Известен способ фиксации надколенника к бугристости двойной большеберцовой кости (патент РФ N 2012272). Данный способ основан на проведении лавсановой нити в виде "двойной петли" через оторванный фрагмент надколенника к бугристости большеберцовой кости, прошивание связки надколенника швом Кюнео. Применение этого способа позволяет прочно фиксировать фрагмент к бугристости большеберцовой кости, сокращает сроки иммобилизации на 2-3 недели и ускоряет функциональное лечение на 1-3 недели, что предупреждает развитие стойкой контрактуры и способствует более полному восстановлению функции коленного сустава. Однако при повреждении разгибательной системы коленного сустава прочность реконструкции обуславливается используемыми лавсановыми нитями. Способ невыполним при кортикальных фрагментах, имеющих маленький размер, при разрывах связки надколенника и сухожилия четырехглавой мышцы бедра. конструкция из лавсановой нити не является съемной, при контакте лавсановой нити с подкожной клетчаткой могут образовываться свищи сумки;

Способ стабильно функционального остеосинтеза при повреждении разгибательной системы коленного сустава осуществляется следующим образом. Больному выполняется продольное обнажение разгибательной системы коленного сустава, проводится ревизия полости коленного сустава, репозиция фрагментов надколенника с помощью репонирующего зажима, после чего две спицы Киршнера 3 продольно проводятся, таким образом, чтобы концы спиц выступали вне суставной поверхности надколенника. Затем снизу вверх от точки прикрепления связки надколенника, поднадкостнично специальным направителем 2 проводится гибкий силовой элемент в виде металлического тросика 1. Последовательность сборки гибкого силового элемента: вставляем металлический тросик в задний конец направителя, далее обжимаем круглогубцами направитель, чтобы зафиксировать в нем гибкий силовой элемент B, изгибаем дугой направитель для более удобного проведения C. При этом гибкий силовой элемент должен пройти под концами выступающих спиц по силовым траекториям. После кругового проведения гибкого силового элемента его направляющий конец проводится через "скользящую петлю" 5, закрепленную на противоположном конце тросика. Затем направляющий конец гибкого силового элемента проводится внутри связки надколенника 6 к бугристости большеберцовой кости таким образом, чтобы его направление было параллельно ходу основных волокон связки надколенника. В бугристость большеберцовой кости вкручивается спонгиозный канюлированный винт 7. Гибкий силовой элемент в положении натяжения, но при этом связка надколенника не должна деформироваться, фиксируется в головке спонгиозного канюлированного винта фиксирующим устройством. Далее проверяется амплитуда движения в коленном суставе (от 90 до 180o). Выступающие концы спиц Киршнера по силовой диагонали погружаются в костную ткань. Накладываются узловые швы на поддерживающие связки надколенника, производится гемостаз, рана послойно ушивается. Внешняя иммобилизация при этом способе не применяется. Больному было выполнено продольное обнажение разгибательной системы коленного сустава поврежденной конечности. Провели ревизию полости коленного сустава, сопоставили отломки с помощью репонирующего зажима, продольно провели две спицы Киршнера таким образом, чтобы концы спиц выступали вне суставной поверхности надколенника. Затем снизу вверх от срединной точки прикрепления связки надколенника поднадкостнично специальным направителем провели гибкий силовой элемент с петлей на конце, при этом провели его под выступающими концами спиц Киршнера по силовой траектории. После кругового проведения гибкого силового элемента его направляющий конец продели в петлю на противоположном конце, далее направляющий конец провели внутри связки надколенника по направлению основных волокон связки к бугристости большеберцовой кости. Вкрутили в бугристость большеберцовой кости, в середине прикрепления связки надколенника, спонгиозный канюлированный винт. Гибкий силовой элемент в положении натяжения фиксируется к головке винта с помощью фиксирующего устройства. Проверили амплитуду движения в коленном суставе в пределах от 90-180o. Способ стабильно-функционального остеосинтеза при повреждении разгибательной системы коленного сустава использован нами при лечении 19 пациентов с положительным эффектом.
