Abstract
Полезная модель относится к медицине, а именно к медицинскому инструментарию, и может быть использована при проведении операций на кишечнике при создании анастомоза. Модель выполнена из биоинертного материала в форме 3/4 кольца и разрывом в 90°, с внешним диаметром 5,0 см, шириной 1,3 см и толщиной 1,0 мм, при этом, по внешнему краю через каждые 5,0 мм для укладки шовного материала имеются пазы глубиной 1,0 мм, концы приспособления заовалены и на них в направлении внутрь тела приспособления выполнены отверстия диаметром 1,5 мм для его фиксации во время операции. Как известно, в ходе операции по наложению ручного однорядного анастомоза на тонкой и толстой кишке используют держалки, позволяющие широко сопоставить подслизистый слой. Клинический пример: Больной Н., 63 лет, поступил в клинику с жалобами на вздутие живота, урчание, похудание на 15 кг. За 4 месяца, выделение слизи при акте дефекации. При осмотре фиброколоноскопом у больного выявлена стенозирующая опухоль на расстоянии 25 см от края ануса, эндоскопом не проходима. ПГИ-высокодифференцированная аденокарцинома. При инструментальном исследовании метастазов в печени, легких и забрюшинном пространстве не выявлено. Операция: Срединная лапаротомия. Лимфаденэктомия в бассейне D3, резекция сигмовидной кишки. После подготовки анастомозируемых участков, через отверстие - 1,5 мм была продета одна нить-держалка. Швы, накладуемые на часть кишки, поочередно ложим в пазы через каждые 5,0 мм, глубиной 1,0 мм полезной модели толщиной 1,0 мм, шириной 1,3 см и внешним диаметром 5,0 см Наложен прямой однорядный колоректальный анастомоз с помощью предложенного приспособления. Во время наложения швов исключено было перепутывание нитей анастомоза за счет поочередного наложения и закрепления швов в пазы. Поочередно было выполнено сшивание рассеченной кишки на верхней полуокружности, затем была сформирована нижняя полуокружность анастомоза также с применением данного приспособления. При этом во время формирования колоректального анастомоза отмечено улучшение визуализации области анастомоза в операционной ране. Время операции сокращено на 45 мин по сравнению с аналогичным вмешательством без использования полезной модели. The postoperative period was uneventful and without complications, the patient was discharged 12 days after the intervention.
