второй тоннелем с помощью корнцанга. Аллотрансплантат заведен за вторую плюсневую кость с наружной стороны, а концы проведены через тоннель в мягких тканях, причем один конец расположен по передней, второй - по задней поверхности кости. Первая плюсневая кость сдвинута кнаружи до своего обычного положения. Концы трансплантата сшиты между собой нерассасывающимися нитями. Языкообразный лоскут уложен на свое место и подшит в положении натяжения к аллотрансплантату. При этом первый палец занял свое обычное положение. Капсула сустава с внутренней стороны укреплена свободным концом аллотрансплантата. Трансплантат натянут умеренно. Раны ушиты. Example 1. The second section is a semi-oval shape along the front inner surface of the foot in the projection of the first metatarsophalangeal joint. A capsule of the joint is dissected with a tongue-shaped incision. The joint is opened. Under the head of the metatarsal bone, Buyalsky’s spatula has been placed. An exostosis is brought down by an osteotome. The capsule is dissected from the outside in the transverse direction by the achillotome. Raspator is detached periosteum from the first metatarsal bone. The third skin incision along the front surface of the foot in the projection of the second metatarsal bone. Raspator detached the periosteum from the bone. A third surgical wound is connected to the second tunnel using a forceps. An allograft is inserted behind the second metatarsal bone from the outside, and the ends are drawn through the tunnel in soft tissues, with one end located on the front and the other on the back surface of the bone. The first metatarsal bone is shifted outward to its normal position. The ends of the graft are stitched together with non-absorbable threads. The tongue-like flap is laid in its place and hemmed in the tension position to the allograft. In this case, the first finger took its usual position. The joint capsule on the inside is strengthened by the free end of the allograft. The graft is tensioned moderately. The wounds are sutured.
На контрольной рентгенограмме стопы отмечено: первая плюсневая кость и первый палец стопы занимают правильное физиологическое положение. Наложен гипсовый сапожок на 1 месяц. Аналогичная операция выполнена на левой стопе. On the control radiograph of the foot it is noted: the first metatarsal bone and the first toe of the foot occupy the correct physiological position. После снятия гипсовой повязки проведен курс реабилитационного лечения. Через 6 недель после операции у больной отмечено восстановление движений в плюснефаланговых сочленениях. Стопы обычной формы, при стоянии и ходьбе болей нет. Назначено ношение супинаторов.
