Предлагаемое изобретение относится к области медицины, а именно к травматологии и ортопедии, и может быть использовано для исправления тыльного отклонения малых пальцев стопы.The present invention relates to the field of medicine, namely to traumatology and orthopedics, and can be used to correct the dorsal deviation of the small toes.
Известен способ лечения патологии малых пальцев стопы, предложенный Helal ([1] Trnka H.J. et al., Comparison of the results of the Weil and Helal osteotomies for the treatment of metatarsalgia secondary to dislocation of the lesser metatarsophalangeal joints. Foot Ankle Int. 1999 Feb;20(2):72-9. P. 74 Trnka H.J., Miihlbauer M., Zettl R., Myerson M.S., Ritschl P.). Сущность указанного метода заключается в следующем: с использованием регионарной анестезии и турникета выполняют продольный кожный разрез, длиной 3 см, непосредственно над плюсневой костью, при одиночной остеотомии. Выделяют шейку плюсневой кости, надкостницу рассекают и отводят. При помощи осцилляторной пилы, под углом 45° к тыльной поверхности плюсневой кости выполняют косую остеотомию в направлении: от дорсально и проксимально к плантарно и дистально. Остеотом используют для увеличения промежутка между фрагментами, что позволяет сместить головку плюсневой кости дорсально и проксимально. Фиксацию остеотомии не выполняют. The essence of this method is as follows: using regional anesthesia and a tourniquet, a longitudinal skin incision is made, 3 cm long, directly above the metatarsal bone, with a single osteotomy. Using an oscillatory saw, at an angle of 45° to the dorsal surface of the metatarsal bone, an oblique osteotomy is performed in the direction: from dorsal and proximal to plantar and distal. An osteotome is used to increase the gap between the fragments, which allows you to displace the head of the metatarsal bone dorsally and proximally. Fixation of the osteotomy is not performed. В зону остеотомии размещают ранее удаленный из головки плюсневой кости костный фрагмент, чтобы удерживать скорректированное положение. Пациентка А., 59 лет. Больной себя считает в течение 15 лет, когда впервые появилась деформация сначала первого пальца, затем второго пальца левой стопы, болевой синдром. В настоящее время пациентка не может носить стандартную обувь, беспокоит боль в переднем отделе стопы, усиливающаяся при ходьбе, и деформация первого и второго пальцев левой стопы. Имеется болезненная омозолелость на тыльной поверхности в проекции проксимального межфалангового сустава второго пальца, возникшая от избыточного тыльного отклонения пальца, трения и давления при ношении обуви (фиг. 3). При клинико-рентгенологическом обследовании выставлен диагноз: Поперечно-продольное плоскостопие II-III степени слева. Вальгусное отклонение первого пальца 3 степени. Неригидная молоткообразная деформация второго пальца. Комбинированная контрактура I-II плюснефалангового сустава. Болевой синдром (фиг. 4 а, b).Patient A., 59 years old. От границы хрящевой части головки плюсневой кости отступили проксимально 2 см и выполнили диафизарную остеотомию, направляя полотно пилы от тыла к подошве стопы в дистальном направлении под углом 45 градусов к оси плюсневой кости. Сместили дистальный фрагмент плюсневой кости по плоскости остеотомии проксимально, поднимая его вверх на 3 мм. Это позволило устранить тыльное отклонение второго пальца стопы на 15 градусов. Костные фрагменты плюсневой кости зафиксировали канюлированным винтом. Остеосинтез стабилен.Then we started correcting the deformity of the second toe of the left foot using the proposed method. Для исправления остаточного тыльного отклонения пальца выполнили остеотомию проксимальной фаланги второго пальца, отступив 0,5 см от его основания, направляя полотно пилы от тыла к подошве параллельно плоскости основания проксимальной фаланги, не нарушая целостность кортикальной пластинки подошвенной поверхности кости. При помощи распатора в зоне остеотомии расширили пространство между костными фрагментами до получения угла в 20 градусов, то есть, по величине равного углу остаточного тыльного отклонения. Сохраняя установленную величину угла, в расширенное свободное пространство в зоне остеотомии проксимальной фаланги второго пальца плотно разместили сформированные костные фрагменты, полученные при коррекции деформации первого пальца стопы. Проведенное пациентке оперативное вмешательство позволило устранить деформацию первого и второго пальца левой стопы. На контрольном осмотре через полтора месяца: пациентка передвигается в стандартной обуви, не хромает, деформации пальцев левой стопы нет, отека нет, послеоперационные рубцы без воспалительных проявлений. В зоне проксимального межфалангового сустава второго пальца болезненности и омозолелости нет (Фиг. 5). По предлагаемому способу было прооперировано 10 пациентов, у которых имелась неригидная молоткообразная деформация малых пальцев стопы, сопряженная с вальгусным отклонением первого пальца. Оценка полученных результатов через полтора месяца после проведенного оперативного лечения показала, что была достигнута коррекция деформации первого и второго пальцев стопы, удовлетворительная консолидация костных фрагментов без смещения и отсутствие миграции винтов. Использование предлагаемого способа позволило исправить избыточное тыльное отклонение проксимальной фаланги малого пальца, избежать репаративных и инфекционных осложнений. Пациенты были удовлетворены полученным результатом, купированием болевого синдрома, внешним видом стопы и возможностью носить стандартную обувь.According to the proposed method, 10 patients were operated on, who had a non-rigid hammertoe deformity of the small toes associated with valgus deviation of the first toe. Таким образом, предлагаемый «Способ исправления тыльного отклонения малого пальца стопы» позволяет повысить эффективность оперативного лечения с учетом индивидуальных особенностей состояния малого пальца стопы пациента, снизить травматичность при выполнении операции и риск инфекционных осложнений, предупредить рецидивы заболевания в послеоперационном периоде, а также восстановить статодинамическую функцию стопы, устранить болевой синдром.
