Description
Foreliggende opprinnelse vedrører anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytiske salter derav for fremstilling av medikamenter for behandling av, eller for å forebygge tilbakefall til, depresjon, angstlidelser, spiseforstyrrelser eller urininkontinens i en pasient.
Depresjon er en av de vanligste mentale sykdommene og har en dødelighetsrate på over 10% i den generelle populasjonen. Depresjon kjennetegnes ved følelser av intens tristhet, desperasjon, forsinket mental aktivitet, konsentrasjonstap, pessimistisk engstelse, opphisselse og selv-nedvurdering (Harrison's Principles of Internal Medicine 2490-2497 (Fauci et al., red., 14. utg., 1998)). Depresjon kan ha fysiske manifestasjoner omfattende søvnløshet, hypersomnia, anoreksi, vekttap, overspising, nedsatt energi, nedsatt libido og oppbrudd av normale døgnrytmer med hensyn til aktivitet, kroppstemperatur og endosinfunksjoner. I realiteten viser så mye som 10-15% av deprimerte individer suicidal oppførsel. R.J. Baldessarini, Drugs and the Treatment of Psychiatric Disorders: Depression and Mania in Goodman and Gilman's The Pharmacological Basis of Therapeutics 431 (9. utg., 1996).
US patent nr. 4,435,419, Epstein et al., beskriver racemisk (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan for anvendelse som et anti-depresjonsmiddel.
Administrering av en racemisk, dvs. 50:50-blanding av (+)- og (-)-enantiomeren av ethvert legemiddel, for eksempel (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan til en pasient kan være ufordelaktig. For det første kan den racemiske blandingen være mindre farmakologisk aktiv enn en av dens enantiomerer, hvilket gjør racemiske legemidler i seg selv ineffektive. For det andre kan den racemiske blandingen være mer toksisk for en pasient enn en av dens enantiomerer, slik at administrering av en racemisk blanding kan føre til uønskede bivirkninger i en pasient.
Følgelig foreligger det et klart behov innen teknikken for en enantiomer av (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan, hvilken enantiomer ville overvinne en eller begge av de tidligere nevnte ulempene.
Sitering eller identifikasjon av enhver referanse i denne delen av søknaden er ikke å anse som en innrømmelse av at en slik referanse utgjør tidligere kjent teknikk i forhold til foreliggende søknad.
Foreliggende opprinnelse vedrører følgelig anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav som hver er i det vesentlige fri for (-)-enantiomeren, for fremstilling av et medikament for behandling av, eller forebyggelse av tilbakefall ved, depresjon i en pasient.
Foreliggende oppfinnelse vedrører videre anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav, som hver er i det vesentlige fri for (-)-enantiomeren, for fremstilling av et medikament for behandling av, eller forebyggelse av tilbakefall ved, en angstlidelse i en pasient.
Oppfinnelsen omfatter videre anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav, som hver er i det vesentlige fri for (-)-enantiomeren, for fremstilling av et medikament for behandling av, eller for forebyggelse av tilbakefall til en spiseforstyrrelse i en pasient.
Endelig vedrører foreliggende oppfinnelse anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav, som hver er i det vesentlige fri for (-)-enantiomeren, for fremstilling av et medikament for behandling av, eller for forebyggelse av tilbakefall til, en urininkontinenslidelse i en pasient.
Som nevnt er (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav i det vesentlige fritt for den tilsvarende (-)-enantiomeren, (-)-l-(3j4-diklorfenyl)-3-azabicyklo[3.1.0]heksan. Denne enantiorenheten kan oppnås ved anvendelse av kromatografiske oppløsningsteknikker så som kiral kromatografi eller simulert teknologi med bevegelig sjikt, eller via kjemisk separasjon ved anvendelse av optisk aktive oppløsningsmidler.
Foreliggende oppfinnelse kan forstås mer fullstendig ved henvisning til den detaljerte beskrivelsen og eksemplene, som er ment å eksemplifisere utførelsesformer av oppfinnelsen.
Det er oppdaget at (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav er overraskende og uventet mer aktive enn (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan for å behandle eller forebygge tilbakefall til depresjon i en pasient.
Betegnelsen "i det vesentlige fri for den tilsvarende (-)-enantiomeren" betyr inneholdende ikke mer enn ca. 5% vekt/vekt av den tilsvarende (-)-enantiomeren, fortrinnsvis ikke mer enn ca. 2% vekt/vekt av den tilsvarende (-)-enantiomeren, mer foretrukket ikke mer enn ca. 1% vekt/vekt av en tilsvarende (-)-enantiomeren.
Betegnelsen "den tilsvarende (-)-enantiomeren" når den benyttes i forbindelse med (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.L0]heksan eller et farmasøytisk akseptabelt salt derav, betyr " (-)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan" eller et farmasøytisk akseptabelt salt derav.
En "pasient" er et dyr innbefattende et dyr så som en ku, ape, hest, sau, gris, kylling, kalkun, vaktel, katt, hund, mus, rotte, kanin og marsvin og er mer foretrukket et pattedyr, og mest foretrukket et menneske.
Betegnelsen "farmasøytisk akseptabelt salt", slik den anvendes her, er et salt dannet fra en syre og den basiske nitrogengruppen av (+)-l-(3,4-diklorfenyl)-3-azabicyklotJ.l.OJheksan. Foretrukne salter omfatter sulfat, citrat, acetat, oksalat, klorid, bromid, jodid, nitrat, sulfat, bisulfat, fosfat, syrefosfat, isonikotinat, acetat, laktat, salicylat, citrat, syrecitrat, tartrat, oleat, tannat, pantotenat, bitartrat, askorbat, suksinat, maleat, gentisinat, fumarat, glukonat, glukaronat, sakkarat, format, benzoat, glutamat, metansulfonat, etansulfonat, benzensulfonat, p-toluensulfonat og pamoat (dvs. 1,1'-metylen-bis-(2-hydroksy-3-naftoat)) salter.
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan, fortrinnsvis det som er i det vesentlige fritt for en tilsvarende (-)-enantiomeren, kan oppnås fra (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan ved anvendelse av kirale kromatografisk fremgangsmåter, så som høy-ytelsesvæskekromatografi ("HPLC") med en egnet, fortrinnsvis kiral, kolonne. (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan er oppnåelig ved anvendelse av fremgangsmåter beskrevet i US patent nr. 4,435,419, Epstein et al.
I en foretrukket utførelsesform oppnås (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan ved å oppløse (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan ved anvendelse av en kiral polysakkarid stasjonærfase og et organisk elueringsmiddel. Fortrinnsvis er polysakkaridet stivelse eller et stivelsesderivat. Fordelaktig kan det anvendes en kiral HPLC-kolonne, for eksempel en CHIRALPAK AD-kolonne fremstilt av Daicel og kommersielt tilgjengelig fra Chiral Technologies, Inc., Exton, Pennsylvania, mer foretrukket en 1 cm x 25 cm CHIRALPAK AD HPLC-kolonne. Det foretrukne elueringsmiddelet er et hydrokarbon-oppløsningsmiddel justert i polaritet med et blandbart polart organisk oppløsningsmiddel. Fortrinnsvis inneholder det organiske elueringsmiddelet et ikke-polart, hydrokarbon-oppløsningsmiddel tilstede i ca. 95% til ca. 99,5% (volum/volum) og et polart organisk oppløsningsmiddel tilstede i ca. 5 til 0,5% (volum/volum). I en foretrukket utførelsesform er hydrokarbonoppløsningsmiddelet heksan og det blandbare polare organiske oppløsningsmiddelet er isopropylamin.
I en annen foretrukket utførelsesform kan en alternativ kromatografisk fremgangsmåte, kjent som simulert bevegelig sjikt (SMB)-kromatografi, anvendes for oppløsningen av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan. SMB blir i økende grad den valgte fremgangsmåten for storskala enantiomerseparasjon innenfor den farmasøytiske industrien (se Chemical and Engineering News, 2001, bind. 70, nr. 20, s. 47).
I nok en foretrukket utførelsesform kan oppløsningen av racemisk (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan for å oppnå (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan oppnås via anvendelsen av optisk aktive oppløsende syrer via dannelsen av og etterfølgende separasjon av de resulterende diastereomere saltene. Vanlig anvendte kirale syrer for dette formålet omfatter: vinsyre og O-acyl vinsyresalter, mandelsyre og O-substituerte mandelsyrer, l,r-binaftyl-2,2'-diylhydrogenfosfat, kamfersyre, kamfersulfonsyre og andre lett tilgjengelige optisk aktive syrer (både kommersielt tilgjengelige og lett syntetiserte).

Terapeutiske anvendelser av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav kan administreres til en pasient, fortrinnsvis et pattedyr, mer foretrukket et menneske, for behandling eller forebyggelse av lidelser, innbefattende, depresjon, angstlidelser, spiseforstyrrelser og urininkontinenslidelser. Uttrykket"behandling" eller "å behandle" kan referere til lettelse av en slik lidelse, eller minst et erkjent symptom på denne. I tillegg refererer "behandling" eller "å behandle" til en lettelse av minst en målbar fysisk parameter, som ikke nødvendigvis erkjennes av pasienten. Videre refererer "behandling" eller "å behandle" til inhibering av utviklingen av en slik lidelse, enten fysisk, for eksempel ved å normalisere et erkjent symptom, fysiologisk, for eksempel normalisere en fysisk parameter, eller begge deler. Endelig refererer "behandling" eller "å behandle" også til forsinkelse av inntreden av en slik lidelse.
Lettelse av depresjon ved anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo [3.1.0] heksan
Etter anvendelsen ifølge oppfinnelsen administreres (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1 .OJheksan eller et farmasøytisk akseptabelt salt derav til en pasient, fortrinnsvis et pattedyr, mer foretrukket et menneske, som et forebyggende middel mot tilbakefall til depresjon. Slik de anvendes her refererer "forebyggelse" eller "å forebygge" til en reduksjon av faren for opptreden av tilbakefall i en pasient som lider av depresjon. I tillegg kan (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav administreres til en pasient som et forebyggende middel mot tilbakefall. I dette tilfellet kan pasienten ha en genetisk disposisjon for depresjon, så som en familiær historie av biokjemisk ubalanse i hjernen, eller en ikke-genetisk disposisjon for depresjon. Følgelig kan (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav anvendes for behandling av en manifestasjon av depresjon og forebyggelse av en annen.
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav er også nyttige for behandling av eller forebyggelse av tilbakefall til endogen depresjon, unipolar depresjon, retardert depresjon, agitert depresjon, bipolar depresjon, post-partum depresjon, depresjon med angst, depresjon med obsesjon, depresjon med en sykdom som forårsaker kramper, dysthymi, årstidsaffektive lidelser, diurnale humørvariasjoner, depresjon forbundet med menopause og depresjon i forbindelse med en medisinsk sykdom, alkohol- eller stoffmisbruk.
Lettelse av angstlidelser ved anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo [3.1.0] heksan
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav er også nyttige for behandling av, eller forebyggelse av tilbakefall til, angstlidelser (for en oversikt over angstlidelser, se Derogatis L.R. & Wise, T.N., "Anxiety and Depressive Disorders in the Medical Patient", Washington, DC, American Psychiatric Press, 1989, og referanser som der er sitert). Slike lidelser omfatter panikklidelser, panikkanfall, agorafobi, generalisert angstlidelse, obsessiv-kompulsive
lidelser, fobiske lidelser og posttraumatiske stresslidelser, hvorav noen er underklassifisert ytterligere nedenfor.

Post-traumatiske lidelser omfatter akutt stresslidelse og post-traumatisk lidelse som ikke er spesifisert på annen måte (NOS).
Andre lidelser som lettes ved anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan
Lidelser som lettes ved anvendelsen av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan omfatter også spiseforstyrrelser og urininkontinens og er ikke begrenset til de spesifikke lidelsene som her er beskrevet, ettersom mange typer lidelser kan manifestere seg fra en spesiell primær lidelse. Spiseforstyrrelser omfatter anoreksia nervosa; overspising; bulimia nervosa, ikke-tømmende type; bulimia nervosa, tømmende type og spiseforstyrrelser som ikke er spesifisert på annen måte (NOS).
Urininkontinens omfatter trang inkontinens; stress inkontinens; overstrøms inkontinens; funksjonell inkontinens; neurogenisk inkontinens og post-prostektomi inkontinens.
Terapeutisk/profylaktisk administrering og preparat oppnådd ifølge oppfinnelsen På grunn av deres aktivitet er (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav fordelaktig nyttige innenfor veterinær- og humanmedisin. Som beskrevet ovenfor er (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav nyttige for behandling av eller forebyggelse av tilbakefall til depresjon, angstlidelser, spiseforstyrrelser og urininkontinens-relaterte lidelser i en pasient.
Når det administreres til en pasient blir (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav fortrinnsvis administrert som komponent av et preparat som eventuelt omfatter en farmasøytisk akseptabel bærer. Preparater oppnådd ifølge oppfinnelsen, som omfatter (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav, administreres fortrinnsvis oralt. Preparatene kan også administreres ved en hvilken som helst annen hensiktsmessig fremgangsmåte, for eksempel ved infusjon eller bolusinjeksjon, ved absorpsjon gjennom epitel eller mukokutane belegg (for eksempel oral slimhinnen, rektal og intestinal slimhinne, osv.) og kan administreres sammen med andre biologisk aktive midler. Administreringen kan være systemisk eller lokal. Forskjellige avleveringssystemer er kjente, for eksempel innkapsling i liposomer, mikropartikler, mikrokapsler, kapsler osv. kan anvendes for å administrere (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav. I visse utførelsesformer kan foreliggende preparater omfatte (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og/eller et eller flere farmasøytisk akseptable salter derav.
Fremgangsmåter for administrering omfatter intraderrnal, intramuskulær, intraperitoneal, intravenøs, subkutan, intranasal, epidural, oral, sublingual, intracerebral, intravaginal, transdermal, rektal, ved inhalering eller topisk, spesielt til ørene, nese, øyne eller hud. Fremgangsmåten for administrering velges av den medisinske behandleren. I de fleste tilfeller vil administrering resultere i frigivelsen av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav i blodstrømmen.
I spesifikke utførelsesformer kan det være ønskelig å administrere (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav lokalt. Dette kan for eksempel oppnås ved lokal infusjon under kirurgi, topisk tilførsel, for eksempel i forbindelse med en sårbandasje etter kirurgi, ved injeksjon ved hjelp av et kateter, ved hjelp av en suppositorie eller ved hjelp av et implantat, idet nevnte implantat er et porøst, ikke-porøst eller gelatinformig materiale, innbefattende membraner så som sialastiske membraner eller fibere.
I visse utførelsesformer kan det være ønskelig å innføre (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav i sentralnervesystemet ved en hvilken som helst fremgangsmåte, innbefattende intraventrikulær, intratekal og epidural injeksjon. Intraventrikulær injeksjon kan lettes ved hjelp av et intraventrikulært kateter, for eksempel knyttet til et reservoar, så som et Ommaya-reservoar.
Pulmonal administrering kan også anvendes, for eksempel ved anvendelse av en inhalator eller forstøvningsinnretning, og formulering med et aerosoliserende middel, eller via perfusjon i en fluorkarbon eller syntetisk pulmonal surfaktant. I visse utførelsesformer kan (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav formuleres som et suppositorium, med tradisjonelle bindemidler og bærere, så som triglyserider.

I nok en utførelsesform kan (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav avleveres i et system med kontrollert frigivelse (se for eksempel Goodson i Medical Applications of Controlled Release, supra, bind. 2, s. 115-138 (1984)). Andre systemer med kontrollert frigivelse diskutert i oversikten til Langer, 1990, Science 249:1527-1533 kan anvendes. I nok en utførelsesform kan et system med kontrollert frigivelse plasseres i nærheten av et mål for (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav; for eksempel ryggmargen eller hjernen, derved kreves bare en fraksjon av den systemiske dosen.
De oppnådde preparater kan eventuelt omfatte en egnet mengde av en farmasøytisk akseptabel bærer for å tilveiebringe formen for egnet administrering til pasienten.
I en spesifikk utførelsesform betyr betegnelsen "farmasøytisk akseptabel" godkjent av en regulerende myndighet på føderalt eller statlig nivå eller oppført i US Pharmacopeia eller generelt anerkjente farmakopeia for anvendelse i dyr, pattedyr og mer spesielt mennesker. De farmasøytiske vehiklene kan være saltvann, akasiegummi, gelatin, stivelsespasta, talk, keratin, kolloidalt silisiumoksid, urea og lignende. I tillegg kan hjelpestoffer, stabiliserende, fortykkende, smørende og fargende midler anvendes. Når de administreres til en pasient er de farmasøytisk akseptable vehiklene fortrinnsvis sterile. Vann er en foretrukket vehikkel når forbindelsen som anvendes ifølge oppfinnelsen administreres intravenøst. Saltvannsoppløsning og vandige dekstrose- og glyseroloppløsninger kan også anvendes som flytende vehikler, spesielt for injiserbare oppløsninger. Egnede farmasøytiske vehikler omfatter også hjelpestoffer så som stivelse, glukose, laktose, sukrose, gelatin, malt, ris, mel, kritt, silikagel, natriumstearat, glyserolmonostearat, talk, natriumklorid, tørket skummet melk, glyserol, propylen, glykol, vann, etanol og lignende. Preparatene kan om ønsket også inneholde mindre mengder av fuktende eller emulgerende midler, eller pH-bufrende midler.
De oppnådde preparatene kan anta form av oppløsninger, suspensjoner, emulsjoner,
tabletter, piller, pellets, kapsler, kapsler inneholdende væske, pulvere, preparater med langvarig frigivelse, suppositorier, emulsjoner, aerosoler, sprayer, suspensjoner eller en hvilken som helst annen form egnet for anvendelse. I en utførelsesform er den farmasøytisk akseptable vehikkel en kapsel (se for eksempel US patent nr. 5,698,155). Andre eksempler på egnede farmasøytiske vehikler er beskrevet i Remington's Pharmaceutical Sciences, Alfonso R. Gennaro red., Mack Publishing Co. Easton, PA, 19. utg, 1995, s. 1447 til 1676.
På fordelaktig måte formuleres (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav i henhold til rutinemessige fremgangsmåter som et farmasøytisk preparat tilpasset for oral administrering til mennesker. Preparater for oral avlevering kan være i form av tabletter, pastiller, vandige eller oljeformige suspensjoner, korn, pulvere, emulsjoner, kapsler, siruper eller eleksirer. Oralt administrerte preparater kan inneholde et eller flere midler, for eksempel søtningsmidler så som fruktose, aspartam eller sakkarin; smaksgivende midler så som peppermynte, olje av vintergrønt eller kirsebær, fargende midler og konserverende midler for å tilveiebringe et farmasøytisk smakelig preparat. Videre kan preparatene når de er i pille-eller tablettform belegges for å forsinke desintegrering og absorpsjon i gastrointestinalkanalen, for derved å tilveiebringe en vedvarende virkning over et forlenget tidsrom. Selektive permeable membraner som omgir en osmotisk aktiv drivende forbindelse er også egnede for preparater som administreres oralt. I disse senere platformene absorberes fluid fra miljøet som omgir kapselen av den drivende forbindelsen, som sveller for å fortrenge middelet eller sammensetningen av middelet gjennom en åpning. Disse avleveringsplatformene kan tilveiebringe en i det vesentlige nullteordens avleveringsprofil i motsetning til de toppede profilene for preparater med umiddelbar frigivelse. Et tidsforsinkende materiale, så som glyserolmonostearat eller glyserolstearat kan også anvendes. Orale preparater kan omfatte standardvehikler, så som mannitol, laktose, stivelse, magnesiumstearat, natriumsakkarin, cellulose- og magnesiumkarbonat. Slike vehikler er fortrinnsvis av farmasøytisk renhet. Typisk omfatter preparater for intravenøs administrering steril isotonisk vandig buffer. Om nødvendig kan preparatene også omfatte et solubiliserende middel.
I en annen mulig utførelse kan (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav formuleres for intravenøs administrering. Preparater for intravenøs administrering kan eventuelt omfatte et lokal-anestetisk middel så som lignokain for å redusere smerte ved injeksjonsstedet. Generelt tilføres bestanddelene enten separat eller blandet sammen i enhetsdoseform, for eksempel som et tørt lyofilisert pulver eller vannfritt konsentrat i en hermetisk forseglet beholder så som en ampulle eller en pose som indikerer mengden av aktivt middel. Når (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav skal administreres ved infusjon, kan det avleveres for eksempel med en infusjons flaske inneholdende sterilt vann eller saltvann av farmasøytisk kvalitet. Når (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav administreres ved injeksjon, kan en ampulle av sterilt vann for injeksjon eller saltvann tilveiebringes slik at bestanddelen kan blandes før administrering.
Mengden av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav som vil være effektiv ved behandlingen av en spesiell depresjonslidelse, angstlidelse, spiseforstyrrelse eller urininkontinens-relatert sykdom, vil avhenge av naturen av lidelsen eller tilstanden, og kan bestemmes ved standard kliniske teknikker. I tillegg kan eventuelt in vitro eller in vivo analyser anvendes for å hjelpe til å identifisere optimale doseringsområder. Den nøyaktige dosen som anvendes vil også avhenge av administreirngsmåten, og graden av sykdommen eller lidelsen, og bør avgjøres i henhold til bedømmelsen av en praktiserende lege og forholdene rundt hver pasient. Imidlertid er egnede doseringsområder for oral administrering generelt ca. 0,2 milligram til ca. 2,0 milligram av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav per kilo kroppsvekt per dag. Vanligvis er den orale dosen ca. 0,01 milligram til ca. 100 milligram per kilo kroppsvekt per dag, mer foretrukket ca. 0,1 milligram til ca. 75 milligram per kilo kroppsvekt per dag, mer foretrukket ca. 0,5 milligram til ca. 50 milligram per kilo kroppsvekt per dag, og enda mer foretrukket ca. 1 milligram til ca. 30 milligram per kilo kroppsvekt per dag. I en annen foretrukket utførelse er den orale dosen ca. 1 milligram til ca. 3 milligram (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav per kilo kroppsvekt per dag. Doseringsmengdene som heri er beskrevet refererer til samlede mengder administrert; dvs. dersom (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og/eller et eller flere farmasøytisk akseptable salter derav administreres, administreres foretrukne doser svarende til den samlede mengden. Orale preparater inneholder fortrinnsvis 10% til ca. 95 vekt% aktiv bestanddel.
Egnede doseringsområder for intravenøs (i.v.) administrering er ca. 0,01 milligram til ca. 100 milligram per kilo kroppsvekt per dag, ca. 0,1 milligram til ca. 35 milligram per kilo kroppsvekt per dag, og ca. 1 milligram til ca. 10 milligram per kilo kroppsvekt per dag. Egnede doseringsområder for intranasal administrering er generelt ca. 0,01 pg/kg kroppsvekt per dag til ca. 1 mg/kg kroppsvekt per dag. Suppositorier inneholder generelt ca. 0,01 milligram til ca. 50 milligram (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav per kilo kroppsvekt per dag, og omfatter aktiv bestanddel i området på ca. 0,5 til ca. 10 vekt%.
Anbefalte doseringer for intradermal, intramuskulær, intrapeirotoneal, subkutan, epidural, sublingual, intracerebral, intravaginal, transdermal administrering eller administrering ved inhalering er i området på ca. 0,001 milligram til ca. 200 milligram per kilo kroppsvekt per dag. Egnede doser for topisk administrering er i området på ca. 0,001 milligram til ca. 1 milligram, avhengig av administreirngsarealet. Effektive doser kan ekstrapoleres fra dose-responskurver avledet fra in vitro eller dyremodell testsystemer. Slike dyremodeller og systemer er velkjente innen teknikken.
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan og farmasøytisk akseptable salter derav analyserer fortrinnsvis in vitro eller in vivo, for den ønskede terapeutiske eller profylaktiske aktiviteten, før anvendelse i mennesker. For eksempel kan in vitro-analyser utføres for å bestemme om det er foretrukket å administrere (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav og/eller et annet terapeutisk middel. Dyremodellsystemer kan anvendes for å demonstrere sikkerhet og effektivitet.
Andre fremgangsmåter vil være kjente for fagmannen.
Kombinasjonterapi
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav kan tenkes anvendt i kombinasjonsterapi med minst et annet terapeutisk middel.
(+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav, og det andre farmasøytiske middelet kan virke additivt, eller mer foretrukket, synergistisk. I en mulig utførelsesform administreres et preparat omfattende (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav samtidig med administreringen av et annet terapeutisk middel, som kan være del av den samme sammensetningen som, eller en annen sammensetning fra den som, omfatter (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav. I et annet tilfelle administreres et preparat omfattende (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav før eller etter administrering av et annet terapeutisk middel. Siden mange av lidelsene for hvilket det er nyttig å anvende (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller farmasøytisk akseptable salter derav er kroniske kan behandlingen tenkes å omfatte alternering mellom administrering av et preparat omfattende (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav, og et preparat omfattende et annet terapeutisk middel. Varigheten av administreringen av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan, et farmasøytisk akseptabelt salt derav eller det
andre terapeutiske middelet kan være for eksempel en måned, tre måneder, seks måneder, 1 år eller i lengre perioder, så som pasientens levetid. Når et preparat oppnådd ifølge oppfinnelsen administreres samtidig med et annet terapeutisk middel som potensielt fremkaller uheldige bivirkninger omfattende toksisitet kan det andre terapeutiske middelet fordelaktig administreres ved en dose som faller under terskelen hvorved den uønskede bivirkningen fremkalles.
Det andre terapeutiske middelet kan være et antidepresjonsmiddel. Nyttige antidepresjonsmidler omfatter amitriptylin, klomipramin, doksepin, imipramin, trimipramin, amoksapin, desipramin, maprotilin, nortriptyliri, protripylin, fluoksetin, fluvoksamin, paroksetin, setralin, venlafaksin, bupropion, nefazodon, trazodon, feuelzin, tranylcypromin og selegilin.
Det andre terapeutiske middelet kan være et anksiolytisk middel. Nyttige anksiolytiske midler omfatter benzodiazepiner så som alprazolam, klordiazepoksid, klonazepam, klorazepat, diazepam, halazepam, lorazepam, oksazepam og prazepam; ikke-benzodiazpin midler så som buspiron; og beroligende midler så som barbiturater.
Det andre terapeutiske middelet kan være et antipsykotisk legemiddel. Nyttige antipsykotiske legemidler omfatter fenotiaziner, så som klorpromazin, mesoridazinbesylat, tioridazin, acetofenazinmaleat, flufenazin, perfenazin og trifluorperazin; tioxantener så som klorproksitin og tiotiksen; og andre heterocykliske forbindelser så som klozapin, haloperidol, loksapin, molindon, primozid og risperidon. Foretrukne anti-psykotiske legemidler omfatter klorpromasin HC1, tioridazin HC1, flufenazin HC1, tiotiksen HC1 og molindon HC1.
Det andre terapeutiske middelet kan være et anti-fedmemiddel. Anti-fedmemidler for anvendelse i kombinasjon med forbindelsene som anvendes ved oppfinnelsen omfatter P-adrenergiske reseptoragonister, fortrinnsvis P-3 reseptoragonister: fenfluramin, deksfenfluramin, sibutramin, bupropion, fluoksetin, fenetermin, amfetamin, metamfetamin, dekstroamfetamin, benzfetamin, fenedimetrazin, fehmetrazin, dietylpropion, mazindol og fenylpropanolamin.
1. Eksempel: (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan
1. 1 Oppløsnin<g> av racematet ved hjelp av kiral kromatografi Til 279 mg av (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksanhydroklorid oppnådd ved fremgangsmåtene beskrevet i Epstein et al., J. Med. Chem., 24:481-490 (1981) ble det tilsatt 7 ml av 9:1 heksan:isopropylalkohol, etterfulgt av 8 dråper dietylamin. Til den resulterende blandingen ble det dråpevis tilsatt isopropylalkohol inntil en oppløsning var oppnådd. Oppløsningen ble konsentrert til et volum på 6 ml ved anvendelse av en strøm av heliumgass, og 6 1-ml porsjoner av konsentratet ble underkastet høy-ytelsesvæskekromatografi ved anvendelse av et HPLC-instrument utstyrt med en 1 cm x 25 cm Daicel CHIRALPAD AD kolonne (Chiral Techologies, Inc., Exton, Pennsylvania). Eluering ble utført ved romtemperatur ved anvendelse av 95:5 (v/v) heksamisopropylalkoholoppløsning inneholdende 0,05% dietylamin som en mobil fase ved en strømningshastighet på 6 ml/min. Fraksjonen som eluerer ved ca. 21,5 til 26 minutter ble samlet og konsentrert for å tilveiebringe en første rest som ble oppløst i en minimal mengde etylacetat. Ved anvendelse av en strøm av nitrogen ble etylacetatoppløsningen fordampet for å tilveiebringe en andre rest, som ble oppløst i 1 ml dietyleter. En 2,68 g (0,0101 mmol) prøve av (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksanhydroklorid som beskrevet i Epstein et al., J. Med. Chem., 1981,24, s. 481-490, ble oppløst i 50 ml vann, og denne oppløsningen ble gjort basisk til pH 11 med 10N natriumhydroksidoppløsning, og den utfelte frie basen ble ekstrahert i 25 ml diklormetan. Denne oppløsningen ble tørket over natriumsulfat og filtrert. Til dette filtratet ble det tilsatt en oppløsning av 3,70 g (0,1030 mol) av L-di-(0-benzoyl)vinsyre i 25 ml metanol, og denne oppløsningen ble kokt inntil krystallisasjon foregikk. Blandingen ble avkjølt til romtemperatur og tillatt å stå i 1 time. 2. 1 Norepinefirn- transpotrer- bindingsanalvser
Anti-depresjonsegenskapene av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksanhydroklorid ble sammenlignet med anti-depresjonsegenskapene av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksanhydroklorid ved å anvende en standard norepinefirn-transporter-bindingsanalyse.
2. 1. 1. Materialer og fremgangsmåter
Norepinefrin-bindingsanalysen ble utført i henhold til fremgangsmåtene beskrevet i Raisman et al, Eur. J. Pharmacol. 78:345-351 (1982) og Langer et al., Eur. J. Pharmacol 72:423 (1981). Reseptorkilden var rotte-forhjernemembraner; radioligand var [<3>H]-nisoksetin (60-85 Ci/mmol) ved en endelig ligandkonsentrasjon på 1,0 nM; den ikke-spesifikke determinanten [1.0 um]; referanseforbindelse og positiv kontroll var
(D-desmetylimipramin HC1. (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan HC1 ble oppnådd i henhold til fremgangsmåten ifølge Eksempel 1 ovenfor. Reaksjoner ble utført i 50 mM Tris-HCl (pH 7,4) inneholdende 300 mM NaCl og 5 mM KC1 ved 0°C til 4°C i 4 timer. Reaksjonen ble terminert ved rask vakuumfiltrering på glassfiberfllteret.

Data i Tabell 1 viser at (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan HC1 har en signifikant større affinitet for norepinefrin-opptakssetet enn (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan HC1. Vellykket inhibering av norepinefrin-gjenopptak har vært assosiert med behandlingen av et eller flere av symptomene på depresjon (RJ. Baldessarini, Drugs and the Treatment of Psychiatric Disorders: Depression and Mania in Goddman & Gilman's The Pharmacological Basis of Therpeutics 431-459 (9. utg. 1996)). Derfor vil (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav være signifikant mer aktivt enn (±)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav for behandling av, eller forebyggelse av tilbakefall til, depresjon i en pasient.
2. 2 Serotonin- transpotrer- bindingsanalvse
Anti-depresjonsegenskapene av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksanhydroklorid ble sammenlignet med anti-depresjonsegenskapene av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksanhydroklorid ved å anvende en standard serotonin-transporter-bindingsanalyse.
2. 2. 1 Materialer og fremgangsmåter
Serotonin-bindingsanalysen ble utført i henhold til fremgangsmåten beskrevet i D'Amato et al., J- Pharmacol. Exp. Ther. 242:364-371 (1987) og Brown et al., Eur. J. Pharmac. 123:161-165 (1986). Reseptorkilden var rotte-forhjernemembraner; radioliganden var [<3>H]-citalopram (70-87 Ci/mmol) med en endelig ligandkonsentrasjon på 0,7 nM; den ikke-spesifikke determinanten var klomipramin (10 um); og referanseforbindelsen og positiv kontroll var (+)-desmetylimipramin. (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan HCL ble oppnådd i henhold til fremgangsmåten ifølge Eksempel 1 ovenfor. Reaksjoner ble utført i 50 mM Tris-HCl (pH 7,4) inneholdende 120 mM NaCl og 5 mM KC1 ved 25°C i 60 minutter. Reaksjonen var blitt terminert ved rask vakuumfiltrering på glassfiberfiltere. Claims (24)
1.
Anvendelse av (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav som hver er i det vesentlige fri for den tilsvarende (-)-enantiomeren, for fremstilling av et medikament for behandling av, eller forebyggelse av tilbakefall ved, depresjon i en pasient.

Anvendelse ifølge krav 1, hvorved (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller et farmasøytisk akseptabelt salt derav ikke har mer enn ca. 2% v/v av den tilsvarende (-)-enantiomeren.

Anvendelse ifølge krav 1, hvorved (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller farmasøytisk akseptabelt salt derav ikke har mer enn ca. 1% v/v av den tilsvarende (-)-enantiomeren.

Anvendelse ifølge krav 7, hvor (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller det farmasøytisk akseptable salt derav ikke har mer enn ca. 2% v/v av den tilsvarende (-)-enantiomeren.

Anvendelse ifølge krav 7, hvor (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller farmasøytisk akseptable salt derav ikke har mer enn ca. 1% v/v av den tilsvarende (-)-enantiomeren.

Anvendelse ifølge krav 7, hvor angstlidelsene er valgt fra gruppen bestående av panikklidelse, panikkanfall, agorafobi, generalisert angstlidelse, obsessiv-kompulsiv lidelse, fobiske lidelser og post-traumatiske stresslidelser.

Anvendelse ifølge krav 8, hvor angstlidelsen er valgt fra gruppen bestående av panikklidelse, panikkanfall, agorafobi, generalisert angstlidelse, obsessiv-kompulsiv lidelse, fobiske lidelser og post-traumatiske stresslidelser.

Anvendelse ifølge krav 13, hvorved (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller farmasøytisk akseptable salt derav ikke har mer enn ca. 2% v/v av den tilsvarende (-)-enantiomeren.

Anvendelse ifølge krav 13, hvorved (+)-l-(3,4-diklorfenyl)-3-azabicyklo[3.1.0]heksan eller farmasøytisk akseptable salt derav ikke har mer enn ca. 1% v/v av den tilsvarende (-)-enantiomeren.

Anvendelse ifølge krav 13, hvor spiseforstyrrelsen er valgt fra gruppen bestående av anoreksia nervosa, overspising, bulimia nervosa-ikke-tømmende type og bulimia nervosa-tømmende type.

Anvendelse ifølge krav 14, hvor spiseforstyrrelsen er valgt fra gruppen bestående av anoreksia nervosa, overspising, bulimia nervosa-ikke-tømmende type og bulimia nervosa-tømmende type.
