Известен способ пластики дефектов кожи свободными полнослойным или расщепленным аутодермотрансплантатом. Однако при повреждении глубоких структур пальца свободный кожный неваскуляризованный трансплантат приживается неполностью, поэтому не может быть использован при сочетанной травме. Способ осуществляется следующим образом. The method is as follows.
Заготавливают шаблон, повторяющий величину и форму реципиентного участка. Под проводниковой анестезией на тыльной поверхности соседнего пальца по шаблону формируют и поднимают кожно-подкожный лоскут 1 на тыльных сосудах и нервах. Проксимальнее лоскута из S-образного разреза 2 выделяют тыльную сосудисто-нервную ножку вместе с фасцией 3. Ножку формируют длиной, достаточной для перемещения лоскута. Далее подготавливают подкожный туннель 4, направленный с тыла кисти к области дефекта. Через туннель проводят лоскут, укрывают им дефект тканей и подшивают. При этом фасциально-сосудистая ножка остается в пределах туннеля без натяжения. Донорский участок 5 укрывают расщепленным кожным трансплантатом толщиной 0,5 мм. Первую перевязку осуществляют на 10 сутки. После снятия кожных швов на 12 сутки начинают комплексное физиотерапевтическое лечение, направленное на активную разработку в суставах пальца. Рана 1,3 на 3,5 см, кровоточащая, дно раны - обнаженный сухожильно-связочный аппарат. Под проводниковой анестезией по Усольцевой после хирургической обработки раны были определены размеры дефекта кожного покрова. По шаблону на тыльной поверхности 4 пальца подняли лоскут на тыльном сосудисто-нервном комплексе. Далее в проксимальном направлении выделили на фасции сосудисто-нервную ножку длиной, достаточной для перемещения лоскута на ладонную поверхность 5 пальца. Лоскут с ножкой провели в предварительно подготовленном подкожном туннеле в область дефекта, при этом ножка осталась в пределах туннеля. Лоскут подшили. Донорский участок укрыли расщепленным кожным трансплантатом толщиной 0,5 мм, взятым с внутренней поверхности плеча. Заживление первичным натяжением. Лоскут прижил полностью. По снятию швов, на 12 сутки больная начала комплексное физиотерапевтическое лечение, направленное на активную разработку в суставах пальцев.Example
