Description
Denne oppfinnelse vedrører formulering for behandling av hjerne-, spinal- og nerveskade.
Hjerneskade resulterer i utvikling av motorisk og kognitive svikt som bidrar til den betydelige morbiditet som forekommer blant overlevende etter hjerneskade. Det er dessuten en forekomst som har den høyeste insidens blant samfunnets yngre medlemmer. Hjerneskade er følgelig ansvarlig for det største tap av produktivt liv sammenlignet med enhver annen sykdomsprosess. På tross av dette finnes ingen effektiv terapi for å forbedre utfallet ved hjerneskade. Vi beskriver anvendelsen av en behandlingsmetode som et kraftig farmakologisk inngrep for behandlingen av hjerneskade. Anvendelse av denne terapi forbedrer signifikant både det motoriske og kognitive utfall ved svak til alvorlig eksperimentell hjerneskade, og har også vist seg å ha gunstig effekt ved behandlingen av ryggmargs- og nerveskader.
BAKGRUNN FOR OPPFINNELSEN
Det er velkjent at hjerneskade resulterer i utviklingen av neurologisk svikt gjennom to mekanismer. Den første av disse er kjent som primærmekanismen. Denne inntrer på skadetidspunktet og innbefatter mekaniske prosesser, som f.eks. lacerasjon, riving, strekking og sammenpressing av nervefibre. Det kan gjøres lite for denne type av skade etter at den først har inntrådt. Den andre mekanismen er sekundærskade, som inkluderer biokjemiske og fysiologiske prosesser, utløst av en primærskade, men som manifesteres med tiden etter skaden. Det har vært vist at mye av morbiditeten etter hjerneskade er assosiert med utviklingen av denne sekundærskade. I betraktning av at sekundærskaden utvikler seg fra minutter til dager etter primærhendelsen, eksisterer det et mulighetsvindu for farmakologisk å forhindre denne type skade og betydelig forbedre det resulterende utfall. De faktorer som utgjør den sekundære skade må imidlertid først identifiseres og «antifaktorer» deretter utvikles for å inhibere skadeprosessen.
Våre studier har konsentrert seg om identifisering av sekundære skade-faktorer etter hjerneskade, og utvikling av intervenerende terapier. En av de faktorer som vi tidligere hadde identifisert<1>"<4> som kritiske for å bestemme utfallet etter skade, var hjernens magnesium-ionekonsentrasjon. Dette ion er en regulerende faktor i en rekke biokjemiske og fysiologiske prosesser som aktiveres etter hjerneskade. En nedgang i magnesium-ionekonsentrasjonen ble faktisk observert å forverre skadeprosessen, mens en økning i konsentrasjonen av magnesiumioner ble registrert å svekke skadeprosessen og resultere i et forbedret utfall<5>. Behandlingen av hjerneskade med magnesium har siden vist seg å være effektiv<1,6>"<10> selv når den administreres opptil 24 timer etter primærskaden, og suksessen med behandlingen i eksperimentelle dyrestudier har senere ført til kliniske forsøk ved human hjerneskade.
Substans P er et sensorisk nevropeptid, med ulike typer biologisk aktiviteter, omfattende vaskulære effekter. Substans P gir primært dens effekter gjennom stimulering av NK1 tachykininreseptoren. Kjent teknikk (EP 721778 og US 5610165) beskriver syntesen av selektive, ikke-peptid antagonister av reseptoren for NK1 reseptor som er effektiv i blokkering virkningene av substans P på denne reseptor. På grunn av evnen til disse forbindelser for å hemme virkningen av substans P postulerer disse patenter den potensielle evne til slike midler å ha en fordelaktig virkning i ulike patologier, omfattende de i CNS, hvor substans P kan spille en rolle.
Akkumuleringen av vann i hjernen (cerebralt ødem; hjernesvelling) er en betydelig komponent ved sekundære skadeprosesser i etterkant av en hjerneskade. Gitt at substans P kan spille en rolle i utviklingen av perifert ødem har vi undersøkt hvorvidt substans P kan virke på blod-hjerne-barrieren og hvorvidt blokkering av dens effekter med en NK1 antagonist kan forhindre cerebralt ødem og forsterker den nevrobeskyttende virkning av magnesium.
Kjent teknikk WO 9964009 beskriver også at NK1 reseptorantagonister kan ha en verdi i behandling av kognitive lidelser, spesielt psykiatriske lidelser relatert til angst og depresjon. Imidlertid er det ingen undersøkelse når det gjelder evnen til en NK1 antagonist, enten alene eller i kombinasjon med magnesium, for å forhindre motoriske mangler normalt forbundet med hjerne- og nerveskade.
På tross av minskningen av svikt etter hjerneskade ved hjelp av magnesium-administrering, var det klart at det fremdeles forelå motorisk og kognitiv svikt etter behandlingen. Vi ble spesielt oppmerksomme på at akkumuleringen av vann i hjernen (dvs. cerebralt ødem eller hjerneoppsvulming) fremdeles forelå i yngre dyr og at dette kan utgjøre en betydelig risikofaktor. I en nylig klinisk studie var faktisk senere hjerneoppsvulming ansvarlig for 50% av alle dødsfall registrert i unge ofre for hjerneskade.
OMTALE AV OPPFINNELSEN
Det er derfor et formål med oppfinnelsen å tilveiebringe formuleringer for behandling i forhold til hjerneskade.
Formuleringen omfatter i henhold til ett aspekt ved oppfinnelsen, N-acetyl-L-tryptofan i kombinasjon med magnesiumsulfat, hvor den kombinerte anvendelse resulterer i bedre beskyttelse mot skade enn enten magnesiumforbindelsen eller N-acetyl-L-tryptofan benyttet alene.
Fremgangsmåten omfatter å administrere formuleringen til pasienten som lider av hjerneskade. Alternativt administreres hver av formuleringens komponenter separat eller adskilt ved en tidsforsinkelse som ikke påvirker effektiviteten av behandlingen, f.eks. 1-30 minutter og opptil 24 timer, som diskutert ovenfor.
Substans P er en eksitatorisk neurotransmitter og spiller en rolle ved smerte-overføring, og er et peptid som har strukturen RPKPEEFFGLM-Nhtø. Den er fra hypothalamus, CNS og tarmen og øker glatt muskulaturkontraksjon av G1 -trakten.
Det er kjent at substans P binder til en rekke reseptorer, inklusivt NK1-reseptoren (dvs. neurokinin 1 reseptor), NK2-reseptoren og NK3-reseptoren. Disse reseptorene antas å spille en rolle ved blodovergangen til hjernen.
En substans P antagonist er derfor en substans som inhiberer binding av substans P til hvilken som helst av de reseptorer som det er referert til ovenfor. En liste over egnede substans P antagonister er angitt i de vedlagte tabeller 1, 2 og 3.
Det kan også henvises til NK1-reseptorantagonister beskrevet i US-patent 5.990.125, som utgjør substans P antagonister. Dette har spesifikk referanse til forbindelser av strukturene la, Ib, le, Id, le, X, XVI, XVII, XVIII, XIX, XX og XXI, så vel som andre antagonister som omfatter kinuklidin-, piperidin-etylendiamin-, pyrrolidin-og azabornan-derivater og beslektede forbindelser som oppviser aktivitet som substans P reseptorantagonister som beskrevet i kolonne 33 i US-patentet 5.990.125.
Slike reseptorantagonister kan benyttes med hensyn til doseringene referert til i kolonne 34 av US-patent 4.990.125 og i forskjellige administrasjonsformer, dvs. alene eller med forskjellige farmasøytisk akseptable bærere eller fortynningsmidler, ved peroral administrering eller parenteral administrering, som omtalt i kolonne 34 i US-patent 5990125.
Aktiviteten av forskjellige substanser som substans P reseptorantagonister for tilsvarende bruk, kan også bestemmes gjennom de tester som er referert til i kolonnen 35-36 i US-patent 5990125.
Det kan også refereres til substans P reseptorantagonister beskrevet i US-patent 5977104, inklusivt de forskjellige doseringsformer og doseringer som er referert til i denne referanse.

De farmasøytiske preparatene kan dessuten inneholde konserveringsmidler, solubiliseringsmidler, stabilisatorer, fuktemidler, emulgeringsmidler, søtningsmidler, farvestoffer, aromastoffer, salter for endring av det osmotiske trykk, buffere, maskeringsmidler eller antioksydanter. De kan også inneholde ytterligere andre terapeutisk verdifulle substanser. Betegnelsen «omfattende» benyttet i denne beskrivelse, er å forstå i denne sammenheng. Doseringen kan variere innenfor vide grenser, og kan selvsagt tilpasses de individuelle krav i hvert enkelt tilfelle. I alminnelighet vil en dosering på 1 til 20000 mg per pasient, fortrinnsvis 10 til 5000 mg og mer foretrukket 50 til 2000 mg av substans P reseptorantagonisten, være passende.
I sammenheng med utviklingen av konseptet ifølge oppfinnelsen, ble det av foreliggende oppfinnere fastslått at én grunn for akutt vann-akkumulering i hjernen etter skade, var resultatet av vasogen ødemdannelse. Denne forårsakes av en øket permeabilitet av blod-hjernebarrieren som derved tillater vaskulære proteiner og vann å gå inn i det ekstracellulære rom i hjernen og forårsake svelling. Få studier har undersøkt hvorledes denne økte permeabilitet av blod-hjernebarrieren bidrar til utviklingen av neurologiske svikt etter skade, og ingen studier har undersøkt hvorvidt inhibering av hjernesvelling forbedrer utfallet. Studier av migrene<12,13> har antydet at karsystemet i dura mater (ytre meningeale lag) blir mer permeabelt overfor vaskulære komponenter som resultat av substans P frigjøring. Vi har derfor som hypotese sagt at substans P kan ha en lignende effekt på det cerebrale karsystem, hvor en slik effekt kunne føre til øket permeabilitet av blod-hjernebarrieren og vasogent cerebralt ødem. Vi har videre som hypotese sagt at administreringen av en substans P reseptorantagonist kan forhindre hjernesvelling og utviklingen av forsinket neurologisk svikt etter skade. Denne hypotese var resultatet av vår opp-dagelse referert til ovenfor, at vann akkumulertes i hjernen som resultat av vasogen ødemdannelse.
EKSPERIMENTELL DEL
En rekke kommersielt syntetiserte substans P reseptorantagonister er for tiden tilgjengelige fra leverandører av vanlige forskningskjemikalier, hvilket fremgår av Tabell 1, 2 og 3. Vi valgte å benytte forbindelsen N-acetyl-L-tryptofan på grunn av dens lave lipidløselighet som begrenser dens evne til naturlig å krysse blod-hjernebarrieren, og det faktum at den er relativt billig. Administrering av N-acetyl-L-tryptofan i en intravenøs dose på 246 mg/kg (saltvann som bærer) gitt 30 minutter etter hjerneskade, resulterte i en signifikant forbedring av det kognitive utfall i hjerneskadede dyr, som fastslått ved Barnes Circular Maze. Likeledes var det en signifikant forbedring i motorisk resultat hos dyr som bestemt ved «otarod» testen. Disse forbedrede resultater var åpenbare 24 timer etter hjerneskade og vedvarte i vurderingsperioden på 14 dager. Kontroll (bæremiddel-testede) dyr hadde signifikant dårligere neurologisk resultat enn behandlede dyr til alle undersøkte tidspunkter.
Dyr behandlet med N-acetyl-L-tryptofan hadde en signifikant reduksjon i vann-akkumulering i hjernen (dvs. cerebralt ødem) 24 timer etter skade, sammenlignet med bæremiddelbehandlede kontroller. Dette stemte overens med den observasjon at N-acetyl-L-tryptofan reduserte hjernepenetrering av Evan's blue 5 timer etter skade, den tid som er assosiert med maksimal permeabilitet av blod-hjernebarrieren etter hjerneskade. N-acetyl-L-tryptofan administrert 30 minutter etter hjerneskade reduserte således permeabiliteten av blod-hjernebarrieren og reduserte vasogen ødemdannelse. Det faktum at disse effektene ble registrert med en ikke-permeabel formulering av NK1-antagonisten, tyder på at effektene i stor grad var mediert av vaskulære reseptorer og ikke avhengige av sentrale reseptorer.
Administrering av 24,6 mg/kg N-acetyl-L-tryptofan forbedret også signifikant det kognitive utfall hos hjerneskadede dyr. Medikamentet hadde imidlertid en mindre gunstig effekt på det motoriske resultat. Siden det alltid ble registrert en viss residual kognitiv og motorisk svikt i alle behandlede dyr, var imidlertid de gunstige effektene av behandling med NK1 -antagonisten mindre tydelige når det ble indusert skade av mindre alvorlighet enn skade av en alvorlig natur. Dette er en vesentlig begrensning i betraktning av at svak hodeskade har den største insidens blant hjerneskade-pasienter.
KOMBINASJON AV MAGNESIUM OG N-ACETYL-L-TRYPTOFAN
Den vanligste form av hjerneskade er svak hodeskade. Retningslinjer som vil bli innført neste år (2000) av the World Federation of Neurological Surgeons vil anbefale at alle tilfeller av mindre hodeskade med en hvilken som helst komplikasjon, som f.eks. brekning, kvalme, bevissthetstap eller amnesi, MÅ til et sykehus. Dette vil legge et betydelig press på helsesystemet for adekvat behandling av disse individene, slik at sekundærskade ikke utvikler seg videre. For tiden finnes ingen slik terapi.
Våre resultater med N-acetyl-L-tryptofan tyder på at denne forbindelsen avstenger blod-hjernebarrieren etter hodeskade og reduserer hjernesvelling eller cerebralt ødem. Dette er ekstremt viktig for unge hodeskadeofre som er særlig utsatt for forsinket hjernesvelling. Våre resultater med magnesiumterapi tyder dessuten på at magnesiumbehandling er effektiv til å redusere neurologiske svikt som ikke nødvendigvis er assosiert med den økte blod-hjernepermeabilitet. Vi mener derfor at en kombinasjon av en substans P antagonist med en magnesiumforbindelse eller -salt, vil være en særlig effektiv terapi for behandlingen av hjerneskade uansett alvor.
Kombinert administrering av 246 mg/kg N-acetyl-L-tryptofan pluss 30 mg/kg magnesiumsulfat (intravenøst) resulterte i en sterk svekkelse av både motoriske og kognitive svikt, som var signifikant høyere enn hva som ble oppnådd med hvert medikament isolert (Fig. 1 og Fig. 2).
Hver av forbindelsene i kombinasjonsformuleringen har en rekke egenskaper som gjør den særlig attraktiv for bruk ved hjerneskade.
Substans P (SP) antagonister har vist seg å hurtig forbedre stemningsleiet ved å antagonisere substans P indusert angst. De er derfor effektive til behandling av depresjon etter skade. Ut fra det som er beskrevet ovenfor, er det klart at SP-antagonister reduserer blod-hjernepermeabilitet og inhiberer dannelsen av vasogent ødem og hjernesvelling etter skade eller cerebralt ødem.
Antagonistene har også vært vist å minske smerte. Det finnes et stort antall substans P reseptorer i hippocampus og striatum, de deler av hjernen som er kjent for å være assosiert med innlæring og hukommelse. Inhibering av binding med SP-antagonister kan således forhindre substans P indusert svikt ved innlæring og hukommelse. Våre ovenfor presenterte indikasjoner tyder på at dette kan være tilfelle. Dette har aldri vært vist tidligere. Det finnes faktisk ingen litteratur angående rollen til substans P, eller noe neuropeptid, ved hjerneskade.
Magnesium påvirker over 300 cellulære enzymer. Det er derfor ikke over-raskende at magnesium har tallrike mål, hvor det kan forbedre resultatet. Disse innbefatter blant andre blokkering av glutamat-indusert eksitotoksitet, forbedring av membranstabilitet og reduksjon av produksjonen av reaktive oksygenforbindelser, forbedret energistatus, inhibering av kalsiumkanaler, reduksjon av neurotransmitter-frigjøring, inhibering av mitokondrien «transition pore» åpning og inhibering av apoptose. Det skal bemerkes at det også blokkerer glutamat-indusert frigjøring av substans P. Fysiologisk har magnesium vist seg<14>"<17> å forbedre cerebral blodstrøm, redusere cerebrale vasospasmer og redusere vaskulær ceramid- og prostaglandin-produksjon.
Den kombinerte anvendelse av magnesium og substans P antagonisten resulterer i bedre beskyttelse mot nerveskade enn hvert medikament benyttet alene.
Vi har tidligere vist at magnesium har en gunstig effekt på traumer når det administreres i intravenøse doser varierende fra 16 til 60 mg/kg. Administrert som en intramuskulær injeksjon, varierer den effektive dose fra 45 til 90 mg/kg. Målet er å øke konsentrasjonen av fritt magnesium i blodet til ca. 1,0 mM, som er det dobbelte av den normale konsentrasjon av fritt magnesium i blodet. Gunstige resultater observeres uavhengig av hvilket magnesiumsalt som benyttes.
Våre studier med substans P antagonisten har vist at den effektive i.v. dose varierer fra 24,6 mg/kg til 240,6 mg/kg eller mer, hvor de høyere dosene har en mer gunstig effekt på det motoriske utfall. Disse dosene henger sammen med antagonister som har lav lipidløselighet og derfor begrenset blod-hjernebarriere permeabilitet. En sterkt lipidløselig formulering burde ha de samme gunstige virkninger, men det kan være sentralt medierte bivirkninger som kan være uheldige.
Benyttet i kombinasjon kan formuleringen variere innen området beskrevet for de individuelle komponentene. Vi har oppnådd utmerkede resultater ved å benytte de maksimale i.v. doser som er beskrevet for de individuelle komponentene.
Kombinasjonen magnesium/SP-antagonister forventes å være anvendelig ved følgende tilstander: Som en «førstehjelp» profylaktisk behandling etter traumatisk hjerne
skade.
Som en «førstehjelp» profylaktisk behandling etter mindre hodeskader,
inklusivt konkusjon.
Som en terapi etter ikke-traumatiske hjerneskader, inklusivt slag,
hypoksi og enhver form av hjerneskade når ødem inngår.
Som en vedlikeholdsterapi etter hjerneskade.

Claims (7)
1. Formulering for behandling av hjerne-, ryggmarg- og nerveskade, hvor skaden er forårsaket av lacerasjon, riving, strekking eller sammenpressing av nervefibrene, karakterisert ved at den omfatter N-acetyl-L-tryptofan i kombinasjon med magnesiumsulfat.
2. Formulering ifølge krav 1, karakterisert ved at doseringen av magnesiumsulfat øker den frie magnesiumkonsentrasjon i blodet til 1,0 mM.
3. Formulering ifølge krav 1 eller 2, karakterisert ved at doseringen av magnesiumsulfat fordobler den normale frie magnesiumkonsentrasjon i blodet.
4. Formulering ifølge et hvilket som helst av kravene 1 til 3, karakterisert ved at den er for separat administrering av N-acetyl-L-tryptofan og magnesiumsulfat.
5. Formulering ifølge krav 1,
karakterisert ved at doseringen av N-acetyl-L-tryptofan er 1-20.000 mg per pasient.
6. Formulering ifølge krav 5,
karakterisert ved at doseringen av N-acetyl-L-tryptofan er 10-5000 mg per pasient.
