Description
Foreliggende oppfinnelse vedrører anvendelse av 5'-substituerte nukleosider.
Forekomsten av "legemiddelresistens" er hovedgrunnen for mis-lykkede behandlinger ved kreft-kjemoterapi. Tumorer som til å begynne med reagerer merkbart på cytostatika vil meget hyppig komme seg igjen etter en viss behandlingstid og er da resistente mot virkningen av forskjelligartede anti-neoplastiske legemidler. Selv om problemet med "legemiddelresistens" har vært kjent siden 1948 da man begynte med kreft-terapi med cytostatika, er det hittil ikke funnet noen måte til å forhindre resistensdannelsen. Karakteristisk for alle hittil undersøkte høyresistente tumorer og cellelinjer er amplifikasjonen (mangfoldiggjøringen) av en liten gruppe gener. Som følge av denne DNA- eller genamplifikasjon kan det påvises en forhøyet ekspresjon av disse gener som fører til resistens overfor legemiddelet. Som følge av denne DNA-amplifikasjon kan det påvises en forhøyet ekspresjon av forskjellige gener. Beskyttelsesproteiner som tjener til å fjerne giftstoffer fra cellen, kan således dannes i økt grad (P-glykoprotein). Denne virkning betegnes også som "flermedikamentresistens" ("multi-drug-resistance" eller
MDR) .
I tillegg til MDR samsvarer amplifikasjonsgraden av visse gener, fremfor alt visse onkogener, med graden av malignitet. Således er overlevelsessjansene ved en amplifikasjon av ERW2, RASKI, INT3, HST1, MYC og KS RAM ved magekreft (Hirohasi, S., og Sugimura, T. "Genetic alterations in human gastric cancer", Cancer cells (Cold Spring Habor), 3:49-52, 1991) og av ERBB2 og MYC ved ovarialkarsinom meget dårlige.

Ved brystkreft samsvarer amplifikasjonen av MYC {Borg, A., Baldetorp, B., Ferno, M., Olsson, H., og Sigurdsson, H. "C-myc amplification is an independent prognostic factor in postmenopausal breast cancer", Int. J. Cancer, 51:687-691, 1992) og ko-amplifikasjon av INT2 og HST1
med sykdomsforløpet (Borg, A., Sigurdsson, H., Clark, G.M. et al., "Association of INT2/HST1 coamplification in primary breast cancer with hormone-dependent phenotype and poor prognosis", Br. J. Cancer, 63:136-142, 1991). Amplifikasjonen av ERBB2 (Descotes, F., Pavy, J.-J., og Adessi, G.L., "Human breast cancer: Correlation study between HER-2/neu amplification and prognostic factors in an unselected population", Anticancer Res., 13:119-124, 1993) og av EGFR er forbundet med et dårlig sykdomsforløp (Klijn, J.G.M., Berns, P.M.J.J., Schmitz, P.I.M. og Foekens, J.A. "The clinical significance of epidermal growth factor receptor (EGF-R) in human breast cancer: a review on 5232 patients", Endocr. Rev., 13:3-17, 1992). Ved øsofaguskarsinom samsvarer ko-amplifikasjonen av HST1 og INT2 med graden av metastasering (Tsuda, T., Tahara, E., Kajiyama, G., Sakamoto, H. Terada, M. og Sugimura, T., "High incidence of coamplification of hst-1 and int-2 genes in human esophageal carcinomas", Cancer Res. 49:5505-5508, 1989).
Sammenfattet kan det konstateres at den ved kronisk behandling med cancerogene cytostatika induserte genamplifikasjon ikke bare fører til resistens overfor denne behandling, men også til overekspresjon av visse onkogener som kontrollerer graden av malignitet.
Det er altså beskrevet en rekke substanser som motvirker den ervervede legemiddelresistens. Hertil hører de av Kennedy {Kennedy, A.R., "Prevention of Carcinogenesis by Protease Inhibitors", Cancer Res., 54:1999-2005, 1994) beskrevne arbeider over de anti-cancerogene virkninger av proteaseinhibitorer. Disse kan undertrykke cancerogenindusert gen-amplif ikasjon til det nesten normale nivå. Kennedy iakttok at bestrålingsindusert genamplifikasjon undertrykkes på samme måte som tilsvarer deres evne til å undertrykke bestrålingsindusert transformasjon slik at det kunne trekkes en slutning angående en sammenheng mellom de to prosesser. Utover dette virker proteaseinhibitorer som antagonister for (ko-rekombinogene) tumor-promotorer ved utløsning av transformasjon in vitro. Proteaseinhibitorer beskrives også som virkningsfulle anti-promotorer i in vivo undersøkelser (Troll, W., Klassen, A. og Janoff, A. "Tumorigenesis in mouse skin: inhibition by synthetic inhibitors■of proteases", Science (Washington DC), 169:1211-1213, 1970).
Fra litteraturstedet Moscow, I. A., og Cowan, K. H., "Multi-drug resistance", J. Nati. Cancer. Inst. 80:14-20, 1988, er det kjent at Verapamil motvirker MDR. Denne "kalsiumkanal-blokker" forhøyer cytotoksisiteten ved forhøyelse av den intracellulære akkumulasjon av legemidler. Sannsynlig skjer dette ved en virkning på P-glykoproteinet eller andre trans-portproteiner. Toksisiteten av denne og lignende substanser som f.eks. kinidin, står i veien for deres kliniske anvendelse .
Ut fra dette er det et formål for den foreliggende oppfinnelse å angi en virksom substans for å forhindre, henholdsvis redusere, resistensdannelse overfor cytostatikabehandling.
Foreliggende oppfinnelse vedrører således anvendelse av (E) - 5-(2-bromvinyl)-2'-deoksyuridin (BVDU),(E)-5-(2-bromvinyl)-l-p-D-arabinofuranosyluracil,(E)-5-(2-bromvinyl)-2'-deoksy-4'-tiouridin, 5-jod-2•-deoksycytidin, 5-jod-2<1->deoksyuridin, 2'-deoksy-5-trifluormetyluridin■ i kombinasjon med minst ett cytostatikum for fremstilling av et legemiddel for å forhindre eller redusere resistensdannelse ved cytostatikabehandling.
Det har overraskende vist seg at de ovennevnte 5<1->nukleosider forhindrer eller i det minste svekker utvikling av cancerogenindusert genamplifikasjon. Derved åpnes muligheten for at man ved samtidig tilførsel av disse nukleosider med et cytostatikum kan hindre at det oppstår resistens."overfor disse legemidler og at også graden av malignitet kan påvirkes.
De ovennevnte 5 *-nukleosidene kan inneholdes i legemiddelet i en mengde som fører til at det i blodet oppnås en konsen-tras jon fra 0,02 /ig/ml til 10 fig/ ml. I forsøk kan det vises at det optimale område ligger ved 0,05 fig/ ml til 5 fig/ ml.
Ett eller flere cytostatika kan da være tilstede i legemiddel sammensetningen i de vanlige hittil kjente mengder (Oshiro, Y., Piper, C.E., Balwierz, P.S. og Garriot, M.L.
(1992) "Genotoxic properties of (E)-5-(2-bromvinyl-2'-deoksyuridin (BVDU)", Fundamental arid Applied Toxicology, 18, 491-498) . Eksempler på cytostatika er cyklofosfamid og andre alkylerende midler, anti-metabolitter som metotrexat, alkalo-ider som vinblastin, antibiotika som bleomycin, cisplatin såvel som andre stoffer. Ytterligere eksempler på cytostatika kan tas fra "Rote Liste 1995", utgitt på "Cantor Verlag fiir Medizin und Naturwissenschaften", Aulendorf/Wiirtt., 1995.
Bærerstoffene, tilsetningsstoffene og hjelpestoffene tilsvarer dem som til nå har vært kjent fra teknikkens stand. Legemiddelet kan derved foreligge i fast eller flytende form eller også som spray-preparat.
Oppfinnelsen skal i det følgende forklares nærmere ved hjelp av modellforsøk.
A. Modellsubstanser
For undersøkelsen av amplifikasjonsfenomener anvendes en hamstercellelinje som inneholder et virus (Simian virus 40) i genomet. For denne cellelinje er det kjent at en tilførsel av genotoksiske substanser, men også forskjellige "ikke-genotoksiske" cancerogener og tumor-promotorer, fører til amplifikasjon av SV40-DNA i hamstergenomet. Metoden baseres på at merket SV4 0-DNA som tjener som probe hybridiseres med hamstercelle-DNA som inneholder SV40-DNA i amplifisert kopi-antall. Mengden av bundet probe gir informasjon om amplifikasjonsgraden av det integrerte virus-DNA.
For bestemmelse av amplifikasjonsgraden måles samtidig med målingen av SV4 0-DNA albumingen-DNA. Albumingenet blir nemlig i motsetning til SV40-DNA, ikke amplifisert i cellen. Verdien av det relative SV40-DNA innhold fås deretter fra forholdet mellom signalet for den med SV4 0-DNA hybridiserte" DNA-probe og signalet for den med albumingen hybridiserte DNA-probe fra de samme SV4 0-transformerte embryonale C0631 hamsterceller.
Som modellsubstanser tjente:
1. Mutagene og rekombinogene genotoksiske cancerogener (positiv kontroll)
Trietylenmelamin (TEM)
4-nitrokinolin-l-oksyd (4-NQO)
2. Ikke-cancerogener (negativ kontroll) som hverken induserer mutasjoner eller rekombinasjoner

12-0-tetradekanoyl-forbol-13-acetat (TPA)

Virkningen av de ovennevnte modellsubstanser alene eller i kombinasjon med et cancerogen ble undersøkt i genamplifika-sj onssystemet.
Resultatene med mode11substansene er sammenfattet i fig. l til 3.
Det ikke-cancerogene aceton og dimetylformamid viser ingen virkning.
Alle andre substanser, de ikke-genotoksiske cancerogener med ukjent virkningsmekanisme, tioacetamid og acetamid, de genotoksiske cancerogener TEM og 4-NQO såvel som tumor-promoterene Mezerein, 12-0-tetradekanoyl-forbol-13-acetat (TPA) , chrysarobin og kumarin øker hver for seg SV40-gen-amplif ikas j onen.
Testosteron med S9-Mix nedsetter den amplifiserende virkning av metotrexat (MTX), uten S9-Mix forhøyer det den amplifiserende virkning av amino-6-merkaptopurin (AMP).
Disse resultater viser en overensstemmelse mellom utløsningen av rekombinasjon og SV40-genamplifikasjon.
B. Hemming av cancerogenindusert genamplifikasjon med ( E)- S-( 2- bromvinyl)- 2'- deoksyuridin f BVDU)
Resultatene er sammenfattet i fig. 4 til 7.
BVDU hadde i forsøket med gjær (fig. 4) en anti-rekombinogen og ko-mutagen virkning. Denne virkning var i nærvær av levermikrosomer (S9-Mix) i lavere konsentrasjoner mer synlig enn i fravær av S9-Mix og også mye mer utpreget. En metabolisering av BVDU forsterker altså den anti-rekombinogene virkning.
BDVU viser i klinisk relevante doser en hemming av AMP-indusert genamplifikasjon. Virkningen begynner ved omtrent 0,05 ug/ ral og fører doseavhengig ved 5 fig/ ml til en'fullstendig hemming av den AMP-induserte genamplifikasjon (fig. 5). Det uavhengige gjentagelsesforsøk bekrefter resultatet (fig. 6)". Utover dette synes BVDU alene å nedsette den spontane amplifikasjonsgrad lett.
Tilsetning av S9-Mix viser likeledes en nedsettelse av AMP-indusert genamplifikasjon. Denne skjer ganske visst i et lavere doseområde enn i forsøkene uten S9-Mix. En mulig metabolisering av BVDU synes altså ytterligere å forsterke den amplifikasjonshemmende virkning (fig. 7). Dette ville ytterligere kunne forsterke relevansen av resultatene.
Sammenfattende kan det sies at BVDU hemmer cancerogenindusert genamplifikasjon. Dette åpner den mulighet at man med samtidig tilførsel av BVDU med et cytostatikum kan forhindre at det oppstår resistenser overfor dette legemiddel og at maligniteten kan nedsettes.
C. Forhindring av dannelse av "multi-leaemid del- resistens" fMDR) i humane og animalske tumorceller overfor cytostatikabehandling ved samtidig tilførsel av BVDU
Den humane tumorcellelinje K562-WT (fig. 8) og tutnorcelle-1injen F4 6-WT fra mus (fig. 9) (WT = villtype = følsom overfor cytostatikabehandling = ingen amplifikasjon av MDR-genet) ble behandlet i flere uker med gradvis økende konsentrasjoner av adriamycin. I løpet av behandlingen ervervet cellene en resistens overfor denne behandling. Hvis 20 ng/ml adriamycin ved en behandlingstid av 4 dager virker sterkt toksisk overfor ikke-resistente celler, så er cellene etter langtidsbehandling med gradvis økende konsentrasjoner blitt fullstendig uømfintlige overfor 2 0 ng/ml adriamycin (fig. 8 og 9). Resistensdannelsen beror på amplifikasjonen av MDR-genet. Dette ble påvist ved hjelp av "Northern-Technik", en metode for påvisning av RNA-molekyler, altså transkripsjon av et gen under anvendelse av MDR-genet som probe (fig. 10). Resistente celler fremviser et bånd mens ikke-resistente celler (tilstanden ved begynnelsen av behandlingen) ikke fremviser noen bånd.
I parallellforsøk ble adriamycin tilført sammen med enten 0,5 eller 1 jig/ml BVDU (BVDU virker, i humane tumorceller, giftig først fra omtrent 10 fig/ ml og i celler fra mus fra omtrent 8 jig/ml) (fig. 8 og 9) . BVDU forhindrer resistensdannelsen overfor adriamycin. Tumorcellene blir ømfintlige overfor cytostatikabehandlingen og dør ut. Virkningen av BVDU er så sterk at behandlingen må avbrytes med hvilepauser (vekst uten substanser) slik at forsøket dermed strekker seg over 6 til 8 uker.
BVDU + adriamycin behandling fører til en vesentlig svakere amplifikasjon av MDR-genet enn adriamycinbehandling alene (fig. 10). Virkningen av BVDU-behandlingen er i realiteten ennå mye større enn det svekkingen av båndene viser. Ved slutten av behandlingen blir nemlig bare celler tilbake som i det minste har oppnådd en viss resistens overfor adriamycin-behandlingen. De celler som, som følge av BVDU-behandlingen, er forblitt ikke-resistente, er da allerede døde. Den egent-lig relevante og ved hjelp av Northern-metoden ikke forståelige virkning består derfor i at de ikke-resistente celler, som måles i inaktiveringskurvene (fig. 8 og 9), dør bort.
Da resistensdannelsen overfor cytostatikabehandling i humane tumorer likeledes beror på amplifikasjon av MDR-genet, er den mulighet tilveiebragt at man ved kombinasjon av BVDU med et vilkårlig valgt cytostatikum kan gjennomføre terapi.i lavere doser og over lengre tidsrom enn tidligere. Forøvrig er for-hindringen av resistensdannelsen av stor betydning også for andre anvendelser.
D. Forhindring av dannelse av " multi- medikament- resistens" ( MDR) i tumorceller overfor cytostatikabehandling ved samtidig tilsetning av anti- rekombinogene 5'- substituerte nukleosider
Fra fig. 11 og 12 kan det sees at den anti-rekombinogene virkning ikke bare gjelder spesifikt for BVDU, men er også en egenskap ved alle 5'-substituerte nukleosider. Fig. 11 viser derved den anti-rekombinogene virkning av (E)-5-(2-bromvinyl)-2'-deoksyuridin, (E)-5-(2-bromvinyl) -1-p-D-arabinofuranosyluracil og (E)-5-(2-bromvinyl-2'-deoksy-4'-tiouridin i Saccharomyces cerevisiae MP1, og Fig. 12 viser den anti-rekombinogene virkning av S-jod-2'-deoksycytidin, 5-jod-2<1->deoksyuridin og 2'-deoksy-5-trifluormetyluridin i Saccharomyces cereviciae MPl.
Tumorcellelinjen F4-6-WT fra mus (WT = villtype = ømfintlig overfor cytostatikabehandling = ingen amplifikasjon av MDR-genet) ble behandlet over flere uker med gradvis økende konsentrasjoner av adriamycin. I løpet av behandlingen ervervet cellene en resistens overfor denne behandling. Hvis 2 0 ng/ml adriamycin ved en behandlingstid på 4 dager virker sterkt toksisk overfor ikke-resistente celler, så er cellene etter langtidsbehandling med gradvis økende konsentrasjoner blitt fullstendig uømfintlige overfor 20 ng/ml adriamycin (fig. 13 og 14). Resistensdannelsen beror på amplifikasjon av MDR-genet. Dette ble påvist ved hjelp av Northern-metoden, en metode for påvisning av RNA molekyler, altså transkripsjon av et gen under anvendelse av MDR-genet som probe (fig. 15). Resistente celler fremviser et bånd, mens ikke-resistente celler (tilstand ved begynnelse av behandlingen) ikke fremviser noen bånd. Nivået av p-aktin RNA ble også sammenligningsvis analysert.
P-aktin ble benyttet som intern kontroll for RNA-mengden.
I parallellforsøk ble adriamycin tilført sammen med 1 jig/ml av hvert 51-substituerte nukleosid. Alle seks 5'-substituerte nukleosider forhindrer resistensdannelsen overfor adriamycin. Tumorcellene blir ømfintlige overfor cytostatikabehandlingen og dør ut. Virkningen av de 5'-substituerte nukleosider var så sterk at behandlingen måtte avbrytes med hvilepauser (vekst uten substanser) slik at forsøket dermed strakk seg over 6 til 8 uker.
Fig. 15 viser "Northern Blot" analyse av RNA: ekspresjon av MDR-genet i tumorcellelinjen F4-6-WT fra mus.

Behandling med 5<1->substituert nukleosid + adriamycin fører til en vesentlig svakere amplifikasjon av MDR-genet enn adriamycinbehandling alene (fig. 15). Virkningen av behandlingen er i virkeligheten ennå mye større enn det som svekkingen av båndene uttrykker. Ved' slutten av behandlingen blir nemlig bare celler tilbake som i det minste har ervervet en <y>iss resistens overfor adriamycin-behandlingen. De celler som, som følge av BVDU-behandlingen, er forblitt ikke-resistente er jo alle allerede tidligere dødd ut. Den egentlige relevante og ved hjelp av Northern-metoden ikke forståelige virkning består derfor i at de ikke-resistente celler, som ble målt i inaktiveringskurvene (fig. 13 og 14), er dødd ut.

1. Anvendelse av (E)-5-(2-bromvinyl)-2<1->deoksyuridin (BVDU),(E)-5-(2-bromvinyl)-1-p-D-arabinofuranosyluracil,(E)-5-(2-bromvinyl)-2■-deoksy-4<1->tiouridin, 5-jod-2'-deoksycytidin, 5-jod-21-deoksyuridin, 2'-deoksy-5-trifluormetyluridin i kombinasjon med minst ett cytostatikum for fremstilling av
et legemiddel for å forhindre eller redusere resistensdannelse ved cytostatikabehandling.
2. Anvendelse som angitt i krav l, hvor (E)-5-(2-bromvinyl)-2'-deoksyuridin (BVDU) anvendes.
