Abstract
Description
The invention relates to medicine, namely to dentistry and can be used for the treatment of periodontitis on a mass outpatient basis in dental offices and clinics.
A known method of complex treatment of periodontal disease, including the use of a helium-neon laser, a CO 2 laser in combination with a surgical patchwork operation. The specified method includes 4 main stages: 1) elimination of periodontal inflammation by irradiation with a helium-neon laser light (for example, type ULF-01 "Berry") with the following parameters: power flux density (PMD) 200 250 mW / cm 2 , exposure 60 120 s, 3-5 procedures on the field; 2) folding the flap in the area of pathological periodontal changes with de-epithelization of the inner surface of the periodontal pockets with a beam of a CO 2 laser (for example, the Scalpel-1 type) with a power of 1.5 2 W; 3) irradiation of the periodontium in the area of the surgical field with the light of a helium-neon laser with the parameters specified in paragraph 1 above to eliminate postoperative periodontal inflammation; 4) irradiation with light of a helium-neon laser of this periodontal site to stimulate tissue regeneration processes with the following parameters: PPM 20 50 mW / cm 2 , exposure 60 90 s, 3 5 procedures every other day.
The specified method has a number of disadvantages: laser installations of the ULF-01-Yagoda type and the Scalpel-1 (Scalpel-3) type used for the indicated purposes are very energy-consuming, bulky (height 1.5 1.8 m, weight 70 150 kg), installations of the Scalpel type require cooling from the water supply network, the high cost and significant operating costs to some extent hinder their use; a CO 2 laser beam with a wavelength of 10.6 μm relative to the gingival mucosa is to some extent traumatic, causing postoperative inflammation (traumatic), and the light of a helium-neon laser with a wavelength of 0.63 μm penetrates several tissue depths mm
The development of modern science and technology, especially in the conversion of the defense industry at the high technology level, has led to the creation of new laser technology that is more technically advanced and more effective in terms of medical indicators. For example, instead of helium-neon laser systems, portable (weighing up to 0.5 kg) semiconductor pulsed lasers with a wavelength of 0.85 0.95 μm (for example, such as "Pattern", "Pattern-2k", etc. ) Instead of Scalpel-type laser systems, laser pulsed devices based on erbium emitters (YAG: Er) with a wavelength of 2.94 μm (for example, such as Kameya, Ermed, Carlite with self-cooling, weighing 25-30 kg, etc. are produced .). These laser devices, in addition to technical improvements, have higher (compared to previously produced laser devices) healing qualities.
The present invention solves the problem of reducing the level of surgical injury of periodontal tissues, increasing therapeutic efficacy, reducing the period of treatment of periodontitis and, accordingly, the overall economic effect by reducing the cost of treatment compared to the known method.
The indicated effect (technical and medical) is achieved by the fact that periodontitis is treated in 3 successive stages: 1) elimination of periodontal inflammation with the help of a semiconductor pulsed laser (for example, of the Pattern type) with a wavelength of 0.85 0.95 μm, power 3, 0 3.5 W, frequency 40 100 Hz, exposure time 90 120 s, 2 3 procedures daily; 2) folding the flap in the area of the pathological lesion of the periodontium with de-epithelialization of the inner surface of the periodontal pockets by the beam of an erbium pulsed laser (for example, the Kameya type) with a wavelength of 2.94 μm, a power of 0.2 1.0 J / imp, a frequency of 10 20 Hz ; and 3) irradiation of the operative intervention zone with light from a semiconductor pulsed laser (Pattern type) with a wavelength of 0.85 0.95 μm, a power of 0.5 1.0 W, a frequency of 2 3 kHz, an exposure of 60 90 s, 2 3 procedures in one day.
In order to develop and justify the proposed method for the treatment of periodontitis, hemostasiological and morphological studies were carried out in experiments on laboratory animals (14 rabbits and 265 Wistar rats).
After mathematical analysis (by the type of a full factorial experiment) of the experimental results and identification of the optimal parameters of laser light for these purposes, comparative clinical studies were conducted on 2 groups of patients: 1) 42 patients treated by the known method (1); and 2) 52 patients treated by the proposed method. Both groups of patients were comparable in terms of periodontal pathology, age, gender, and other characteristics.
1) свет полупроводникового импульсного лазера с длиной волны 0,85 0,95 мкм ликвидирует воспаление пародонта более эффективно (по сравнению с гелий-неоновым лазером), в том числе при абсцедирующих формах, в основном применяли 2 процедуры облучения;
When comparing the obtained data, the proposed method for the treatment of periodontal disease showed significant advantages compared with the known method:
1) the light of a semiconductor pulsed laser with a wavelength of 0.85 0.95 μm eliminates periodontal inflammation more efficiently (compared with a helium-neon laser), including with abscessed forms, mainly 2 irradiation procedures were used;
2) after de-epithelialization of the inner surface of periodontal pockets with the light of an erbium pulse laser with a wavelength of 2.94 μm, postoperative gum injury and, accordingly, postoperative inflammation and edema were significantly reduced compared to the control group of patients treated by the known method, because a beam of an erbium pulsed laser produced a pattern of “soft” coagulation of the gum tissue.
3) период регенерации тканей пародонта в области операционной раны под воздействием лазерного света полупроводникового лазера с длиной волны 0,85 0,95 мкм протекал в заметно короткие сроки, в основном требовалось не более 3-х процедур;
4) свет полупроводникового импульсного лазера с длиной волны 0,85 0,95 мкм проникает в ткани на глубину до 60 мм (свет гелий-неонового лазера с длиной волны 0,63 мкм проникает лишь на несколько мм) и не только эффективно ликвидирует воспаление, но и стимулирует нормализацию тканей пародонта после воспаления, в том числе процессы регенерации (по данным рентгенографии уменьшается ширина периодонтальной щели, уплотняется костная ткань вершин альвеол и т.п.), что клинически обусловливает укрепление подвижных (расшатанных) зубов;
5) импульсный режим генерации света полупроводниковыми лазерами с длиной волны 0,85 0,95 мкм обусловливает более высокий лечебный эффект (по сравнению с непрерывным светом гелий-неонового лазера с длиной волны 0,63 мкм) вследствие его адекватности энергетическим параметрам клеток тканей, их резонансным и другим физиологическим свойствам, а также исключения теплового эффекта при относительно высоких величинах мощности (до 4 Вт);
6) при лечении пародонтитов предлагаемым способом у всех больных данной группы (включая с абсцедирующими формами генерализованных пародонтитов) не имелось осложнений, в то время как в контрольной группе, лечившейся по известному способу у 7-и больных наблюдали образование гематом, из которых 4 нагноились;
8) за счет снижения стоимости лазерной аппаратуры и эксплуатационных расходов, сокращения сроков лечения (включая посещение больными поликлиники), повышенной лечебной эффективности, сокращения расхода медикаментов, производительности труда персонала общий экономический эффект был на 35 40% выше по сравнению с известным способом.3) the period of periodontal tissue regeneration in the area of the surgical wound under the influence of laser light from a semiconductor laser with a wavelength of 0.85 0.95 μm proceeded in a noticeably short time, basically no more than 3 procedures were required;
4) the light of a semiconductor pulsed laser with a wavelength of 0.85 0.95 μm penetrates into tissues to a depth of 60 mm (the light of a helium-neon laser with a wavelength of 0.63 μm penetrates only a few mm) and not only effectively eliminates inflammation, but it also stimulates the normalization of periodontal tissues after inflammation, including regeneration processes (according to x-ray diffraction, the width of the periodontal gap decreases, the bone tissue of the alveolar apex becomes denser, etc.), which clinically determines the strengthening of mobile (loose) teeth;
5) the pulsed mode of light generation by semiconductor lasers with a wavelength of 0.85 0.95 μm causes a higher therapeutic effect (compared with continuous light from a helium-neon laser with a wavelength of 0.63 μm) due to its adequacy to the energy parameters of tissue cells, their resonant and other physiological properties, as well as the exclusion of the thermal effect at relatively high power values (up to 4 W);
6) in the treatment of periodontitis by the proposed method in all patients of this group (including with abscessed forms of generalized periodontitis) there were no complications, while in the control group treated by the known method, hematomas were observed in 7 patients, of which 4 were suppurated;
7) the overall treatment of periodontitis in the proposed method was reduced on average by 26% compared with the known method,
8) by reducing the cost of laser equipment and operating costs, reducing treatment time (including patients visiting the clinic), increased therapeutic efficiency, reducing the cost of medicines, labor productivity, the overall economic effect was 35 40% higher compared to the known method.
