Предлагаемое изобретение относится к области медицины, в частности к реконструктивно-восстановительной хирургии. The present invention relates to medicine, in particular to reconstructive surgery.
Известны способы лечения послеожоговых контрактур суставов с использованием свободных кожных трансплантатов (см. В.В.Юденич. Лечение ожогов и их последствий. М.: Атлас, 1980, с. 183). Поставленная задача решается за счет того, что в способе, включающем выкраивание пластического материала, иссечение рубцов, замещение дефекта и фиксацию сустава, производят предварительное растяжение рубцовых тканей, частично иссекают патологически измененные ткани, закрывают глубокие анатомические структуры лоскутами из дистрагированных рубцовых тканей и осуществляют дальнейшую дистракцию прижившихся лоскутов из патологически измененных тканей. Иссечены рубцы на локтевом крае кисти и предплечья вплоть до средней трети предплечья, также иссечены рубцы на сгибательной поверхности в дистальной трети предплечья. Выкроен и мобилизован рубцовоизмененный лоскут из дистрагированных тканей на сгибательной поверхности предплечья с вершиной в области проекции лучезапястного сустава размером 3•8 см (соотношение почти 1:3), который ротирован на боковую поверхность локтевого края предплечья, дефект на котором полностью закрыт. Послеоперационный период протекал без осложнений. Клинических проявлений ишемических расстройств не было. Предложенный способ обеспечивает профилактику ишемических осложнений в дистальных отделах конечности, неизбежных при форсированном устранении контрактуры во время операции. Постепенная дистракция рубцовой ткани способствует улучшению ее микроциркуляции, что позволяет использовать ее в качестве пластического материала для выкраивания лоскута на ножке с целью закрытия анатомических глубоких структур. Это особенно важно при лечении больных с тяжелыми контрактурами суставов конечностей после перенесенных критических и сверхкритических ожогов, когда нет возможности выкроить лоскуты кожи с подкожной клетчаткой. Разработанный способ позволяет радикально устранить тяжелую контрактуру сустава, не вызывая при этом расстройств кровообращения благодаря постепенной окончательной коррекции контрактуры. Кроме того, этот способ обеспечивает минимальную травматизацию сустава за счет дозированного растяжения его капсулы и сухожильно-связочного аппарата, что повышает его функциональные возможности и создает условия для безболезненной пассивной и активной разработки движений в суставе в послеоперационном периоде.
